Background: Interferon-γ release assays (IGRAs) for tuberculosis infection (TBI) cannot distinguish different stages of the TBI spectrum (including spontaneously cleared infection). We investigated patterns of Mtb-specific blood mediators in people with and without TBI during tuberculosis preventive therapy (TPT).

Methods: Individuals with likelihood of recent Mtb exposure, aged 15-25 years, with valid IGRA results, in whom tuberculosis (TB) had been excluded, were included.

Does Caffeine Consumption Influence Postcompetition Sleep in Professional Rugby League Athletes? A Case Study.

Purpose: To examine the sleep of rugby league athletes the night before and following an evening match and to investigate the association between caffeine intake and sleep on the night of competition.

Methods: On the night prior to, night of, and night after a professional rugby league match, 15 athletes had their sleep monitored using wrist activity monitors. Additionally, saliva samples were collected 60 minutes before and 30 minutes after the competition to assess salivary caffeine concentration.

The impact of global health opportunities on residency selection.

Background: An increasing number of medical trainees across specialties desire and expect Global Health (GH) experiences during training. It is useful for residency programs to know the impact that offering GH opportunities has on resident recruitment. The study objectives were to explore the importance of GH opportunities in residency selection among fourth-year medical students, examine the relationship between interest in GH and career plans, and describe students' perspectives on prior GH experiences.

Ophthalmoplegia and cranial nerve deficits in an adolescent with headache.

Tolosa-Hunt syndrome is an idiopathic, inflammatory condition involving the cavernous sinus and is characterized by unilateral, painful ophthalmoparesis. The condition often begins with retro-orbital pain followed by select cranial nerve involvement. We report the case of a 17-year-old female whose presentation with progressive left-sided headache and ophthalmoparesis culminated in the diagnosis of Tolosa-Hunt syndrome.

Hemoglobin Sunshine Seth: A Case Report of Low-Oxygen-Affinity Hemoglobinopathy.

Pulse oximetry is routinely used in the newborn nursery for clinical monitoring and to detect critical congenital heart disease. The differential diagnoses for reduced peripheral oxygen saturation in an infant include congenital heart disease, respiratory distress syndrome, transient tachypnea of the newborn, persistent pulmonary hypertension of the newborn, meconium aspiration syndrome, pneumonia, pneumothorax, and sepsis. The diagnostic evaluation for neonatal hypoxemia can be invasive and expensive.

Sertraline-Induced Rhabdomyolysis, Trismus, and Cardiac Arrest in a Child.

Selective serotonin reuptake inhibitors are a commonly used and often effective class of medications in the treatment of mood disorders such as anxiety and depression. Sertraline (1S,4S-N-methyl-4-[3,4-dichlorophenyl]-1,2,3,4-tetrahydro-1-naphthylamine [Zoloft; Pfizer, New York City, NY]) is a frequently used selective serotonin reuptake inhibitor that has shown efficacy in children, adolescents, and adults. We report the case of a 13-year-old boy with sertraline-induced rhabdomyolysis and renal failure, trismus, and cardiopulmonary arrest.

Pediatric Sepsis Secondary to an Occult Dental Abscess: A Case Report.

Background: In general, hematogenous spread of bacteria in children is uncommon. Bacteremia, however, is a known complication of dental procedures and severe caries, but is infrequently associated with primary, asymptomatic, non-procedural-related, dentoalveolar infection.

Case Report: The patient is a 7-year-old previously healthy boy who presented to the Emergency Department (ED) with "fever, mottling, and shaking chills.

Influence of pre-annealing on the thermal regeneration of fiber Bragg gratings in standard optical fibers.

A detailed study of the dynamics during thermal regeneration of fiber Bragg gratings, written in hydrogen-loaded standard single-mode fibers using a ns pulsed 213 nm UV laser, is reported. Isothermal pre-annealing performed in the range 85 °C to 1100 °C, with subsequent grating regeneration at 1100 °C, resulted in a maximum refractive index modulation, Δn(m) ~1.4⋅10(-4), for gratings pre-annealed near 900 °C while a minimum value of Δn(m) ~2⋅10(-5) was achieved irrespective of pre-annealing temperature.

P-wave characteristics and histological atrial abnormality.

Background: Fibro-fatty transformation is believed to be the leading cause of deteriorated atrial conduction; however, any direct assessment in relation to P-wave characteristics is lacking. We sought to assess P-wave morphology (PWM) and duration (PWD) in relation to histology of the atrial myocardium.

Objective: Atrial specimens were collected from 11 patients who died from cardiovascular causes (7 men; median age 73 years).

Purpose: To evaluate the ability of T2* mapping to help differentiate damaged from normal acetabular cartilage in patients with femoroacetabular impingement (FAI).

Materials And Methods: The institutional review board approved this retrospective study, and the requirement to obtain informed consent was waived. The study complied with HIPAA guidelines.

Carotid endarterectomy: a Southern North Island regional consensus statement.

Aims: The aim of this project was to employ interdepartmental and cross district health board collaboration to reach a regional consensus on the management of patients who may benefit from carotid endarterectomy.

Methods: All regional stroke physicians, neurologists, and vascular surgeons met to review relevant literature and local audits and to discuss best management strategies suited to the region.

Results: A consensus statement was agreed upon and is presented here along with a summary of the supporting scientific evidence.

Secondary brain injuries in thalamus and hippocampus after focal ischemia caused by mild, transient extradural compression of the somatosensori cortex in the rat.

The development and distribution of secondary brain lesions, subsequent to ischemic stroke, are of considerable clinical interest but so far only a limited number of studies have investigated the distribution and development of these secondary lesions in detail. In this study, we used an animal model of focal ischemia caused by extradural compression of the sensorimotor cortex. This paradigm of focal ischemia was shown to produce a consistent pattern of secondary lesions located distally from the primary lesion.
