Objective Assessment and Standard Setting for Basic Flexible Ureterorenoscopy Skills Among Urology Trainees Using Simulation-Based Methods.

To objectively assess the performance of graduating urology residents performing flexible ureterorenoscopy (fURS) using a simulation-based model and to set an entrustability standard or benchmark for use across the educational spectrum. Chief urology residents and attending endourologists performed a standardized fURS task (ureterorenoscopy and repositioning of stones) using a Boston Scientific Lithovue ureteroscope on a Cook Medical URS model. All performances were video-recorded and blindly scored by both endourology experts and crowd-workers (C-SATS) using the Ureteroscopic Global Rating Scale, plus an overall entrustability score.

Dual usage of a stone basket: Stone capture and retropulsion prevention.

Introduction: Stone migration during ureteroscopy (URS) for proximal ureteric calculi is a constant challenge. Several retropulsion prevention devices have been developed to optimize URS outcomes. Our technique involves capturing the stone within a four-wire Nitinol stone basket and then performing laser lithotripsy to dust the stone while it is engaged in the basket.

The Army Selected Reserve Dental Readiness System: overview, assessment, and recommendations.

The Army Selected Reserve Dental Readiness System (ASDRS) is a key dental program directed by the Assistant Secretary of the Army (Manpower & Reserve Affairs) starting in fiscal year 09. The Army National Guard and Army Reserve have steadily implemented ASDRS over the past 3 years as a means to improve the historically abysmal dental readiness of the Army Reserve Component (RC). Dental readiness is essential for sustaining an Army RC Operational Force.

Inspiratory muscle fatigue after race-paced swimming is not restricted to the front crawl stroke.

The occurrence of inspiratory muscle fatigue (IMF) has been documented after front crawl (FC) swimming of various distances. Whether IMF occurs after other competitive swimming strokes is not known. The aim of the present study was to assess the impact of all 4 competitive swimming strokes on the occurrence of IMF after race-paced swimming and to determine whether the magnitude of IMF was related to the breathing pattern adopted and hence breathing frequency (f(b)).

The Canadian StoneBreaker trial: a randomized, multicenter trial comparing the LMA StoneBreaker™ and the Swiss LithoClast® during percutaneous nephrolithotripsy.

Background And Purpose: Percutaneous nephrolithotripsy (PCNL) is the treatment of choice for patients with large renal stones. The StoneBreaker™ (SB) is a novel handheld pneumatic lithotriptor, powered by a compressed carbon dioxide cartridge. The purpose of this study was to compare the efficiency of the SB to a standard pneumatic lithotriptor, the Swiss LithoClast(®) (LC).

Ready for practice? A study of confidence levels of final year dental students at Cardiff University and University College Cork.

Aim: The aim of this study was to describe the self-reported confidence levels of final year students at the School of Dentistry, Cardiff University and at the University Dental School & Hospital, Cork, Ireland in performing a variety of dental procedures commonly completed in primary dental care settings.

Method: A questionnaire was distributed to 61 final year students at Cardiff and 34 final year students at Cork. Information requested related to the respondents confidence in performing a variety of routine clinical tasks, using a five-point scale (1=very little confidence, 5=very confident).

Rectal gastrointestinal stromal tumor mimicking a primary prostatic lesion.

The interstitial cells of Cajal have been identified in locations beyond the gastrointestinal tract, including the prostate, uterus and bladder. Indeed, there are reports of primary gastrointestinal stromal tumor (GIST) arising from each of these sites. We report the case of a 72-year old male who presented with benign prostatic hypertrophy and was diagnosed on retropubic prostatectomy as having a GIST.

Materials And Methods: A total of 78 patients with 1 cm or less isolated lower pole stones were randomized to SWL or URS.

This report describes an illustrative case of malignant pheochromocytoma of the urinary bladder in a 28-year old man. A combined-modality treatment plan with partial cystectomy and post-operative radiotherapy and concurrent chemotherapy with single agent cisplatin weekly was performed. Three weeks after completing concurrent chemoradiation the first of four planned cycles of cisplatin 25 mg/m2 and etoposide 100 mg/m2 was administered daily over 3 days.

Purpose: Herbal remedies high in phytoestrogens have been shown to reduce serum prostate specific antigen (PSA) and have been proposed as a treatment for prostate cancer. Soy proteins used to lower serum cholesterol are rich sources of phytoestrogens. Therefore, we assessed the effect of soy consumption on serum PSA in men who had participated in cholesterol lowering studies.

High urinary excretion of uric acid combined with high excretion of calcium links kidney stone disease to familial hypertension.

Background: Past studies identified an association between kidney stone disease (KSD) and hypertension. We recently reported a high occurrence of hypertension in families of patients with hyperuricosuric KSD. As hypercalciura frequently coexists with hyperuricosuria and high urinary excretion of calcium is found in patients with hypertension, we hypothesized that hyperuricosuria that is accompanied by hypercalciuria better describes the familial association between KSD and hypertension.

Assessing the effectiveness of direct digital radiography barrier sheaths and finger cots.

Background: Effective cross-contamination prevention is critical for direct digital radiography, or DDR, sensors, which are not sterilizable; however, current manufacturers' recommendations for standard precautions are limited to the use of plastic barrier sheaths, which are commonly known to tear or leak. The authors sought to determine the incidence of digital radiography barrier-sheath leakage, with and without additional latex finger cot protection, as measured by a water pressure test.

Methods: Four hundred plastic barrier sheaths were randomly assigned to four groups based on intraoral radiograph positioning device use and supplemental barrier protection with a latex finger cot.

Objective: To determine whether kidney stone disease (KSD) and hypertension (HTN) share a common familial component that is determined by a specific urinary biochemical abnormality.

Design: Familial aggregation study.

Patients: Two hundred and twelve KSD patients, aged 18-50 years, collected a 24-h urine sample to measure the urinary excretion of uric acid, calcium, oxalate, magnesium and citrate, and were interviewed about the occurrence of HTN among first-degree relatives.

Over 14 years of clinical use of extracorporeal shockwave lithotripsy (SWL), great technical modifications resulted in the development of many second-generation lipthotripters. The Siemens Lithostar machine, with its standard shockwave tube, was introduced in 1986. The objective of this study was to assess the safety and effectiveness of the newly proposed Lithostar shock tube C in the treatment of urinary calculi.

Hospital charges for people with AIDS in Washington State: utilization of a statewide hospital discharge data base.

We analyzed Washington State inpatient hospital utilization for 165 AIDS (acquired immunodeficiency syndrome) cases with 344 hospitalizations from July 1984 through December 1985. We found that mean charges per hospitalization were $9,166 and mean length of stay was 13.3 days.
