Article Synopsis
  • A 47-year-old man with a long history of ulcerative colitis experienced sudden acute epigastric pain, leading to a medical examination that revealed eosinophilic gastroenteritis.
  • After initial treatment with steroids proved ineffective, the patient was given vedolizumab, which successfully alleviated his symptoms and showed significant improvement in both conditions during follow-up examinations.
  • The study suggests that vedolizumab, known for treating ulcerative colitis, may also be a viable treatment option for eosinophilic gastroenteritis due to the shared mechanism of targeting eosinophils.

Background: Pancreatic neuroendocrine neoplasms (PanNENs) are a heterogeneous group of tumors. Although the prognosis of resected PanNENs is generally considered to be good, a relatively high recurrence rate has been reported. Given the scarcity of large-scale reports about PanNEN recurrence due to their rarity, we aimed to identify the predictors for recurrence in patients with resected PanNENs to improve prognosis.

Objective: This study was performed to evaluate whether the use of CAD EYE (Fujifilm, Tokyo, Japan) for colonoscopy improves colonoscopy quality in gastroenterology trainees.

Methods: The patients in this multicenter randomized controlled trial were divided into Group A (observation using CAD EYE) and Group B (standard observation). Six trainees performed colonoscopies using a back-to-back method in pairs with gastroenterology experts.

Purpose: Therapeutic efficacy of ustekinumab in the real-world data is limited in patients with refractory Crohn's disease (CD). In addition, factors predictive of better therapeutic efficacy of ustekinumab remains unsolved in CD. We aimed to evaluate therapeutic efficacy of ustekinumab in patients with refractory CD and to identify the factors associated with the efficacy of ustekinumab.

Article Synopsis
  • - Atopic dermatitis (AD) is a chronic skin condition characterized by itching and is influenced primarily by type 2 inflammation, although the role of non-type 2 inflammation is not fully understood.
  • - Researchers developed the FADS mouse model to study AD, finding that disrupting or inhibiting periostin, a molecule involved in type 2 inflammation, reduces eczema symptoms and inflammation without affecting type 2 inflammation levels.
  • - The study suggests that periostin connects inflammation's effects on skin cells to the sensation of itching, making it a potential target for new treatments for atopic dermatitis.

Background: Given the increasing health concerns for patients with inflammatory bowel disease (IBD), amidst the COVID-19 pandemic, we investigated the impact of the pandemic on the anxiety and behavioral changes in Japanese patients with IBD.

Methods: We analyzed 3032 questionnaires from patients with IBD, aged 16 years or older visiting 30 hospitals and 1 clinic between March 2020 and June 2021. The primary outcome was the score of the anxiety experienced by patients with IBD during the pandemic.

Article Synopsis
  • - Systemic sclerosis (SSc) is a complex connective tissue disease characterized by fibrosis of organs, vascular injury, and autoimmunity, with TGF-β playing a crucial role in fibrosis development.
  • - The study highlights the role of SOX11, a transcription factor, in promoting periostin expression which, in turn, enhances TGF-β signaling in dermal fibroblasts from SSc patients.
  • - Genetic deletion of Sox11 in fibroblasts expressing periostin reduces skin fibrosis, suggesting that the interplay between SOX11, periostin, and TGF-β is significant in the pathogenesis of SSc.

Backgroud: Peutz-Jegers syndrome (PJS) is an autosomal dominant hereditary disorder characterized by hamartomatous polyposis of the entire gastrointestinal tract. Fibrinogen (Fbg) is synthesized by the liver, and hypofibrinogenemia is often asymptomatic and manifests with bleeding after trauma or invasive surgical procedures. Here, we present a case of a pediatric patient with PJS and hypofibrinogenemia who manifested with gastrointestinal bleeding after endoscopic mucosal resection (EMR) of small intestinal polyps.

A 60-year-old man with type 2 diabetes mellitus treated with a dipeptidyl peptidase-4 (DPP-4) inhibitor was referred to our hospital because of his refractory watery diarrhea. Ileocolonoscopy revealed increased capillary growth, fine granular mucosa, and longitudinal mucosal tears mainly in the left side of the colon. A bioptic examination revealed thickened subepithelial collagen bands, thus confirming the diagnosis of collagenous colitis.

Objective: The aim of this study was to determine the characteristics of patients without Helicobacter pylori infection who were prescribed antacid medications (potassium-competitive acid blockers, proton pump inhibitors, and/or H2 receptor antagonist) and had no upper gastrointestinal lesions detected by endoscopy.

Methods: This cross-sectional study included the patients who underwent upper gastrointestinal endoscopy in our institution between August 2017 and July 2018. They were aged from 55 to 89 years, had no upper gastrointestinal lesions detected by endoscopy, and no H.

Introduction/aims: Immune checkpoint inhibitors (ICIs) sometimes cause immune-related adverse events (irAEs), of which there is little information in the literatures. The objective of this study was to characterize the clinical features of gastrointestinal irAEs (GI irAEs).

Materials And Methods: From a total of 250 patients who were administered anti-PD-1 antibodies (nivolumab and pembrolizumab), we retrospectively identified 9 patients with grade 2 or higher GI irAE based on medical records.

Background: In both Western countries and in Japan, the incidence of colonic diverticular bleeding has increased with increased use of antithrombotic and nonsteroidal anti-inflammatory drugs (NSAIDs). Therefore, the Japan Gastroenterological Association issued guidelines for colonic diverticular bleeding and colonic diverticulitis in Japanese in 2017 and in English in 2019. However, the guidelines contain unresolved problems.

Background: Endoscopic retrograde pancreatography (ERCP) is sometimes complicated by post-ERCP pancreatitis (PEP), which is a severe adverse effect.

Objective: The present study was performed to (i) evaluate the risk factors for PEP and (ii) compare the risk of PEP after ERCP performed in the off hours versus regular hours.

Methods: This retrospective study included 374 patients who underwent ERCP from January 2013 to December 2017.

Background And Aim: Barrett's esophagus and colorectal polyps have several overlapping risk factors. Whereas several reports in Western countries have indicated a close relationship between Barrett's esophagus and colorectal polyps, the relationship between these two diseases remains unclear in Japan. This study was performed to determine whether the prevalence of Barrett's esophagus is related to that of colorectal polyps in Japanese patients.
