Background: Bronchiectasis has high rates of hemoptysis and recurrent hemoptysis, which is inconsistent among various etiologies. Idiopathic bronchiectasis and post-tuberculous bronchiectasis are two important etiologies in China, but the differences in clinical features and risk factors of recurrent hemoptysis have not been elucidated.

Methods: Patients hospitalized for idiopathic bronchiectasis or post-tuberculosis bronchiectasis were included.

Background: Allergic bronchopulmonary aspergillosis (ABPA) is characterized by enhanced T2 inflammatory response. Fractional exhaled nitric oxide (FeNO) measurement has been used as a valuable tool in predicting the development and management of asthma, another typical T2 inflammation. However, the clinical significance of FeNO in ABPA remains unclear.

Article Synopsis
  • * Researchers found that certain VOC levels were significantly higher in patients experiencing hypoxia and those with infections, indicating a potential link to the severity of the disease.
  • * Specifically, 10-heptadecenoic acid was identified as a key predictive marker for hypoxia, suggesting it could help assess bronchiectasis patient conditions.

Article Synopsis
  • ABPA patients have increased levels of Th2 cells and elevated T-helper type cells linked to IgE production, with decreased Th1 and Th17 cells compared to healthy controls.
  • Flow cytometry and RNA sequencing revealed that Th2-skewed T-helper (Tph) cells significantly contribute to B-cell differentiation and IgE synthesis in ABPA patients.
  • This distinct Th2-skewed Tph cell population may serve as a potential biomarker and therapeutic target for allergic bronchopulmonary aspergillosis.

Chronic (PA) infection significantly contributes to morbidity and mortality in bronchiectasis patients. Initiating antibiotics early may lead to the eradication of PA. Here we outline the design of a trial (ERASE; NCT06093191) assessing the efficacy and safety of inhaled tobramycin, alone or with oral ciprofloxacin, in bronchiectasis patients with a new isolation of PA.

The relationship between symptoms, measured using a validated disease-specific questionnaire, and longitudinal exacerbation risk has not been demonstrated in bronchiectasis. The aim of this study is to investigate whether baseline symptoms, assessed using the Quality-of-Life Bronchiectasis Respiratory Symptom Scale (QoL-B-RSS) and its individual component scores, could predict future exacerbation risk in patients with bronchiectasis. The study included 436 adults with bronchiectasis from three tertiary hospitals.

The World Health Organization has identified high-priority target product profiles for new TB diagnostics which include rapid biomarker-based, non-sputum-based diagnostic testing, using an easily accessible sample. The Cepheid 3-gene Host Response Fingerstick Blood Prototype Test (MTB-HR) quantifies relative mRNA levels of a 3-gene signature (, , and ) from a whole-blood sample on the GeneXpert platform. The objective of the present study was to evaluate the performance of the MTB-HR to distinguish between active tuberculosis (ATB), latent infection (LTBI), other pulmonary diseases, and healthy volunteers at a tertiary care centre.

Background: Previous research demonstrated that a homozygous mutation of g.136372044G>A (S12N) in caspase recruitment domain family member 9 ( CARD9 ) is critical for producing Aspergillus fumigatus -induced ( Af -induced) T helper 2 (T H 2)-mediated responses in allergic bronchopulmonary aspergillosis (ABPA). However, it remains unclear whether the CARD9S12N mutation, especially the heterozygous occurrence, predisposes the host to ABPA.
