Robotic hiatus hernia surgery: learning curve and lessons learned.

New procedures like the robotic approach require proficiency to ensure patient safety and satisfactory functional results. Hiatal hernia surgery serves as a suitable training procedure for upper gastrointestinal tract surgeons transitioning to the robotic approach. This study aims to evaluate the outcomes of implementing the robotic approach in hiatal hernia surgery at a tertiary hospital and to assess the associated learning curve.

DOse and administration Time of Indocyanine Green in near-infrared fluorescence cholangiography during laparoscopic cholecystectomy (DOTIG): a randomized clinical trial.

Background: Different techniques have been proposed to reduce the incidence of the intraoperative bile duct injury during laparoscopic cholecystectomy (LC). Among these, Near-Infrared Fluorescence Cholangiography (NIFC) with Indocyanine Green (ICG) represents a relatively recent addition. At present, there is considerable variation in the protocols for the administration of ICG.

Study of the Prevalence of Atypical Scheuermann's Kyphosis Using Computed Tomography Scans.

 The aim of the present study was to detect atypical Scheuermann's disease through computed tomography scans and estimate its prevalence.  This cross-sectional observational study involved 1,287 computed tomography scans from patients aged 18 to 40 years of both genders. The criteria for diagnosing atypical Scheuermann's disease included wedging of 5° in 3 consecutive vertebrae, combined with a total Cobb angle of 10° or more within the thoracolumbar interval from T8 to L2.

Relationship between a Radiographic Grading Scale of Degenerative Lumbar Disease and Quality of Life.

 To evaluate the correlation between a radiographic scale of lumbar degenerative disease and the Oswestry Disability Index (ODI).  A cross-sectional study in which the ODI quality of life questionnaire and the radiographic parameters for the classification of lumbar degenerative disease into different grades were compared to try to establish a relationship between them.  The relationship between the radiographic parameters and quality of life indicators does not behave homogeneously, considering the different grades of the lumbar grading scale.

Gestational breast cancer: distinctive molecular and clinico-epidemiological features.

Gestational breast cancer (GBC), defined as breast cancer (BC) diagnosed during pregnancy or the first-year post-partum, accounts for 6-15% of BC cases in women aged 20-44 years. GBC has worse prognosis than non-GBC, but reasons behind are not clear. The GEICAM/2012-03 Study (Molecular Characterization of Gestational Breast Cancer) is a multicenter prospective/retrospective observational registry of patients diagnosed with GBC.

QUANTITATIVE AND QUALITATIVE INVESTIGATION OF THREE-DIMENSIONAL GAIT ANALYSIS IN PATIENTS WITH CERVICAL MYELOPATHY.

Objective: This study aims to describe a kinematic gait assessment protocol and identify its main alterations in individuals with cervical spondylotic myelopathy (CSM) compared to healthy patients.

Methods: In total, 14 patients diagnosed with CSM were enrolled and submitted to a three-dimensional gait analysis. The movement of patients was captured with infrared emission cameras that identified tracking markers placed on the lower limbs.

Leukocyte-Poor Platelet-Rich Plasma as a Treatment for Patellar Tendinopathy A Multicenter, Randomized Controlled Trial.

Purpose: The purpose of this study was to compare regimens of eccentric exercise and dry-needling, with and without an ultrasound-guided leukocyte-poor platelet-rich plasma (LP-PRP) injection, in patients with patellar tendinopathy.

Methods: Patients with symptomatic patellar tendinopathy based on physical examination and magnetic resonance imaging and who had failed at least 6 weeks of nonoperative treatment were enrolled and randomized at two centers to receive ultrasound-guided dry-needling (DN) alone or in addition to an injection of LP-PRP coupled with standardized eccentric strengthening exercises. Participants completed patient-reported outcome surveys at baseline and at 3, 6, 9, 12, and 26 weeks post-treatment.

Factors Associated with Patient Satisfaction After Periacetabular Osteotomy.

Background: The periacetabular osteotomy (PAO) was developed to treat acetabular dysplasia and avoid the progression of osteoarthritis. Prior research has correlated demographic and radiographic variables with postoperative patient outcomes but with limited focus on patient expectations and correlation with satisfaction. The purpose of this study was to determine whether any such associations exist with the PAO.

Evaluation of the use of tranexamic acid in the postoperative period in patients with scoliosis undergoing posterior fusion.

Scoliosis is the most prevalent type of spinal deformity, with a 2-3% prevalence in the general population. Moreover, surgery for scoliotic deformity may result in severe blood loss and, consequently, the need for blood transfusions, thereby increasing surgical morbidity and the rate of complications. Several antifibrinolytic drugs, such as tranexamic acid, have been regarded as safe and effective options for reducing blood loss.

Deciphering the impact of STAT3 activation mediated by PTPRT promoter hypermethylation as biomarker of response to paclitaxel-plus-cetuximab in patients with recurrent or metastatic squamous cell carcinoma of the head and neck.

Background: Squamous cell carcinoma of the head and neck (SCCHN) is an aggressive disease with poor prognosis. It is known that the activation of STAT3 signaling pathways promotes the development and progression of this neoplasia and it has been described the role of PTPRT as a negative regulator of STAT3. Then, we have evaluated the impact of them as biomarkers of outcome in a series of patients with recurrent and/or metastatic SCCHN treated with weekly paclitaxel-plus-cetuximab (ERBITAX) regimen.

