Cost-effectiveness analysis of recruitment strategies in a large diabetes prevention trial conducted across two sites in Sydney, Australia.

Background: Diabetes prevention trials require large samples and community-based recruitment, which can be protracted and expensive. We analysed the cost-effectiveness of recruitment strategies used in a randomised placebo-controlled supplement trial in adults with prediabetes and overweight or obesity conducted in Sydney, Australia.

Methods: Recruitment strategies included advertising through local radio stations and newspapers, television news coverage, online advertising and editorials, advertising in and referral from primary care settings, university- and hospital-based advertising, and attending or hosting local events.

Conceptualizing the peer contribution in Open Dialogue practice.

In English mental health services, people with their own experience of mental distress have trained as Open Dialogue practitioners and have been employed as peer practitioners, co-working as equals alongside workers with professional backgrounds in Network Meetings. The conceptual underpinnings of the peer practitioner role have been drawn from the principles and relational approach of Intentional Peer Support. These have significant similarities with Open Dialogue, in terms of philosophical and theoretical orientations, with a particular focus on what happens in the "between" of a relational encounter.

A Qualitative Process Evaluation of Participant Experiences in a Feasibility Randomised Controlled Trial to Reduce Indulgent Foods and Beverages.

There is a growing interest in the effects of ultra-processed/energy-dense nutrient-poor foods on health outcomes, and few interventions to reduce their consumption have been tested. We tested a simple intervention to help people reduce the indulgences they consume (energy-dense nutrient-poor (EDNP) foods). Herein, we report the qualitative findings to understand how participants reduced their consumption by exploring intervention fidelity and the factors affecting consumption.

Defluoroalkylation of Trifluoromethylarenes with Hydrazones: Rapid Access to Benzylic Difluoroarylethylamines.

Here, we report an efficient and modular approach toward the formation of difluorinated arylethylamines from simple aldehyde-derived -dialkylhydrazones and trifluoromethylarenes (CF-arenes). This method relies on selective C-F bond cleavage via reduction of the CF-arene. We show that a diverse set of CF-arenes and CF-heteroarenes react smoothly with a range of aryl and alkyl hydrazones.

The neuropeptide galanin adopts an irregular secondary structure.

Human galanin is a 30-residue neuropeptide targeted for development of analgesics, antidepressants, and anticonvulsants. While previous work from our group and others has already produced significant insights into galanin's N-terminal region, no extant structures of galanin in databases include its full-length sequence and the function of its C-terminus remains ambiguous. We report the NMR solution structure of full-length human galanin C-terminal amide, determined from 2D H-H COSY, TOCSY, and ROESY NMR data.

Minimal Increments of Hydrophobic Collapse within the N-Terminus of the Neuropeptide Galanin.

The neuropeptide galanin has a 35-year history as an intriguing target in drug design owing to its implication as a potential anticonvulsant and neuronal trophic factor among many other therapeutically interesting functions including analgesia and mood alteration. In this study, we report the structural characterization of three synthetic fragments of the galanin N-terminus in buffered aqueous solution: hGal(2-12)KK, hGal(1-12)KK, and hGal(1-17)KK. High-field two-dimensional H-H nuclear magnetic resonance (NMR) data were acquired for these fragments and used to derive distance restraints.

Switchable Regioselective 6- or 5- Radical Cyclization via Photoredox Catalysis.

Controlling the regioselectivity of radical cyclizations to favor the 6- mode over its kinetically preferred 5- counterpart is difficult without introducing substrate prefunctionalization. To address this challenge, we have developed a simple method for reagent controlled regioselective radical cyclization of halogenated heterocycles onto pendant olefins. Radical generation occurs under mild photoredox conditions with control of the regioselectivity governed by the rate of hydrogen atom transfer (HAT).

A short-form version of the Australian English Communicative Development Inventory.

The Australian English Communicative Development Inventory (OZI) is a 558-item parent report tool for assessing language development at 12-30 months. Here, we introduce the short form (OZI-SF), a 100-item, picture-supported, online instrument with substantially lower time and literacy demands. In tool development (Study 1), 95 items were drawn from the OZI to match its item distribution by age of acquisition and semantic categories.

Radical Chain Reduction via Carbon Dioxide Radical Anion (CO).

We developed an effective method for reductive radical formation that utilizes the radical anion of carbon dioxide (CO) as a powerful single electron reductant. Through a polarity matched hydrogen atom transfer (HAT) between an electrophilic radical and a formate salt, CO formation occurs as a key element in a new radical chain reaction. Here, radical chain initiation can be performed through photochemical or thermal means, and we illustrate the ability of this approach to accomplish reductive activation of a range of substrate classes.

Effects of α-Cyclodextrin on Cholesterol Control and Hydrolyzed Ginseng Extract on Glycemic Control in People With Prediabetes: A Randomized Clinical Trial.

Importance: Effective strategies for preventing type 2 diabetes are needed. Many people turn to complementary medicines, but there is little well-conducted scientific evidence to support their use.

Objective: To assess the efficacy of α-cyclodextrin for cholesterol control and that of hydrolyzed ginseng for glycemic control in people with prediabetes and overweight or obesity.

Targeting the Intestinal Microbiota to Prevent Type 2 Diabetes and Enhance the Effect of Metformin on Glycaemia: A Randomised Controlled Pilot Study.

Early treatment may prevent or delay the onset of type 2 diabetes mellitus (T2DM) in individuals who are at high risk. Lifestyle interventions and the hypoglycemic drug metformin have been shown to reduce T2DM incidence. The effectiveness of such interventions may be enhanced by targeting environmental factors such as the intestinal microbiota, which has been proven to predict the response to lifestyle interventions and play a part in mediating the glucose-lowering effects of metformin.

