Venovenous extracorporeal life support after pulmonary endarterectomy: indications, techniques, and outcomes.

Background: Pulmonary endarterectomy is the accepted therapy for thromboembolic pulmonary hypertension. A recognized complication of this surgery is the postoperative development of reperfusion edema, a potentially fatal cause of respiratory failure. Because reperfusion edema can be a reversible process, temporizing support measures may be life saving.

Attention as a controller.

We investigate, by constructing suitable models, the manner in which attention and executive function are observed to interact, including some aspects of the influence of value/emotion on this interaction. Attention is modelled using the recent engineering control model (Corollary Discharge of Attention Movement, CODAM), which includes suitable working memory components. We extend this model to take account of various executive functions performed in working memory under attention control, such as rehearsal, substitution and transformation of buffered activity.

The micro-structure of attention.

We investigate three possible methods of specifying the microstructure of attention feedback: contrast gain, additive and output gain, using simple single node and 3-layer cortical models composed of graded or spiking neurons. Contrast gain and additive attention are also tested in a spiking network which is simplified by mean field methods. The simulation task uses two stimuli, probe and reference, presented singly or together within the neuronal receptive fields whilst attention is directed towards or away from the receptive field.

Ionic Contra-Viral Therapy (ICVT); a new approach to the treatment of DNA virus infections.

The sequestration of cellular K(+) has been shown elsewhere to elicit a broad spectrum of antiviral activity. The obligatory, coupled cotransports of Na(+), K(+) and Cl(-) (NKCC1) and of Na(+) and K(+) (NKATPase) effect net cellular K(+) influx. We examined the effects of specific inhibitors of these transports; a cardiac glycoside (Digoxin) and a loop diuretic (Furosemide) on virus replication in vitro.

High-dose inhaled fluticasone does not replace oral prednisolone in children with mild to moderate acute asthma.

Background: Inhaled corticosteroids are not as effective as oral corticosteroids in school-aged children with severe acute asthma. It is uncertain how inhaled corticosteroids compare with oral corticosteroids in mild to moderate exacerbations.

Primary Objective: The purpose of this work was to determine whether there is a significant difference in the percentage of predicted forced expiratory volume in 1 second in children with mild to moderate acute asthma treated with either inhaled fluticasone or oral prednisolone.

Hepatocellular carcinoma occurring in a patient with Crohn's disease treated with both azathioprine and infliximab.

Hepatocellular carcinoma is known to be associated with underlying liver diseases, such as cirrhosis, hemochromatosis, and chronic viral hepatitis. All reported cases of hepatocellular carcinoma in association with Crohn's disease involve patients treated previously with azathioprine or both azathioprine and steroids. However, hepatocellular carcinoma associated with the use of azathioprine and infliximab has not been reported.

Toxoplasma gondii infection in two common wombats (Vombatus ursinus).

Toxoplasmosis is a common cause of death in wild and captive Australian marsupials, yet descriptions of clinical disease, diagnosis and pathological lesions are limited and incomplete. Infection with Toxoplasma gondii was diagnosed, using immunohistochemical techniques, following the acute death of a juvenile common wombat, Vombatus ursinus, that was being bottle raised by wildlife carers. This animal's cage mate developed dyspnoea, tachycardia and anorexia 3 weeks later.

Impact of early vs delayed admission to rehabilitation on functional outcomes in persons with stroke.

Objective: Delayed admission to rehabilitation may result in poorer outcomes by reducing exposure to therapeutic interventions at a time when the brain is primed for neurological recovery. The present study examined the effects of early vs delayed admission on functional outcome and length of stay in patients admitted to a rehabilitation unit for first-ever unilateral stroke.

Design: Retrospective chart review.

Mini-dose glucagon rescue for mild hypoglycaemia in children with type 1 diabetes: the Brisbane experience.

Objectives: To evaluate the use of small doses of glucagon using an insulin syringe in mild or impending hypoglycaemia in children with type 1 diabetes.

