Article Synopsis
  • Seven low molecular weight peptides were identified as potential biomarkers for diagnosing hypertensive disorders of pregnancy (HDP) and their relationship with leg ischemia and atherosclerosis in patients with lower extremity arterial disease (LEAD) was explored.
  • * A study involving 165 outpatients (mainly men) assessed symptoms of leg ischemia and blood flow using the Rutherford classification, ankle-brachial index (ABI), and carotid artery intima-media thickness (IMT).
  • * Results indicated that certain peptides (m/z 2091 and 2378) were positively correlated with leg ischemia severity, while peptide m/z 2081 showed an inverse relationship; however, these peptides did not correlate with carotid atherosclerosis.

Objectives: Cardio-ankle vascular index (CAVI) is a blood pressure-independent measure of heart-ankle pulse wave velocity and is used as an indicator of arterial stiffness. However, there is a paradox that CAVI is inversely associated with leg ischemia in patients with lower extremity arterial disease (LEAD). The aim of this study was to clarify the significance of the absolute value of left and right difference in CAVI (diff-CAVI).

Measurements of ankle-brachial index (ABI) and toe-brachial index (TBI) are standard examinations for evaluating arterial blood flow in lower extremities and diagnosing lower extremity artery disease (LEAD). It remains to be clarified whether cardio-ankle vascular index (CAVI), a blood pressure-independent parameter of arterial stiffness, is associated with ABI and TBI in patients with LEAD. The subjects were 165 outpatients with LEAD.

Article Synopsis
  • Dysgeusia, a taste disorder caused by cancer chemotherapy, negatively affects patients' quality of life, and this study aimed to evaluate its prevalence and causes among patients with gastrointestinal cancer.
  • The research involved 21 patients undergoing chemotherapy and utilized both subjective and objective assessment tools to measure taste alterations, finding that dysgeusia occurred in 38% of patients treated with certain regimens, particularly those containing platinum-based drugs.
  • The study concluded that a combined assessment approach was effective in diagnosing dysgeusia and suggested that factors other than zinc deficiency might contribute to this condition.

Article Synopsis
  • Gastrectomy with D2 dissection and adjuvant chemotherapy is the usual treatment for locally advanced gastric cancer (LAGC) in Asia, but administering post-surgery chemotherapy can be challenging, especially in older patients.
  • This phase II study (KSCC1801) investigated the safety and efficacy of neoadjuvant chemotherapy (NAC-SOX) in patients aged 70 and above, finding that it resulted in a high resection rate (92.3%) and a significant pathological response (62.5%).
  • Though NAC-SOX showed promise, there were notable adverse events like neutropenia and a treatment-related death due to severe diarrhea, indicating the need for careful management.

Background And Aims: Seven circulating peptides, consisting of 18-28 amino acids, were identified as possible biomarkers of hypertensive disorders of pregnancy (HDP) in our previous study. However, it is unknown whether these peptides are relevant to cardiovascular disease. The purpose of this study was to clarify the relationships between serum levels of these peptides and leg arterial blood flow in patients with lower extremity arterial disease (LEAD).

Background: The risk of cardiovascular disease is lower in light-to-moderate alcohol drinkers than in nondrinkers in the general population. However, it remains to be clarified whether these beneficial effects of alcohol are shown in patients with peripheral arterial disease (PAD).

Methods: The subjects were 153 male outpatients with PAD and they were divided by frequency of drinking into nondrinkers, occasional drinkers (1-4 days/wk) and regular drinkers (5-7 days/wk).

Background: The mainstream first-line chemotherapy for advanced/recurrent gastric cancer (ARGC) is combination therapy including platinum-based agents. With the progressive aging of the society, the incidence of gastric cancer in elderly patients is increasing. However, elderly patients cannot tolerate these agents because of renal dysfunction or low quality of life.

Background: Perioperative chemotherapy is the standard of care for locally advanced gastric cancer (LAGC). This phase II study investigated the efficacy and safety of S-1 and oxaliplatin (SOX) as neoadjuvant chemotherapy (NAC) for LAGC and esophagogastric junction cancer (EGJC).

Methods: Patients completed up to three cycles of SOX (oxaliplatin 130 mg/m on day 1, oral S-1 40-60 mg twice daily for 2 weeks every 3 weeks), followed by gastrectomy and D2 lymphadenectomy.

Article Synopsis
  • Poor preoperative nutritional and immunological status, measured by the Prognostic Nutrition Index (PNI), increases the risk of postoperative complications in patients with hepatocellular carcinoma (HCC) after surgery.
  • A study of 510 patients found that those with lower PNIs experienced significantly more complications than those with higher PNIs, indicating that a PNI below 45 is a strong predictor of adverse outcomes.
  • The research suggests that combining the PNI with the type of surgical procedure (major or minor hepatectomy) can improve predictions for postoperative complications, highlighting the importance of preoperative assessments.

Background: Decompression sickness (DCS) induced by extravascular and intravascular gas bubbles during decompression can present with varying manifestations, such as joint pain, numbness, cutaneous symptoms, and cardiopulmonary dysfunction. However, mesenteric venous thrombosis (MVT) is a rare complication of DCS. To the best of our knowledge, only a few cases have been reported, and surgical cases of MVT secondary to DCS have not yet been reported.

Article Synopsis
  • The Cardiometabolic Index (CMI) combines waist-to-height ratio and triglycerides-to-HDL cholesterol ratio to help identify diabetes risk and assess its relationship with atherosclerotic diseases in patients with peripheral arterial disease (PAD).
  • A study involving 63 PAD outpatients found that higher CMI levels were significantly correlated with increased thickness in the common carotid artery and greater declines in blood pressure after exercise, indicating more severe atherosclerotic progression.
  • Ultimately, CMI serves as an effective measure to distinguish the extent of atherosclerosis and ischemic conditions in patients suffering from PAD.