Nanofluidic resistive pulse sensing for characterization of extracellular vesicles.

This paper describes the development, design and characterization of a resistive pulse sensing (RPS) system for the analysis of size distributions of extracellular vesicles (EVs). The system is based on microfluidic chips fabricated using soft-lithography and operated in pressure-driven mode. This fabrication approach provided reproducible pore dimensions and the best performing chip design enabled, without calibration, sizing of both 252 nm and 460 nm test particles within 8% of theoretically calculated values, based on the size specifications provided by suppliers.

Direct Numerical Analysis of Dynamic Facilitation in Glass-Forming Liquids.

We propose a computational strategy to quantify the temperature evolution of the timescales and length scales over which dynamic facilitation affects the relaxation dynamics of glass-forming liquids at low temperatures, which requires no assumption about the nature of the dynamics. In two glass models, we find that dynamic facilitation depends strongly on temperature, leading to a subdiffusive spreading of relaxation events which we characterize using a temperature-dependent dynamic exponent. We also establish that this temperature evolution represents a major contribution to the increase of the structural relaxation time.

Assessment of inter- and intraobserver agreement for META score in distinguishing osteoporotic from multiple myeloma vertebral fractures.

Purpose: To conduct an independent assessment of inter- and intraobserver agreement for the META score as a tool for differentiating osteoporotic vertebral fractures and multiple myeloma vertebral fractures.

Methods: This is a retrospective observational study. The magnetic resonance imaging analysis was made by two independent spinal surgeons.

Binding and Stabilization of a Semiquinone Radical by an Artificial Metalloenzyme Containing a Binuclear Copper (II) Cofactor.

A binuclear Cu(II) cofactor was covalently bound to a lauric acid anchor. The resulting conjugate was characterized then combined with beta-lactoglobulin (βLG) to generate a new biohybrid following the so-called "Trojan horse" strategy. This biohybrid was examined for its effectiveness in the oxidation of a catechol derivative to the corresponding quinone.

Proton Domino Reactions at an Imidazole Relay Control the Oxidation of a Tyr-His Artificial Mimic of Photosystem II.

A close mimic of P680 and the Tyrosine-Histidine pair in photosystem II (PS II) has been synthesized using a ruthenium chromophore and imidazole-phenol ligands. The intramolecular oxidation of the ligands by the photoproduced Ru(III) species is characterized by a small driving force, very similar to PS II where the complexity of kinetics was attributed to the reversibility of electron transfer steps. Laser flash photolysis revealed biphasic kinetics for ligand oxidation.

Percutaneous Pedicle Screw for Thoracolumbar Fractures: A Long-term Follow-up.

 This is a retrospective cohort study to analyze the long-term outcomes of thoracolumbar spine fracture patients who underwent minimally invasive percutaneous fixation.  The cases of 17 patients with thoracolumbar spine fractures who had percutaneous fixation between 2009 and 2011 were the subject of a retrospective analysis. Clinical and radiographic variables were collected.

Efficacy of Dynamic Magnetic Resonance Imaging in the Diagnosis of Degenerative Cervical Myelopathy: Systematic Review Protocol.

Cervical degenerative myelopathy (CDM) is a cervical spine condition resulting in clinical manifestations of spinal cord compression related to the chronic, non-traumatic, and progressive narrowing of the cervical spinal canal. Conventional magnetic resonance imaging (MRI) is the gold standard test to diagnose and assess the severity of CDM. However, the patient is in a neutral and static position during the MRI scan, which may devalue the dynamic factors of CDM, underestimating the risk of spinal cord injury related to cervical spine flexion and extension movements.

Translation and Cross-cultural Adaptation of the SOSG-OQ 2.0 Questionnaire into Brazilian Portuguese.

 To perform the cross-cultural adaptation and translation into Brazilian Portuguese of the Spine Oncology Study Group - Outcomes Questionnaire 2.0 (SOSG-OQ 2.0) to enable its application to Brazilian patients and to allow Brazilian researchers to use a questionnaire that is on trend in the scientific literature.

The Role of Distraction Osteogenesis in Limb Salvage for Tumors.

Modern technology and advances in medicine have facilitated increasing rates of limb salvage in the treatment of sarcomas. Orthopedic oncologists have a wide array of reconstruction options for limb salvage, ranging from allografts to endoprosthesis reconstruction. Limb lengthening is another option available to an orthopedic oncologist faced with bony defects and limb length discrepancies following resection.

Although gait is one of the most globally ubiquitous concepts-traversing all geographic, cultural, and language barriers-it is often seen as an overwhelming and confusing concept. This review describes the phases and components of gait to help the clinician identify what is normal, evaluate what is not normal, and understand some common pathologic gait patterns seen in the different orthopedic subspecialties.

The verifiability of a suspect's alibi is often interpreted as a sign of innocence. Because the police resources are limited, verifiability could be used to dismiss suspects of minor offenses. We examined whether alibi verifiability actually indicates innocence for minor crimes.