Methods: Data were collected from patients attending the Paediatric Diabetes Clinic at the Queensland Diabetes Centre at the Mater Hospital, Brisbane in 2002-2004 following the institution of a new protocol for home management of mild or impending hypoglycaemia associated with inability or refusal to take oral carbohydrate. The protocol recommended the use of subcutaneous injections of glucagon using insulin syringes at a dose of two 'units' (20 microg) in children 2 years of age or younger, and for older children one unit per year of age up to a maximum of 15 units (150 microg), with an additional doubled dose given if the blood glucose had not increased in 20 min.

Randomized clinical trial comparing lightweight composite mesh with polyester or polypropylene mesh for incisional hernia repair.

Background: Polymer mesh has been used to repair incisional hernias with lower recurrence rates than suture repair. A new generation of mesh has been developed with reduced polypropylene mass and increased pore size. The aim of this study was to compare standard mesh with new lightweight mesh in patients undergoing incisional hernia repair.

Diagnosis of suspected hypovitaminosis A using magnetic resonance imaging in African lions (Panthera leo).

Vitamin A deficiency is described in captive lions. Ante mortem diagnosis can either be made by serum analysis or liver biopsy, both of which may be problematic. This study utilised magnetic resonance imaging to identify vitamin A deficiency in lions with relatively mild clinical signs, which could otherwise be attributed to numerous other neurological conditions.

Pathogenesis of Yersinia pestis infection in BALB/c mice: effects on host macrophages and neutrophils.

The pathogenesis of infection with Yersinia pestis, the causative agent of plague, was examined following subcutaneous infection of BALB/c mice with a fully virulent strain expressing green fluorescent protein. Plate culturing, flow cytometry, and laser confocal microscopy of spleen homogenates throughout infection revealed three discernible stages of infection. The early phase was characterized by the presence of a small number of intracellular bacteria mostly within CD11b+ macrophages and Ly-6G+ neutrophils.

Subfield variations in hippocampal processing-components of a spatial navigation system.

The hippocampus is a part of the brain strongly linked to spatial exploration. Within it exist 'place cells' which fire preferentially when an animal is in certain regions of physical space. Recent research has shown that these place cells and their corresponding representations of space behave differently in the CA3 and CA1 subfields of the hippocampus.

Spinal spondylosis and acute intervertebral disc prolapse in a European brown bear (Ursus arctos arctos).

A 22-year-old male European brown bear (Ursus arctos arctos) was presented to the Onderstepoort Veterinary Academic Hospital after an acute onset of hind limb paralysis 4 days earlier. Previous radiographs revealed marked degenerative joint disease of the stifles, tarsi and digits. The clinical findings were consistent with acute disc prolapse.

Off pump coronary artery bypass grafting in a patient with tracheopathia osteoplastica.

Tracheopathia osteoplastica is a rare disease of the tracheobronchial tree, which is often misdiagnosed and recognised only at post-mortem. We report the case of a 66-year-old man with this condition, mistakenly thought to be suffering from chronic bronchiectasis. He underwent successful coronary artery bypass grafting for unstable angina using an off-pump technique.

Fetal and neonatal exposure to AZT and low-protein diet affects glucose homeostasis: a model with implications for AIDS prevention.

Zidovudine (AZT) lowers the perinatal transmission of HIV but can impair mitochondrial function by depleting mitochondrial DNA (mtDNA). AZT therapy and perinatal nutritional deprivation affect the body fat distribution, which influences glucose tolerance. We sought to model intrauterine exposure to AZT in humans to determine whether it interacts with low-protein diet (LPD) to impact on birth weight and glucose homeostasis in the offspring.

Antibiotic-free plasmid stabilization by operator-repressor titration for vaccine delivery by using live Salmonella enterica Serovar typhimurium.

Live, attenuated bacteria are effective vectors for heterologous antigen delivery. However, loss of heterologous gene-bearing plasmids is problematic, and antibiotics and their resistance genes are not desirable for in vivo DNA vaccine delivery due to biosafety and regulatory concerns. To solve this problem, we engineered the first vaccine delivery strain that has no requirement for antibiotics or other selectable marker genes to maintain the recombinant plasmid.

