Background: Small mesenteric hiatal hernias (SMHHs) are defined as a small group of internal hernias (IHs) that frequently diagnosed in children. However, SMHHs are relatively rare in adults. Bowel loop herniation via an abnormal mesenteric defect can lead to strangulated intestinal obstruction.

Article Synopsis
  • Surgical site infections (SSIs) are common in surgery patients, and this trial aims to compare the effectiveness of olanexidine and chlorhexidine-alcohol in reducing SSI rates during clean-contaminated gastrointestinal surgeries.
  • The trial will involve patients under 18 years old and assess various outcomes, including overall SSI rates and any adverse reactions to the antiseptics over a 30-day postoperative period.
  • The study has received ethical approval and began participant recruitment in January 2023, with the results expected to be published in medical journals following peer review.

Using natural gas as chemical feedstock requires efficient oxidation of the constituent alkanes-and primarily methane. The current industrial process uses steam reforming at high temperatures and pressures to generate a gas mixture that is then further converted into products such as methanol. Molecular Pt catalysts have also been used to convert methane to methanol, but their selectivity is generally low owing to overoxidation-the initial oxidation products tend to be easier to oxidize than methane itself.

Fe(II) complexes with pentadentate ligands, including -heterocyclic carbene moieties, were prepared and electrostatically attached onto the perfluorinated surface of a mesoporous aluminosilicate. The heterogeneous catalysts were applied to the catalytic oxidation of cyclohexane and cyclohexene using HO as an oxidant in CHCN, demonstrating high performance and selectivity in alkane hydroxylation and cyclohexene epoxidation.

Interparietal inguinal hernia, an exceedingly rare type of inguinal hernia in which the hernia sac anatomically lies between the tissue layers of the abdominal wall, is difficult to diagnose from physical findings. Given the few reports on interparietal inguinal hernias, this condition has remained fairly unrecognized. Herein, we report the successful imaging and laparoscopic diagnoses as well as repair of an interparietal inguinal hernia.

Background/aim: Pancreatic cancer, which exhibits resistance to cytotoxic and molecular targeted drugs, has an extremely poor prognosis. Nuclear factor-κB (NF-κB) is constitutively activated in many pancreatic cancer cases. Although the NF-κB inhibitor dehydroxymethylepoxyquinomicin (DHMEQ) has exhibited anti-cancer effects in pancreatic cancer models, its poor solubility limits its use to intraperitoneal administration.

Background: Definitive chemoradiotherapy (dCRT) using cisplatin plus 5fluorouracil (CF) with radiation is considered the standard treatment for unresectable locally advanced T4 esophageal squamous cell carcinoma (ESCC). Recently, induction chemotherapy has received attention as an effective treatment strategy.

Case Summary: We report a successful case of a 59-year-old female with unresectable locally advanced T4 ESCC treated by two additional courses of chemotherapy with CF after induction chemotherapy with docetaxel, cisplatin and fluorouracil (DCF) followed by dCRT.

Article Synopsis
  • Pancreaticoduodenectomy (PD) is rarely done for advanced gastric cancer due to high risks, but advancements in neoadjuvant chemotherapy have improved outcomes, especially for HER2-positive cases.
  • A 66-year-old male with advanced gastric cancer invading the pancreatic head underwent successful PD after receiving neoadjuvant S-1 plus oxaliplatin (SOX) and trastuzumab, leading to a partial response in the tumor.
  • The case highlights that radical resection with PD could be a viable treatment option for patients with locally advanced HER2-positive gastric cancer, provided there are no non-curative factors present.

Background: Inguinal hernia repair is one of the most common general surgical operations worldwide. We present a case of indirect inguinal hernia containing an expanded portosystemic shunt vessel.

Case Summary: We report a 72-year-old man who had a 4 cm × 4 cm swelling in the right inguinal region, which disappeared with light manual pressure.

An Fe complex, 1, having a pentadentate ligand with an NHC moiety catalyzes substrate oxidation to afford 2e-oxidized products with high selectivity by suppression of overoxidation in water. A Bell-Evance-Polanyi plot for the substrate oxidation catalyzed by 1 exhibited an inflection point around 86 kcal mol, indicating strong C-H abstraction ability of the reactive species derived from 1.

Background: Although surgery is the definitive curative treatment for biliary tract cancer (BTC), outcomes after surgery alone have not been satisfactory. Adjuvant therapy with S-1 may improve survival in patients with BTC. This study examined the safety and efficacy of 1 year adjuvant S-1 therapy for BTC in a multi-institutional trial.

We report the case of a 65-year-old male who developed heterochronous local recurrences of gastric cancer in the jejunal pouch (J-pouch) four times after total gastrectomy. He underwent total gastrectomy, J-pouch, and Roux-en-Y reconstruction for stage II gastric cancer in 2005. Four local recurrences appeared on the esophago-jejunal anastomosis, the suture line within the pouch, the esophago-jejunal anastomosis, and the anastomosis between the jejunum and Y-loop, which were resected by partial excision or endoscopic submucosal dissection.

Introduction:: Angiotensin-converting enzyme (ACE) inhibitors are one of the most commonly used medications for hypertension. Rarely, ACE inhibitors have the potential to cause a syndrome of inappropriate secretion of antidiuretic hormone (SIADH).

Case Presentation:: A 70-year-old woman with > 10 years ACE inhibitor therapy with normonatremia suddenly developed severe SIADH when she took a liquid diet in the uneventful perioperative period, with hemodynamic stability and no surgical complications.

Petersen hernia is a rare internal hernia that occurs after Roux-en-Y (R-Y) reconstruction. To our knowledge, there are a few reports on internal hernia, especially Petersen hernia after open gastrectomy for gastric cancer. Two rare cases of Petersen hernia are presented in this report.

We report on a 33-year-old female liver donor candidate who developed intraoperative latex-induced anaphylactic shock during surgery for living donor transplantation. She was the mother of the organ recipient, who was a 9-year-old boy with biliary atresia. We planned extended lateral segmentectomy for her.

We report a case of intrahepatic cholangiocarcinoma arising 33 years after excision of a choledochal cyst. A 61-year-old woman was admitted to our hospital complaining of fever. Thirty-three years ago she had undergone extrahepatic choledochal cystectomy and choledochojejunostomy for a choledochal cyst.

Background: Limited resection is reserved for patients with high operative risk or benign adenomas. We aimed to define indications for limited resection of early ampulla of Vater carcinoma with curative intent through detailed preoperative examinations and histopathological evaluations.

Methods: We performed a retrospective cohort study of all consecutive Japanese patients who underwent resection for ampulla of Vater neoplasms at our hospital from 1986 to 2010.
