Article Synopsis
  • A multicenter study was conducted with 1,742 patients who achieved a sustained viral response (SVR) after chronic hepatitis C treatment to create a machine learning model for predicting the risk of developing hepatocellular carcinoma (HCC).
  • Five machine learning models were evaluated, with the random survival forest (RSF) model performing the best in predicting HCC risk during a follow-up period, achieving a c-index of 0.839 in an independent cohort.
  • The RSF model provides individualized risk predictions and is available online, signaling the potential to enhance surveillance strategies for HCC following SVR, although further studies are needed for tailored surveillance recommendations globally.

Article Synopsis
  • This study assessed how well fluoroscopy and endoscopy screenings worked to lower gastric cancer death rates in the general population.
  • It analyzed data on mortality rates and screening introduction from a Cancer Registry database, using statistical methods to evaluate trends over decades.
  • Results showed a significant decrease in mortality rates due to screening, particularly for women, indicating the effectiveness of these screenings has diminished over time.

A 58-year-old man who had the history of alcohol dependence was referred to our emergency center due to severe nausea, vomiting, and subsequent onset of chest and back pain. Esophagogastroduodenoscopy (EGD) showed black-appearing esophagus mucosa extending from the cervical esophagus to the esophagogastric junction with clear margins, a condition typically referred to as a black esophagus. Alcohol abuse was considered an important factor associated with acute esophageal necrosis in this patient.

Background And Aims: It remains unclear whether obesity increases the risk of hepatocellular carcinoma (HCC) in patients with chronic hepatitis C who achieved a sustained virological response (SVR) with antiviral therapy.

Methods: In this multicenter cohort study, we enrolled patients with chronic hepatitis C who achieved SVR with interferon (IFN)-based therapy (IFN group) or direct-acting antiviral (DAA) therapy (DAA group) between January 1, 1990, and December 31, 2018. The patients underwent regular surveillance for HCC.

Patients with advanced cirrhosis show an abnormal regulation of extracellular fluid volume, resulting in the accumulation of fluid as ascites or edema. As portal hypertension develops, splanchnic arterial vasodilation also does due mainly to the production of nitric oxide (NO). Splanchnic arterial vasodilation decreases effective arterial blood volume, leading to fluid accumulation and renal function abnormalities which are a consequence of the homeostatic activation of vasoconstrictor and antinatriuretic factors.

Background & Aims: Sphingosine 1-phosphate (S1P), a ligand for G protein-coupled endothelial differentiation gene-1 (Edg-1), Edg-3, Edg-5, Edg-6, and Edg-8, elicits a variety of responses by cells. Prominent among these is cell proliferation. S1P is abundantly stored in platelets and released upon their activation, suggesting that S1P plays a pathophysiologic role in vivo.