VP22 enhances antibody responses from DNA vaccines but not by intercellular spread.

In some species DNA vaccines elicit potent humoral and cellular immune responses. However, their performance in humans and non-human primates is less impressive. There are suggestions in the literature that an increase in the intercellular distribution of protein expressed from a DNA vaccine may enhance immunogenicity.

A type IV pilin, PilA, Contributes To Adherence of Burkholderia pseudomallei and virulence in vivo.

The Burkholderia pseudomallei K96243 genome contains multiple type IV pilin-associated loci, including one encoding a putative pilus structural protein (pilA). A pilA deletion mutant has reduced adherence to human epithelial cells and is less virulent in the nematode model of virulence and the murine model of melioidosis, suggesting a role for type IV pili in B. pseudomallei virulence.

Survival of patients with periampullary carcinoma is predicted by lymph node 8a but not by lymph node 16b1 status.

Background: The aim of this study was to assess the impact of metastatic disease in lymph nodes 8a and 16b1 (as defined by the Japanese Pancreas Society) on survival in patients with periampullary malignancy.

Methods: Patients undergoing resection for primary pancreatic ductal adenocarcinoma or intrapancreatic bile duct adenocarcinoma were identified from a prospective database (September 1997-May 2003).

Results: Thirteen of 54 and ten of 44 evaluable patients had metastatic involvement of lymph nodes 8a and 16b1 respectively.

Cytotoxic ribosome-inactivating lectins from plants.

A class of heterodimeric plant proteins consisting of a carbohydrate-binding B-chain and an enzymatic A-chain which act on ribosomes to inhibit protein synthesis are amongst the most toxic substances known. The best known example of such a toxic lectin is ricin, produced by the seeds of the castor oil plant, Ricinnus communis. For ricin to reach its substrate in the cytosol, it must be endocytosed, transported through the endomembrane system to reach the compartment from which it is translocated into the cytosol, and there avoid degradation making it possible for a few molecules to inactivate a large proportion of the ribosomes and hence kill the cell.

Laparoscopy and laparoscopic ultrasound in the evaluation of pancreatic and periampullary tumours.

Background And Aims: The pre-operative determination of resectability of pancreatic and peri-ampullary neoplasia assists the selection of patients for surgical or non-surgical treatment. This study investigated whether the addition of laparoscopy with laparoscopic ultrasound to dual-phase helical CT could improve the accuracy of assessment of resectability.

Patients And Methods: Prospective study of 305 patients referred to a single unit for consideration of pancreatic resection who underwent dual-phase helical CT scanning +/- laparoscopy with laparoscopic ultrasound.

Variation in barometric pressure in Melbourne does not significantly affect the BTPS correction factor.

The conventional BTPS (body temperature and pressure, saturated with water vapour) correction factor varies with ambient barometric pressure (P(B)) and many lung function laboratories measure P(B) daily. The aim was to investigate whether a fixed value for P(B) could replace daily measurements. P(B) was measured daily over a 12-month period.

Protection afforded by heat shock protein 60 from Francisella tularensis is due to copurified lipopolysaccharide.

Heat shock proteins (Hsps) have attracted significant attention as protective antigens against a range of diseases caused by bacterial pathogens. However, more recently there have been suggestions that the protective response is due to the presence of peptide components other than Hsps. We have shown that mice that had been immunized with purified heat shock protein 60 (Hsp60) isolated from Francisella tularensis were protected against a subsequent challenge with some strains of the bacterium.

Genetics of ribosome-inactivating proteins.

Ribosome-inactivating proteins (RIPs) are a heterogeneous group of enzymes found mainly in plants and a few bacteria that possess N-glycosidase activity on ribosomes and a related polynucleotide adenosine glycosidase activity on naked nucleic acids. They encompass single enzymatic chains, heterodimeric toxic lectins and related agglutinins. Plants commonly produce several RIP isoforms encoded by multi-gene families.
