Of 168 urine sediments, which were obtained from 55 patients with chronic pyelonephritis in the course of 3 years when a significant bacteriuria with E. coli was present, we demonstrated antibody-loaded bacteria in 81 cases (48.21%).

Of 59 patients of this group with significant bacteriuria we cultivated 170 strains of E. coli, which were standardised O, K, and H.

In adults with chronic pyelonephritis with and without antibody-coated bacteria in the urine, we tested whether bacterial coating is correlated with the homologous O-antibody titre in the urine. We also determined Ig levels in urine and serum, as well as homologous O-antibody titres in serum.

Urine (24-hour-specimen, 100-fold concentrated) and serum samples from adults with chronic pyelonephritis have been investigated for O antibody against the strain isolated from the urine by means of the indirect hemagglutination and indirect immunofluorescence technique. Using the hemagglutination technique about one third of the urine showed the presence of O antibody. The highest titer demonstrated was 1:16.

In these cases in 82% the histological or tentative diagnosis, respectively, of a glomerulonephritis could be made. As diseases preceding the glomerulonephritis relapsing tonsillitides are occupying the first place, whereas scarlet fever, otitides, furunculoses and sinusitides were observed more infrequently.

In adult age actual acute pyelonephritides are rare. In most cases acute attacks of chronic pyelonephritides are in question.
