Innovating for real-world care: A systematic review of interventions to improve post-detoxification outcomes for opioid use disorder.

Background: Inpatient detoxification is a common health care entry point for people with Opioid Use Disorder (OUD). However, many patients return to opioid use after discharge and also do not access OUD treatment. This systematic review reports on the features and findings of research on interventions developed specifically to improve substance use outcomes and treatment linkage after inpatient detoxification for OUD.

Pre-Columbian fire management and control of climate-driven floodwaters over 3,500 years in southwestern Amazonia.

In landscapes that support economic and cultural activities, human communities actively manage environments and environmental change at a variety of spatial scales that complicate the effects of continental-scale climate. Here, we demonstrate how hydrological conditions were modified by humans against the backdrop of Holocene climate change in southwestern Amazonia. Paleoecological investigations (phytoliths, charcoal, pollen, diatoms) of two sediment cores extracted from within the same permanent wetland, ∼22 km apart, show a 1,500-y difference in when the intensification of land use and management occurred, including raised field agriculture, fire regime, and agroforestry.

Aims: Most inpatient alcohol detoxification patients do not seek treatment post-discharge, which increases the risk of relapse and re-hospitalization. To date, there have been no efforts to synthesize the evidence supporting the broad range of available interventions for this critical transition. The current study is a systematic review and evaluation of interventions designed to promote treatment engagement and recovery following alcohol detoxification.

Reaction pattern to three stresses--electroplexy, surgery, and myocardial infarction--of fibrinolysis and plasma fibrinogen.

The effects of three types of stress-electroplexy, surgery, and myocardial infarction-on blood fibrinolytic activity and plasma fibrinogen levels were studied in 10, eight, and six patients respectively. The fibrinolytic response to electroplexy consisted of an initial short increase followed in half the patients by reduced fibrinolytic activity lasting two to four days. After surgery and myocardial infarction normal fibrinolytic activity was followed by a period of reduced activity; the timing of the measurements on these patients may have precluded recognition of an initial increase in fibrinolysis similar to that following electroplexy.
