The results of histological investigation of musculo-aponeurotic tissues of lumbar-lateral abdominal portion were analyzed. There was established, that relaxation and atrophy of these tissues had occurred due to nervous structures and vessels cutting with consequent development of postoperative lumbar-lateral hernias. The plastic material fixation to the bearing structures of lumbar-lateral portion of abdominal wall, specifically to posterior wall of m.

In 75 patients, suffering unilateral inguinal hernia and destroyed inguinal ligament, the preperitoneal alloplasty method, elaborated in the clinic, was applied, using operative access through inguinal channel with prolene mesh fixation to lacunar ligament. In 18 patients the mesh was sutured, using herniostapler with spiral-like fixation devices.

In 21 patients, suffering primary inguinal hernia recurrence, in concurrence with the connective tissue complex degeneration the pronounced atrophy of tissues was revealed. In 23 patients, suffering multiple inguinal hernia recurrence, the tissues degeneration and atrophy were manifested significantly.

Application of net of polypropylene and the PHS system allows to improve in patients the surgical treatment result significantly because of reliable strengthening of inguinal canal fundus without tension of tissues. Early postoperative rehabilitation and the lowering of recurrences occurrence justify significant advantages of such hernioplasty over autoplastic methods and its wide application in surgical treatment of primary inguinal hernia.

The method stipulates the prosthesis of polyurethane placement in the duplicature from the hernial sac and parietal peritoneum and is sewn under the hernial defect edge, which are then sewn contactly. With the application of method elaborated 36 patients were operated on, suffering from large abdominal hernia.

According to electromyographic and histological investigations of inguinal region the atrophy degree was established: common atrophy was diagnosed if the muscles bioelectric activity constituted 25-40 mcV, profound one--10-25 mcV. Autoplastic operation according to Bassini-Postempsky was done in 48 patients with common muscles atrophy, according to Sholdis-in 48, in 38 patients with profound atrophy of muscles the combined inguinal channel plasty with the implant of polyuretane preperitoneal disposition was done.
