Article Synopsis
  • - Total pelvic exenteration (TPE) is a complex surgical procedure that may be the sole curative option for patients with advanced rectal cancers, resulting in lifelong management of two permanent stomas.
  • - There is a knowledge gap among radiologists regarding the complications that can arise after TPE surgery, leading to potential misdiagnosis or overlooked symptoms that could have serious consequences.
  • - The article aims to enhance understanding of postoperative pelvic anatomy, highlight early and delayed complications, and provide guidance on imaging techniques to accurately diagnose issues related to TPE through illustrative case studies.

Article Synopsis
  • A study aimed to evaluate how accurately contrast-enhanced mammography (CEM) diagnoses breast abnormalities compared to full-field digital mammography (FFDM) using histopathology as the gold standard.
  • The analysis included 150 patients, detecting 202 lesions with higher sensitivity and negative predictive value (NPV) for CEM (98.12% and 91.43%, respectively) than FFDM (90.6% and 65.12%).
  • Results suggest that CEM is a more effective tool than FFDM, particularly valuable for patients with dense breast tissue, as it offers better sensitivity, specificity, and ability to detect additional lesions.

Article Synopsis
  • The incidence of breast cancer in urban India is rising due to lifestyle changes and risk factors, with younger women often diagnosed at advanced stages.
  • The lack of awareness and existing social taboos make it difficult for women to seek timely medical help, leading to significant financial burdens on families and challenges in accessing quality care.
  • This article reviews breast cancer management from diagnosis to treatment, emphasizing the unique context of low-middle-income countries and discussing both early and advanced stages of the disease.

This review article examines the evidence-based management of colorectal cancers, focusing on topics characterized by ongoing debates and evolving evidence. To contribute to the scientific discourse, we intentionally exclude subjects with established guidelines, concentrating instead on areas where the current understanding is dynamic. Our analysis encompasses a thorough exploration of critical themes, including the evidence surrounding complete mesocolic excision and D3 lymphadenectomy in colon cancers.

Purpose: The current study was conducted to examine the role of consolidation chemotherapy after neoadjuvant radiation therapy (NART) in decreasing the involvement of the mesorectal fascia (MRF) in high-risk locally advanced rectal cancers (LARCs).

Methods: In total, 46 patients who received consolidation chemotherapy after NART due to persistent MRF involvement were identified from a database. A team of 2 radiologists, blinded to the clinical data, studied sequential magnetic resonance imaging (MRI) scans to assess the tumor response and then predict a surgical plan.

Magnetic resonance imaging (MRI) with the help of MRI-based tumor regression grade (mrTRG) score has been used as a tool to predict pathological tumor regression grade (pTRG) in patients of rectal cancer post-neoadjuvant chemoradiation. Our study aims to evaluate the ability of MRI in assessing treatment response comparing an objective mrTRG score and a subjective Likert score, with a focus on the ability to predict pathologic complete response (pCR).  Post-treatment MRI studies were retrospectively reviewed for 170 consecutive cases of histopathologically proven rectal cancer after receiving neoadjuvant chemoradiation and prior to surgery by two oncoradiologists blinded to the eventual postoperative histopathology findings.

Unlabelled: Mucopolysaccharidosis type VI (MPS VI), also known as Maroteaux-Lamy syndrome, polydystrophic dwarfism, and arysulfatase B (ASB) deficiency, is a lysosomal storage disorder with autosomal recessive inheritance characterized by progressive multisystem involved that causes many tissues and organs to enlarge and become inflamed. Skeletal deformities are common that progress and worsen in varying degrees thus affecting quality of life and life expectancy. Many studies have shown that allogeneic hematopoietic stem cell transplantation can reduce morbidity and enhance the survival and quality of life in such patients.

Fat necrosis of the breast is a benign non-suppurative inflammation of the adipose tissue and often mimics breast cancers, posing a diagnostic challenge for the clinician and radiologist. It has a myriad of appearances on different imaging techniques, ranging from the pathognomic oil cyst and benign dystrophic calcifications to indeterminate focal asymmetries, architectural distortions, and masses. A combination of different modalities can assist a radiologist in reaching a logical conclusion to avoid unnecessary interventions.

Colorectal cancer is one of the commonest cancers detected as also amongst the most common causes of cancer death. Survival has improved due to better disease understanding and treatment; however, a substantial proportion of patients recur after curative intent therapy. In this article, we will discuss the imaging features of recurrent colorectal cancer and the role of the radiologist in its management.

Introduction: Ductal carcinoma in situ with microinvasion (DCISM); arguably a more aggressive subtype of DCIS, currently has variable recommendations governing its staging and management in the UK. As a result, there is ongoing controversy surrounding the most appropriate management of DCISM, in particular the need of axillary staging.

Method: A search was conducted on the databases MEDLINE and Embase using the keywords: breast, DCISM, microinvasion, "ductal carcinoma in situ with microinvasion", sentinel lymph node biopsy, SLNB, axillary staging was performed.

Digit sucking is a common habit in young children, which if allowed to continue for a prolonged period, can adversely affect the development of the face and dental occlusion. Patients with digit sucking habits often present with an increased overjet, reduced overbite, anterior open bite, posterior crossbite and possible skeletal changes which can be challenging and costly to correct if the habit is not ceased in a timely manner. This article aims to provide guidance for general dental practitioners to recognise and appropriately manage patients with a digit sucking habit.

Variable architecture polymers are of considerable interest for the delivery of therapeutic biopolymers, such as DNA and proteins, to their site of action. Polymers that can respond with a change in conformation to biologically relevant stimuli, such as temperature and pH, are being carefully designed to take advantage of the change in environmental conditions the polymer-drug conjugate encounters upon progression from larger-scale systems in the body to subcellular compartments. Viruses respond to changes in the cellular environment to gain access to their desired region of cells, and much can be learned from the mechanisms they employ in this effort.
