Although the pathophysiology of migraine involves a complex ensemble of peripheral and central nervous system changes that remain incompletely understood, the activation and sensitization of the trigeminovascular system is believed to play a major role. However, non-invasive, in vivo neuroimaging studies investigating the underlying neural mechanisms of trigeminal system abnormalities in human migraine patients are limited. Here, we studied 60 patients with migraine (55 females, mean age ± SD: 36.

Background: In patients with mechanical aortic and mitral valves requiring catheter ablation of ventricular tachycardia (VT), a technique for access from the right atrium (RA) to the left ventricle (LV) via puncture of the inferoseptal process of the LV was previously described in a single-center series.

Objectives: This study sought to report the multicenter experience of VT ablation using this novel LV access approach.

Methods: We assembled a multicenter registry of patients with double mechanical valves who underwent VT ablation with RA-to-LV access.

To broaden our understanding of bradyarrhythmias and conduction disease, we performed common variant genome-wide association analyses in up to 1.3 million individuals and rare variant burden testing in 460,000 individuals for sinus node dysfunction (SND), distal conduction disease (DCD) and pacemaker (PM) implantation. We identified 13, 31 and 21 common variant loci for SND, DCD and PM, respectively.

Background: Patients undergoing first-time atrial fibrillation (AF) ablation can benefit from targeting non-pulmonary vein (PV) triggers. Preprocedural identification of high-risk individuals can guide planning of ablation strategy. This study aimed to create a preprocedural screening tool to identify patients at risk of non-PV triggers during first-time AF ablation.

Purpose: To evaluate risk factors associated with severe retinopathy of prematurity ROP (sROP) in two separate cohorts of infants.

Methods: We performed a retrospective study of the Merative MarketScan Commercial Database between 2003 and 2022. Infants with ROP were stratified into two cohorts: group A (defined as infants with both birth weight [BW] <1000 g and gestational age [GA] <29 weeks) and group B (defined as infants with either BW ≥1000 g or GA ≥29 weeks).

Article Synopsis
  • The study investigates the discrepancy between the increasing incidence of urologic cancer and the stagnation in the growth of the urologic workforce in the U.S. as the population ages.
  • Researchers analyzed case logs from early career urologists between 2003 and 2019 to determine trends in performing open surgical and minimally invasive oncology procedures.
  • Findings showed a decline in open oncologic procedures done by urologists, with a rise in minimally invasive methods, and highlighted that fellowship training in oncology significantly increased the likelihood of performing open surgeries.

Article Synopsis
  • The study investigates the long-term risk of damage to the right coronary artery (RCA) after patients undergo radiofrequency ablation (RFA) for atrial flutter related to the cavotricuspid isthmus (CTI).
  • A comparison was made between patients who had CTI RFA and those who only received ablation for atrial fibrillation (AF), with coronary angiography performed afterward to evaluate any differences in RCA stenosis.
  • Results indicate that there were no significant differences in the incidence or severity of RCA stenoses between the two groups, suggesting that CTI RFA does not increase the risk of RCA damage in the long term.

Background: Cardiac implantable electronic device (CIED) procedures can cause significant postoperative pain. Opioid use for postoperative pain is associated with risk of persistent use. The benefits of pectoral nerve (PECs) blocks have been established for other chest wall surgeries, but adoption in electrophysiology has been limited.

Background: The importance of nonpulmonary vein (PV) triggers for the initiation/recurrence of atrial fibrillation (AF) is well established.

Objectives: This study sought to assess the incremental benefit of provocative maneuvers for identifying non-PV triggers.

Methods: We included consecutive patients undergoing first-time AF ablation between 2020 and 2022.

Introduction: Identifying the origin of nonpulmonary vein atrial fibrillation (AF) triggers (NPVTs) after pulmonary vein isolation (PVI) can be challenging. We aimed to determine if noninvasive electrocardiographic imaging (ECGi) could localize pacing from common NPVT sites. ECGi combines measured body surface potentials with heart-torso geometry acquired from computed tomography (CT) to generate an activation map.

Article Synopsis
  • The study examines the effectiveness of endocardial (ENDO) ablation in treating arrhythmogenic right ventricular cardiomyopathy (ARVC), specifically focusing on its long-term outcomes and factors influencing survival without ventricular tachycardia (VT).
  • A total of 74 patients with ARVC underwent ENDO-only VT ablation between 1998 and 2020, with about 66% achieving noninducibility of VT and a median follow-up of 6.6 years revealing that over 54% remained free of VT recurrence.
  • Important predictors for long-term VT-free survival included being over 45 years at diagnosis and achieving VT noninducibility, suggesting that ENDO-only ablation could be a viable initial

Article Synopsis
  • Epicardial access is important during ventricular tachycardia ablation, and the study compares two methods: traditional ConvEpi access and a newer method called EpiCO, which involves puncturing coronary venous branches.
  • The study included 153 patients and found that both methods had similar success rates, but ConvEpi was quicker and associated with more complications, especially significant bleeding.
  • Overall, EpiCO access showed a decrease in major complications and bleeding compared to ConvEpi, suggesting it might be a safer option for patients.

Glyburide, a sulfonylurea drug used to treat type 2 diabetes, boasts neuroprotective effects by targeting the sulfonylurea receptor 1 (SUR1) and associated ion channels in various cell types, including those in the central nervous system and the retina. Previously, we demonstrated that glyburide therapy improved retinal function and structure in a rat model of diabetic retinopathy. In the present study, we explore the application of glyburide in non-neovascular ("dry") age-related macular degeneration (AMD), another progressive disease characterized by oxidative stress-induced damage and neuroinflammation that trigger cell death in the retina.

Article Synopsis
  • The study investigates the genetic basis of supraventricular tachycardias, focusing on atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular accessory pathways/reciprocating tachycardia (AVAP/AVRT).
  • Through multiancestry meta-analyses of genome-wide association studies, researchers identified significant genetic loci associated with AVNRT and AVAP/AVRT, implicating specific genes in these cardiac conditions.
  • The results suggest that gene regions related to ion channels and cardiac development play crucial roles in susceptibility to supraventricular tachycardias, potentially influencing other cardiovascular issues such as atrial fibrillation

Objective: To characterize differences between urologists and advanced practice providers (APPs) in the utilization of cystoscopy for hematuria.

Methods: We identified patients initially evaluated for hematuria by a urologist or urology APP between 2015 and 2020 in the MarketScan Research Databases. We determined whether they received a cystoscopy within 6 months of their urology visit and the number of days until cystoscopy.

Background: Premature ventricular complexes (PVCs) are common and associated with worse outcomes in patients with heart failure. Class 1C antiarrhythmic drugs (AADs) effectively suppress PVCs, but guidelines currently restrict their use in structural heart disease.

Objectives: This study aimed to assess the safety and efficacy of class 1C AADs in patients with nonischemic cardiomyopathy (NICM) and implantable cardioverter-defibrillators (ICDs).

Purpose: Metformin has been suggested to protect against the development of age-related macular degeneration (AMD) in multiple observational studies. However, the association between metformin and geographic atrophy (GA), a debilitating subtype of AMD, has not been analyzed.

Methods: We conducted a case-control study of patients ages 60 years and older with new-onset International Classification of Diseases (ICD) coding of GA in the Merative MarketScan Commercial and Medicare Databases between 2017 and 2021.
