Against the background of a large-scale modernization of primary health care, introduction of new treatment and diagnostic methods and clinical guidelines, primary care physicians have an increasing need to improve professional skills, expand horizons and adapt to a rapidly changing reality, requiring the development of meta-skills, for example, critical thinking and research competencies. The purpose of the study was to assess the possibility of developing meta-skills among primary care providers through an educational project exemplified by the "Scientific Laboratory: Moscow Polyclinic" urban project.

Primary hyperparathyroidism (PHPT) is an endocrine disorder of parathyroid glands characterized by excessive secretion of parathyroid hormone (PTH) with an upper normal or elevated blood calcium level. Classical PHPT refers to a symptomatic, multi-system disorder, wich can lead to a significant decrease in the quality of life, disability of patients, and even an increased risk of premature death. Hypercalcemia and the catabolic effect of PTH on various cells are considered as the main pathogenetic mechanisms of the PHPT associated complications.

Hypoparathyroidism is a rare disorder characterized by the absent or inappropriately decreased serum parathyroid hormone in the parathyroid glands, which is accompanied by impaired calcium-phosphorus metabolism.The main etiology of hypoparathyroidism remains damage or removal of the parathyroid glands during neck surgery. In view of the incidence of thyroid cancer, primary hyperparathyroidism and other pathologies of the neck organs, which radical treatment can lead to the parathyroid gland impairment, an increased number of patients with hypoparathyroidism is expected.

The strategy of starting treatment with more potent regimens (zoledronic acid, denosumab and/or teriparatide) seems to have the best secondary fracture prevention efficacy.
Objective: This paper describes the various medical therapy regimens prescribed to osteoporotic patients with high fracture risk and the result of treatment.

Background: There are no large-scale epidemiological studies on primary hyperparathyroidism (PHPT) in Russia. The high prevalence of the disease, the high risk of disability and death in this cohort of patients requires the study of the epidemiological and clinical structure of PHPT to determine the extent of medical care.
Aim: Evaluate the frequency of PHPT detection and characterize its clinical forms in Russia using an online registry.
