We have developed quantitative comparative multiplex dosage analysis to detect altered copy number of regions of the phenylalanine hydroxylase gene. Out of 41 alleles (4% of 1,010 PKU chromosomes) on which a mutation had not been characterized previously, this technique has highlighted two novel mutations: deletions of exon 5 and of exon 6 on a total of eight alleles. Restriction-enzyme digestion of genomic DNA and hybridization to an amplified segment of the phenylalanine hydroxylase (PAH) cDNA probe PAH247 established the size of the deletion in five individuals to be between 700 and 900 bases.

alphaNAC is a transcriptional coactivator known to interact with the N-terminal activation domain of the c-Jun transcription factor. In this article, we describe the identification of the c-Jun interaction domain within the alphaNAC protein. Deletion analysis of alphaNAC indicated that the c-Jun binding site was located in the middle part of the protein, between residues 89 and 129.

Two patients with pneumatosis intestinalis (PI) and mixed connective tissue disease/overlap syndrome are discussed. One patient also presented a retropneumoperitoneum, a feature as yet undescribed in PI. A review of previously reported cases revealed that PI should be suspected in those patients known to have systemic sclerosis-type involvement of the oesophagus and the small bowel, and who present with abdominal distension.

Small cell ("oat cell") carcinoma of the esophagus is a rare disease: only seventy-four patients have been reported in the world literature. We add two new patients, one of whom was studied radiologically. Since oat cell carcinoma originates from the neuro-ectoderm, it is not surprising to see such examples arising outside the lung and in particular from the digestive tract.

The authors have analysed the predictive value of the basal and peak gastric acidity levels, measured preoperatively, in a series of 27 supraselective vagotomies. They recommend care in the choice of this operation for hypersecretors and they propose criteria for the preoperative selection of the candidates. They suggest determination of the preoperative basal gastric acidity level and a cimetidine suppression test to identify patients at high risk for recurrence of the ulcer.

Thrombotic occlusion remains a major cause of central venous catheter removal prior to completion of therapy. Injection of a dilute solution of a potent fibrinolytic agent, urokinase or streptokinase, into the occluded Silastic central venous catheter consistently reestablishes its patency. This procedure was performed on 352 occluded silicone elastomer central venous catheters with only one failure and one minor complication.
