Background/aim: To report the efficacy and tolerability of antitumour necrosis factor-alpha therapy (TNF inhibitors [TNFi]) in the management of non-infectious ocular inflammation, including uveitis and scleritis, in adult patients over an 8-year period.

Materials And Methods: This is a prospective cohort study of infliximab and adalimumab in the treatment of non-infectious ocular inflammatory disease. 43 of 85 adult patients on TNFi (34 infliximab, 9 adalimumab) for ≥1 year with non-infectious uveitis or scleritis were followed from 2006 to 2014.

Article Synopsis
  • The study aimed to identify specific ocular characteristics in patients with juvenile systemic granulomatous disease (JSGD) and uveitis, focusing on their clinical features and genetic mutations.
  • Researchers analyzed medical and imaging records of nine patients with JSGD, noting that most experienced bilateral uveitis and various forms of inflammation in the eyes.
  • Findings suggest that chronic bilateral panuveitis and a unique nodular appearance around the optic disc are key indicators of JSGD, highlighting the importance of genetic testing for the NOD2 gene in affected individuals.

Article Synopsis
  • This study aimed to evaluate the safety and effectiveness of a noncorticosteroid triple immunosuppressive therapy in children suffering from chronic noninfectious uveitis that did not respond to previous treatments.
  • Researchers analyzed data from patients under 16 who had been on the triple therapy for at least 6 months after other treatments failed, tracking various medical details and outcomes at the 6-month mark.
  • The findings showed only a moderate improvement in inflammation for a small percentage of patients, with a notable number experiencing significant infections, indicating limited benefits from the additional immunosuppressive treatment.

Objective: To assess the use of adalimumab in the treatment of refractory non-infectious childhood chronic uveitis.

Methods: A case cohort interventional study was performed on patients with uveitis, who were treated with adalimumab after failure of treatment with a combination of corticosteroids and another immunosuppressant drug. Main outcome measures were (i) stability of vision, (ii) stability of inflammation and (iii) reduction of immunosuppressive load.

Objectives: Although patients with Locked-In-Syndrome (LIS) are often stated to have normal cognitive functioning, the few reports of neuropsychological assessment in these cases suggest this is not always true. This paper (a) reviews published reports of neuropsychological assessments of LIS patients, (b) presents a detailed neuropsychological assessment of a patient to determine if she has normal cognitive functioning and (c) presents the views of the patient on what has happened to her.

Methods And Procedures: An in-depth single case report assessing the cognitive and emotional functioning of a young woman with LIS plus a personal account from the patient.
