A 62-year-old man was diagnosed with Stage Ⅲ signet ring cell carcinoma of the lower thoracic esophagus. The patient underwent 2 courses of neoadjuvant cisplatin and 5-fluorouracil(SP therapy), demonstrating stable efficacy. Subsequently, the patient underwent subtotal esophagectomy with thoracoabdominal 2-field lymphadenectomy via right thoracotomy, followed by esophageal reconstruction using a gastric tube through a retrosternal route.

Background: Killian-Jamieson diverticulum (KJD) is a rare diverticulum arising from a muscular gap in the anterolateral wall of the proximal cervical esophagus. The first choice of treatment for KJD remains controversial due to its rare incidence. He became febrile on day 7, and pulmonary failure and multi-organ failure developed subsequently, requiring mechanical ventilation. Chest X-ray and CT scan demonstrated diffuse interstitial shadows, suggesting the development of idiopathic pneumonia syndrome.
