In the field of cancer care, it is still difficult to say that advance care planning(ACP)has been fully integrated, and the perceptions of ACP vary depending on the individual's role. Since oncologists have a deeper understanding of the patient's condition and prognosis than the patients themselves, they are often the first to recognize the future need for ACP. It is important to gather and incorporate patients' preferences and relevant information regarding ACP early in the course of daily cancer treatment.

Postherpetic neuralgia (PHN) is characterized by persistent pain following the resolution of a herpes zoster rash. PHN is often resistant to treatment and can significantly reduce the patient's quality of life. Effective symptom relief is crucial and various treatments, including pharmacotherapy, have been attempted.

Article Synopsis
  • Vitamin B12 deficiency can lead to serious health issues, including macrocytic anemia and mental disorders, and its diagnosis can be challenging.
  • A case study reports a 37-year-old woman experiencing psychiatric symptoms after total gastrectomy due to impaired absorption of vitamin B12, which was verified by low blood levels of the vitamin.
  • The patient showed rapid improvement after starting vitamin B12 supplementation, highlighting the need to consider this deficiency in patients with a history of gastric surgery when they present with unexplained mental health changes.

Cardiotocography (CTG) is widely used to assess fetal well-being. CTG is typically obtained using ultrasound and autocorrelation methods, which extract periodicity from the signal to calculate the heart rate. However, during labor, maternal vessel pulsations can be measured, resulting in the output of the maternal heart rate (MHR).

Cardiotocography (CTG) measurements are critical for assessing fetal wellbeing during monitoring, and accurate assessment requires well-traceable CTG signals. The current FHR calculation algorithm, based on autocorrelation to Doppler ultrasound (DUS) signals, often results in periods of loss owing to its inability to differentiate signals. We hypothesized that classifying DUS signals by type could be a solution and proposed that an artificial intelligence (AI)-based approach could be used for classification.

In recent years, an increasing number of reports have demonstrated the usefulness of neoadjuvant chemoradiotherapy (NACRT). In our department, we consider cT3-4 and/or cN-positive locally advanced rectal cancer as an indication for NACRT. We have retrospectively evaluated the efficacy and safety of NACRT in 11 patients who underwent NACRT from November 2018 to July 2022.

Background: Intraperitoneal chemotherapy is promising for gastric cancer with peritoneal metastasis. Although a phase III study failed to show a statistically significant superiority of intraperitoneal paclitaxel combined with S-1 and intravenous paclitaxel, the sensitivity analysis suggested clinical efficacy. Thus, attempts to combine intraperitoneal paclitaxel with other systemic therapies with higher efficacy have been warranted.

Background: Follicular cholangitis (FC) is a benign bile duct disease that was first reported 2003. Pathologically, it is characterized by lymphoplasmacytic infiltration with multiple lymphoid follicle formations under the mucosal layer of the biliary tract. However, as this disease is extremely rare, little is known about its etiology and pathogenesis.

Objective: Appropriate monitoring and management of chemotherapy-induced nausea and vomiting (CINV) with prophylactic antiemetics is important for cancer patients. This study was performed to validate the clinical practice of antiemetic use with carboplatin-based chemotherapy in lung cancer patients in the Hokushin region (Toyama, Ishikawa, Fukui, and Nagano prefectures), Japan.

Methods: We surveyed retrospective data of newly diagnosed and registered lung cancer patients initially treated with carboplatin-based chemotherapy in 21 principal hospitals in the Hokushin region linked with health insurance claims data between 2016 and 2017.

A 76-year-old woman was diagnosed with left-sided transverse colon cancer invading the pancreatic tail with multiple liver metastases and peritoneal dissemination. Preoperative diagnosis was cT4b(SI)N2aM1c(H3, P1), cStage Ⅳc, harboring BRAF V600E mutation. Transverse colostomy was performed, and FOLFOXIRI plus bevacizumab(BEV)was administered.

Article Synopsis
  • Malignant pleural mesothelioma (MPM) is a serious cancer linked to occupational and environmental exposure, with a study conducted in the Hokushin region focusing on its clinical aspects and treatment.
  • Among the 166 newly diagnosed MPM patients, the majority were men with a median age of 69 years, and the study found varying incidence rates in different areas.
  • The study highlighted the challenges in diagnosing MPM, with many cases found incidentally, and emphasized the need for better multi-disciplinary care to improve treatment outcomes for this rapidly progressing disease.

  • Thymic carcinoma is a rare and aggressive cancer with a poor prognosis, and a study aimed to identify important clinical factors related to its diagnosis and treatment.
  • The analysis included data from 152,921 cancer patients across 22 hospitals, leading to the identification of 88 new cases, predominantly affecting older adults, with a gender breakdown of 50 men and 38 women.
  • The study highlighted difficulties in early diagnosis, with most cases being advanced at detection; it emphasized the need for better screening methods to catch thymic carcinoma earlier in its progression.

Heavy application of nitrogen (N) in tea (Camellia sinensis (L.)) plantations causes various environmental problems. To date, studies on N flows have been limited to the tea plantation level, but the crucial drivers of N flows are consumers, not farmers.

We discuss the counting of Nambu-Goldstone (NG) modes associated with the spontaneous breaking of higher-form global symmetries. Effective field theories of NG modes are developed based on symmetry-breaking patterns, using a generalized coset construction for higher-form symmetries. We derive a formula of the number of gapless NG modes, which involves expectation values of the commutators of conserved charges, possibly of different degrees.

Spectroscopic sensing provides physical and chemical information in a non-destructive and rapid manner. To develop non-destructive estimation methods of tea quality-related metabolites in fresh leaves, we estimated the contents of free amino acids, catechins, and caffeine in fresh tea leaves using visible to short-wave infrared hyperspectral reflectance data and machine learning algorithms. We acquired these data from approximately 200 new leaves with various status and then constructed the regression model in the combination of six spectral patterns with pre-processing and five algorithms.

Nondestructive techniques for estimating nitrogen (N) status are essential tools for optimizing N fertilization input and reducing the environmental impact of agricultural N management, especially in green tea cultivation, which is notably problematic. Previously, hyperspectral indices for chlorophyll (Chl) estimation, namely a green peak and red edge in the visible region, have been identified and used for N estimation because leaf N content closely related to Chl content in green leaves. Herein, datasets of N and Chl contents, and visible and near-infrared hyperspectral reflectance, derived from green leaves under various N nutrient conditions and albino yellow leaves were obtained.

Solid-state photoinduced coloring of boronium complexes consisting of 9-borabicyclononane and 2,2'-bipyridine with π-conjugated substituents at the 4,4'- or 5,5'-positions was investigated. The substitution position affected the highest occupied molecular orbital distribution and determined the coloring capability. The 4,4'-substituted complexes exhibited coloration upon irradiation, whereas most of the 5,5'-substituted complexes did not.

In agricultural soils fertilized with a high amount of ammonium nitrogen, the pH decreases because of the oxidation of ammonia by nitrifiers. Molecular-based analyses have revealed that members of the genus dominate over other nitrifiers in some acidic soils. However, terrestrial are rarely cultivated and little is known about their ecophysiology.
