Background/aim: Although the frequency of small bowel obstructions after liver surgery is generally considered low, previous studies have followed-up patients for less than a year, thus the incidence of small bowel obstructions several years after surgery is unknown. Furthermore, the rise in laparoscopic surgeries and the use of adhesion prevention materials may influence the occurrence of small bowel obstructions. This  study aimed to assess the incidence of small bowel obstructions within a five-year period following liver surgery and identify the associated risk factors.

Article Synopsis
  • The study investigates the impact of postoperative oral nutritional supplementation (ONS) on body weight loss and malnutrition, specifically in patients aged 80 and older who underwent oncological gastrectomy.
  • Among the 286 patients studied, it was found that octogenarians who received ONS showed significant improvements in body weight and muscle mass compared to those who did not receive ONS, while no such benefits were observed in younger patients.
  • The findings suggest that ONS is effective in preventing malnutrition and muscle loss in older patients post-gastrectomy, highlighting its importance for this age group.

  • Minimal change disease (MCD) is a leading cause of nephrotic syndrome, making early and accurate diagnosis crucial, yet it typically requires a kidney biopsy for confirmation.
  • This study explores the use of machine learning algorithms to create non-invasive models for diagnosing MCD by analyzing data from 248 patients with nephrotic syndrome.
  • Among the algorithms tested, TabPFN performed the best, achieving high accuracy metrics and identifying key predictors like C3, total cholesterol, and urine red blood cells, suggesting that machine learning could effectively aid in MCD diagnosis.

  • Nonocclusive mesenteric ischemia (NOMI) involves intestinal ischemia from spasms in small blood vessels, which can become life-threatening if not diagnosed and treated quickly.
  • In this case, an 81-year-old man was diagnosed with NOMI after imaging showed signs of intestinal distress, leading to emergency surgery where ICG fluorescence was used to assess blood flow in the intestines.
  • The use of ICG fluorescence allowed for the precise removal of ischemic areas, improving outcomes and suggesting that more research on this technique could enhance future diagnostics and treatment of NOMI.

Esophageal diverticula are relatively uncommon, especially supradiaphragmatic diverticula. Esophageal diverticula are normally managed by observation; however, surgical treatment is sometimes indicated for large diverticula or diverticula in highly symptomatic patients. Surgical approaches for esophageal diverticula include thoracoscopic or laparoscopic resection; however, consensus has not yet been reached on the optimal approach.

The prognosis of anti-glomerular basement membrane (anti-GBM) nephritis, often accompanied by the presence of antineutrophil cytoplasmic antibodies (ANCA), is poor, and even with aggressive therapeutic approaches, kidney replacement therapy (KRT) is typically required. Here, we present a case of necrotizing crescentic glomerulonephritis in a patient double-seropositive for anti-GBM antibodies and ANCA who successfully achieved dialysis independence following aggressive treatment, including avacopan. The patient was a 77-year-old woman with rapidly progressive glomerulonephritis and double seropositivity for myeloperoxidase-ANCA and anti-GBM antibodies.

Background: Postoperative pneumonia in patients with esophageal cancer occurs due to swallowing dysfunction and aspiration. Recently, maximum phonation time (MPT) assessment and repetitive saliva swallowing test (RSST) have been focused on as swallowing function assessment methods that can identify patients as high risk for pneumonia. We aimed to evaluate the clinical utility of MPT assessment and RSST in patients undergoing oncological esophagectomy.

Background: Autosomal dominant polycystic kidney disease (ADPKD) and autosomal recessive polycystic kidney disease (ARPKD) are major genetic polycystic kidney diseases that can progress to end-stage kidney disease (ESKD). Longitudinal data on the clinical characteristics associated with clinical outcomes in polycystic kidney disease (PKD), including the development of ESKD and cardiovascular disease (CVD) are lacking in Japan. To address this unmet need the authors are establishing a novel, web-based, Nationwide Cohort Registry Study-the Japanese Registry of PKD (JRP).