Effects of alpha-cyclodextrin on cholesterol control and Compound K on glycaemic control in people with pre-diabetes: Protocol for a Phase III randomized controlled trial.

The prevalence of pre-diabetes and of type 2 diabetes mellitus is increasing. Preventing disease progression is important to improve outcomes. Natural products are becoming popular alternatives to pharmaceutical products for preventative health and treatment of disease; however, the evidence to support the use of natural alternatives for pre-diabetes and type 2 diabetes is lacking.

'Say no': a feasibility trial of a brief intervention to reduce instances of indulgent energy-intake episodes.

The aim of this study is to examine the feasibility of a brief intervention to reduce instances of indulgent energy intake. Forty-five participants with a body mass index (BMI) ≥25 kg m- were randomized to one of three groups for 8 weeks. The control group was asked to complete a questionnaire every 4 days, the self-monitoring group was given the same instructions but also asked to 'say no' to indulgences.

Public support for government regulatory interventions for overweight and obesity in Australia.

Background: There is growing recognition among public health circles of the need for regulatory action for overweight and obesity, but there has been limited research into whether the Australian public supports government intervention. This study aimed to determine the level of public support for food-related regulations for obesity, and to assess the determinants of support.

Methods: A nationally representative sample of Australian adults (n = 2011) was recruited by market research company Online Research Unit to complete an online survey.

Transplant-related experience in pharyngo-oesophageal reconstruction: use of organ preservation techniques to improve ischaemic tolerance in free jejunal flaps.

Reconstruction of pharyngo-oesophageal defects remains a surgical challenge. Free jejunal flaps, first described by Seidenberg in 1959, are used to reconstruct circumferential defects, but their main disadvantage is sensitivity to ischaemia. Others are secretions, an unpleasant smell, and problems at the donor site.

Antarctic lakes suggest millennial reorganizations of Southern Hemisphere atmospheric and oceanic circulation.

The phasing of millennial-scale oscillations in Antarctica relative to those elsewhere in the world is important for discriminating among models for abrupt climate change, particularly those involving the Southern Ocean. However, records of millennial-scale variability from Antarctica dating to the last glacial maximum are rare and rely heavily on data from widely spaced ice cores, some of which show little variability through that time. Here, we present new data from closed-basin lakes in the Dry Valleys region of East Antarctica that show high-magnitude, high-frequency oscillations in surface level during the late Pleistocene synchronous with climate fluctuations elsewhere in the Southern Hemisphere.

A randomized, multiple-dose parallel study to compare the pharmacokinetic parameters of synthetic conjugated estrogens, A, administered as oral tablet or vaginal cream.

Objective: A randomized, parallel-design study was conducted to determine the pharmacokinetic profile of synthetic conjugated estrogens A (SCE-A) vaginal cream (0.625 mg SCE-A/g) when administered at intervals (1 g once daily for 7 d, then twice weekly) over a 27-day period as compared with the pharmacokinetic profile of 0.3 mg SCE-A tablets administered once daily orally for 27 days.

Oroantral fistula from bisphosphonate induced osteonecrosis of the jaw.

Bisphosphonates like alendronic acid, disodium etidronate, and risedronate are effective for preventing postmenopausal and corticosteroid induced osteoporosis. They are also useful in the treatment of Paget's disease, hypercalcaemia of malignancy and in bony metastases. However osteonecrosis of the jaw has been reported following intravenous bisphosphonate use and rarely in those taking them orally.

Effects on serum hormone levels of low-dose estrogen in place of placebo during the hormone-free interval of an oral contraceptive.

Background: The study was conducted to evaluate the effects of low-dose estrogen compared to placebo on ovarian activity during the traditional 7-day hormone-free interval (HFI) of an oral contraceptive (OC).

Study Design: Women were randomized to placebo or low-dose estrogen for 7 days during the HFI. Serum levels of estradiol, follicle-stimulating hormone (FSH), luteinizing hormone and inhibin B were obtained before, during and after treatment.

Regional insolation forcing of late Quaternary climate change in the Southern Hemisphere.

In agreement with the Milankovitch orbital forcing hypothesis it is often assumed that glacial-interglacial climate transitions occurred synchronously in the Northern and Southern hemispheres of the Earth. It is difficult to test this assumption, because of the paucity of long, continuous climate records from the Southern Hemisphere that have not been dated by tuning them to the presumed Northern Hemisphere signals. Here we present an independently dated terrestrial pollen record from a peat bog on South Island, New Zealand, to investigate global and local factors in Southern Hemisphere climate changes during the last two glacial-interglacial cycles.

Surgical anatomy of the buccal nerve.

The buccal nerve may be damaged during surgical procedures which require an incision along the external oblique ridge of the mandible and this study was undertaken to clarify its surgical anatomy. The course and relationships of the nerve were determined in 20 formalin-fixed cadaver specimens. The number of major branches of the nerve ranged from 4 to 8 and a mean of 3 branches was present as the nerve crossed the external oblique ridge.

The sensory distribution of the buccal nerve.

The buccal nerve is frequently anaesthetised, and occasionally cut during surgical procedures, but its distribution is poorly documented. The aim of this study was to determine its distribution by mapping areas of sensory loss following administration of a local anaesthetic solution. Observations in 40 patients revealed marked variation in the area supplied solely by the buccal nerve and a substantial overlap with the area supplied by the inferior alveolar nerve.
