Objective: Annuloplasty is the most common strategy for repair of functional tricuspid regurgitation (FTR) but is not effective in patients with massive/torrential FTR or leaflet tethering. To address the deficits of tricuspid annuloplasty, TRicuspid Anterior and Posterior Patch (TRAPP) repair was developed, which is a pericardial patch augmentation of the anterior and posterior leaflets.

Methods: To test this repair, a previously validated ex vivo model in an explanted porcine heart was used, wherein annular and leaflet geometry were evaluated using a 3-dimensional structured light scanner at 4 time points: (1) baseline, (2) induction of FTR, (3) annuloplasty repair, and (4) patch repair.

Background: Failure to rescue (FTR) is mortality after at least 1 postoperative complication. We investigated the impact of nighttime intensivist staffing and FTR after cardiac surgery.

Methods: We included patients who underwent cardiac surgery to examine FTR, defined as mortality in those who experienced a Society of Thoracic Surgeons-defined major complication.

Article Synopsis
  • A study was conducted to assess changes in access to esophagectomy following Maryland's implementation of a Global Budget Revenue (GBR) system in 2014, aiming to provide equitable reimbursement rates across patient insurance types.
  • Researchers analyzed data from 2012-2018, reviewing demographics, insurance statuses, and postoperative outcomes for patients undergoing esophagectomies for cancer.
  • Results indicated a significant increase in African-American patients receiving treatment post-GBR, and a notable rise in patients from lower-income counties, suggesting improved access for traditionally underserved groups without compromising health outcomes.

Background: Despite the high prevalence of neurological complications and mortality associated with extracorporeal cardiopulmonary resuscitation (ECPR), neurologically-focused animal models are scarce. Our objective is to review current ECPR models investigating neurological outcomes and identify key elements for a recommended model.

Methods: We searched PubMed and four other engines for animal ECPR studies examining neurological outcomes.

Article Synopsis
  • ECMO is increasingly used to support patients with respiratory failure who are awaiting lung transplants, but evidence for its effectiveness in this role is still limited.
  • A study analyzed data from 40,866 lung transplant patients over nearly two decades (2000-2019) to evaluate trends in patient characteristics, demographics, and mortality outcomes for those on ECMO compared to those not on it.
  • Results indicate that while ECMO patients are increasingly older and sicker, their post-transplant survival rates have improved significantly in recent years, reflecting better outcomes overall for both ECMO and non-ECMO groups.

We aimed to describe practice patterns and outcomes in patients with extracorporeal membrane oxygenation (ECMO) support throughout the coronavirus 2019 (COVID-19) pandemic, with the hypothesis that mortality would improve as we accumulated knowledge and experience. We included 48 patients supported on veno-venous ECMO (VV-ECMO) at a single institution between April 2020 and December 2021. Patients were categorized into three waves based on cannulation date, corresponding to the wild-type (wave 1), alpha (wave 2), and delta (wave 3) variants.

Background: Because of the limited published information on complications that obstructive sleep apnea (OSA) patients experience during and after cardiac surgery, we investigated OSA as a risk factor for postoperative outcomes.

Methods: This project used the Northern New England Cardiovascular Disease Study Group's data collected between 2011 and 2017 based on The Society of Thoracic Surgeons Adult Cardiac Surgery Database Data Collections form. A retrospective analysis of 1555 patients with OSA and 10,450 patients without OSA across 5 medical centers undergoing isolated coronary artery bypass grafting, isolated valve surgery, and combined coronary artery bypass grafting valve surgery was conducted.
