"Wound, Pressure Ulcer, and Burn Guidelines-5: Guidelines for the management of lower leg ulcers and varicose veins, second edition" is revised from the first edition, which was published in the Japanese Journal of Dermatology in 2011. The guidelines were drafted by the Wound, Pressure Ulcer, and Burn Guidelines Drafting Committee delegated by the Japanese Dermatological Association and intend to facilitate physicians' clinical decisions in preventing, diagnosing and management of lower leg ulcers and varicose veins. We updated all sections by collecting documents published since the publication of the first edition.

Article Synopsis
  • Psoriasis, a complicated skin condition linked to inflammation, has unclear connections between its severity, related cytokine levels, and heart disease risk.
  • This study aimed to identify serum markers to help monitor cardiovascular disease (CVD) risk in psoriasis patients by examining relationships between cytokines and coronary artery health.
  • Key findings highlight that specific cytokines (IL-17A, IL-19, IL-36) correlate with psoriasis severity, while markers like CD31 and resistin show promise for tracking coronary atherosclerosis progression, particularly in patients with psoriasis vulgaris.

Immune checkpoint inhibitors (ICIs) can cause immune-related adverse events (irAEs) in various organs. Herein, we present a rare case involving a patient with immunotherapy-related gastritis who developed refractory vomiting and anorexia after treatment with nivolumab for cutaneous malignant melanoma. The patient was treated with oral corticosteroids, but the efficacy was limited.

Article Synopsis
  • The study investigates the phenomenon of basopenia (decreased basophils in blood) occurring during urticaria (allergic skin reaction), aiming to replicate this in an IgE-mediated mouse model.* -
  • Using TNP-specific IgE and stimulation with TNP-ovalbumin, researchers observed immediate ear swelling and reduced basophils in the blood and skin lesions, while more basophils were found in the lymph nodes.* -
  • Findings suggest that during urticaria, basophils migrate to lymph nodes as part of the immune response, indicating the need to consider systemic immune interactions while studying urticaria pathogenesis in humans.*

Article Synopsis
  • Immuno-oncology (IO) drugs are critical for cancer treatment, but older patients face safety issues, including higher hospitalization rates despite similar rates of immune-related adverse events (irAEs) compared to younger groups.* -
  • A study analyzing clinical data from 181 patients over 80 years old found that 35% experienced irAEs, with 19% discontinuing therapy; elevated eosinophil counts were identified as a significant risk factor for developing these events.* -
  • Overall, IO therapy is deemed safe for older patients, but those starting treatment with high eosinophil counts require careful monitoring.*

Article Synopsis
  • Sarcoidosis is a disease where granulomas (clusters of immune cells) form in the body, and current treatments mainly involve glucocorticoids, with no alternative medications approved yet.
  • Researchers used single-cell RNA sequencing to study sarcoidosis patients and found that certain macrophages (a type of immune cell) associated with the disease were more abundant in skin lesions and exhibited increased metabolism, particularly in a pathway called the pentose phosphate pathway (PPP).
  • It was discovered that inhibiting enzymes from the PPP could reduce granuloma formation in both laboratory cell models and animal studies, suggesting that targeting this pathway could lead to new treatments for sarcoidosis.

Psoriasis is known as an independent risk factor for cardiovascular disease due to its chronic inflammation. Studies have been conducted to evaluate the progress of atherosclerotic plaques in psoriasis. However, inadequate efforts have been made to clarify the relationship between atherosclerosis progress in coronary arteries and other important blood vessels.

A decrease in the number of basophils in the peripheral blood, or basopenia, has been noted, reflecting the activity of chronic spontaneous urticaria (CSU). Infiltration of basophils into the skin has also been reported, but the mechanism of basopenia in CSU has not been clarified. The phenomenon of basopenia during the active phase of urticaria was confirmed, and basophil numbers increased following symptom improvement in 15 out of 17 patients treated with omalizumab and in 13 of 15 patients treated with antihistamines.

Palisaded neutrophilic and granulomatous dermatitis (PNGD) is a relatively rare skin disease that is characterized by a reactive granulomatous histopathological pattern and is often associated with systemic autoimmune diseases. We encountered a case of PNGD that presented with pustules, although the prototypical clinical presentation of PNGD is mainly erythema and papules. Here, this rare case of PNGD with pustules is presented and discussed in relation to the relevant literature.

Pseudoxanthoma elasticum (PXE) is a progressive hereditary disease that affects tissues such as the skin, retina, blood vessels, and gastrointestinal tracts. Therefore, comprehensive medical care across clinical departments specialized in specific organs is needed to provide the best clinical practices to PXE patients. The Japanese version of clinical guidelines developed by the Japanese Dermatological Association was published in 2017, and aimed to promote equal accessibility of PXE-related medical care.

Psoriasis is a systemic inflammatory disease known to affect survival in the presence of cerebral or cardiovascular comorbidities. However, no clear guidelines have been defined regarding the extent of vascular lesion testing that should be performed in patients with psoriasis. We therefore performed coronary computed tomography angiography (CCTA) in 88 Japanese patients with psoriasis who visited Kansai Medical University Hospital between 2015 and 2019 and determined the ankle-brachial pressure index (ABI) for 44 of these patients.
