Article Synopsis
  • The study assessed the safety of the Lmeca.A1000 automatic suction device compared to manual suctioning in ICU patients suffering from acute lung injury and respiratory failure.
  • A multicenter, randomized trial involving 117 critically ill adults showed no significant differences in mucosal injury or pneumonia rates between the two methods.
  • The findings suggest that the automatic suction device is safe and could reduce the workload for healthcare professionals without compromising airway management effectiveness.

Background: The arterial to end-tidal carbon dioxide gradient (P [a-Et] CO) reveals the ventilation-perfusion (V/Q) status of critically ill patients. V/Q mismatch has several causes and affects the clinical outcomes of critically ill patients. We investigated the relationship between P (a-Et) CO and the clinical outcomes in critically ill patients.

Background: This study aimed to understand the collective impact of trace elements, vitamins, cholesterol, and prealbumin on patient outcomes in the intensive care unit (ICU) using an advanced artificial intelligence (AI) model for mortality prediction.

Methods: Data from ICU patients (December 2016 to December 2021), including serum levels of trace elements, vitamins, cholesterol, and prealbumin, were retrospectively analyzed using AI models. Models employed included category boosting (CatBoost), extreme gradient boosting (XGBoost), light gradient boosting machine (LGBM), and multilayer perceptron (MLP).

Article Synopsis
  • *A 65-year-old man sustained severe injuries after being struck by a heavy excavator, including damage to the infrarenal IVC and other associated injuries.
  • *Despite challenges in controlling bleeding during surgery, operative techniques and strategies, such as ligating the superficial femoral vein, ultimately helped stabilize the patient for further interventions.

Article Synopsis
  • Ultrasound is an effective and accessible tool for diagnosing sternal fractures in trauma patients, thanks to its speed and accuracy, especially at the bedside.
  • A study conducted from March 2022 to February 2023 involved 26 patients, with 19 diagnosed with fractures via initial chest CT and 7 without fractures, who underwent ultrasound evaluations.
  • The results showed that ultrasound diagnosed 88.5% of sternal fractures and was particularly accurate in patients already confirmed with fractures via CT scans, demonstrating its value as a supplementary diagnostic method.

Article Synopsis
  • The study investigates the effectiveness and safety of laparoscopy in treating blunt and penetrating abdominal injuries, addressing previous uncertainties regarding its use in blunt trauma cases.
  • Researchers analyzed medical records from a single center over a five-year period, comparing outcomes between patients treated with laparoscopy and those who underwent laparotomy, matching them for demographic factors.
  • Results showed that laparoscopy had lower rates of severe complications, shorter ICU stays, and quicker operation times compared to laparotomy, with no missed injuries reported, indicating it is a viable option for stable trauma patients.

To treat hypovolemic shock, fluid infusion or blood transfusion is essential to address insufficient volume. Much controversy surrounds resuscitation in hypovolemic shock. We aimed to identify the ideal fluid combination for treating hypovolemic shock-induced swine model, analyzing bioelectrical impedance and hemodynamics.

Purpose: To investigate whether tibial-sided anterolateral ligament (ALL) injuries are associated with poor healing and residual pivot shift.

Methods: Patients who underwent anterior cruciate ligament reconstructions within postinjury 6 weeks from January 2008 to March 2021 were included. They had concomitant ALL injury confirmed by preoperative magnetic resonance imaging (MRI) and were followed for a minimum of 20 months.

BACKGROUND According to the current guidelines for liver transplantation (LT) of brain-dead donors with hepatitis B or C virus (HBV or HCV) in Korea, grafts from hepatitis B surface antigen (HBsAg)(+) or HCV antibody (anti-HCV)(+) donors must be transplanted only to HBsAg(+) or anti-HCV(+) recipients, respectively. We aimed to determine the current status and outcomes of brain-dead donor LT with HBV or HCV in Korea. MATERIAL AND METHODS This retrospective observational study included all LTs from brain-dead donors in the Korean Organ Transplantation Registry between April 2014 and December 2020.

Background: According to the current Center for Korean Network for Organ Sharing guidelines for kidney transplantation from brain-dead donors with hepatitis B or C infection, organs from hepatitis B surface antigen-positive (HbsAg) or anti-hepatitis C virus-positive (HCV) donors can only be transplanted into HBsAg or anti-HCV recipients. We aimed to confirm the status and the outcomes of kidney transplantation from brain-dead donors with hepatitis B or C virus in Korea.

Methods: This retrospective study included all kidney transplantations from brain-dead donors in the Korean Organ Transplantation Registry database between January 2015 and June 2020, divided into 3 groups according to donor hepatitis status.

The "Act on hospice and palliative care and decisions on life-sustaining treatment for patients at the end of life" was enacted in February 2018 in Korea. Therefore, we suggest a Korean guideline for organ donation after circulatory death (DCD) category III after the withdrawal of life-sustaining treatment (WLST). Implementation of WLST includes stopping ventilation, extubation, discontinuation of inotropics and vasoconstrictors, cessation of continuous renal replacement therapy, and cessation of extracorporeal membrane oxygenation.

In February 2018, the Withdrawal of the Life-sustaining Treatment (WLST) Decision Act was legalized in Korea. Donation after circulatory death (DCD) after WLST was classified as DCD category III. We report the first case of successful organ donation after WLST in Korea.

BACKGROUND The number of organ donations from hepatitis B virus (HBV) or hepatitis C virus (HCV)-positive donors is gradually increasing; however, the current status of organ donation from brain-dead donors with hepatitis in South Korea has not been analyzed. This study aimed to analyze this. MATERIAL AND METHODS In total, 9210 potential brain deaths were reported in South Korea from January 2013 to December 2017, of which 333 were hepatitis carriers (HBV, n=246; HCV, n=87).
