Article Synopsis
  • This study evaluated trends over six years in the clinical outcomes of patients with subarachnoid hemorrhage (SAH) in Japan, focusing on those treated with either clipping or coiling techniques.
  • It found that in-hospital mortality rates decreased overall, while the percentage of patients undergoing clipping declined, with more patients receiving coiling or remaining untreated.
  • The improvements in Comprehensive Stroke Center capabilities correlated with increased coiling usage but did not affect short-term patient outcomes, indicating a need for further enhancement in care standards.

Background: It is unclear whether hepatocyte function and/or portal hypertension improves if a sustained virologic response (SVR) is achieved with direct-acting antivirals in patients with decompensated hepatitis C-related cirrhosis.

Methods: We examined the safety and efficacy of a 12-week course of sofosbuvir/velpatasvir (SOF/VEL) in 20 patients with decompensated hepatitis C-related cirrhosis. We also investigated changes in the hepatocyte receptor index (LHL15) and blood clearance index (HH15) by Tc-99 m-galactosyl human serum albumin scintigraphy, liver stiffness measurement (LSM) by transient elastography, and hepatic venous pressure gradient (HVPG) in patients who achieved an SVR at 24 weeks after treatment (SVR24).

Endoscopic biliary drainage is the recommended 1st-line treatment for malignant biliary obstruction. Although a high incidence of febrile neutropenia has been reported in patients treated with FOLFIRINOX and a biliary stent, it remains unknown whether the biliary stent contributes to patient survival. Thus, we aimed to elucidate the effects of biliary stents on the survival of patients with advanced pancreatic cancer treated with modified FOLFIRINOX (mFFX).

A direct-acting antiviral (DAA)-induced sustained virological response (SVR) reduces the risk of mortality. However, the risk factors associated with liver-related and non-liver-related mortality following a SVR after DAA treatment are unclear. We assessed the incidence and risk factors of liver-related and non-liver-related mortality in 1180 patients who achieved a SVR after DAA treatment.

  • Cholangiocarcinoma (CC) is a severe form of liver cancer with poor outcomes, necessitating new treatment strategies and understanding of its underlying genetic causes.
  • This study identified a specific microRNA, miRNA-3648, that affects the expression of the tumor suppressor gene SKI, which is linked to tumor growth in CC.
  • By manipulating SKI levels in cancer cells, researchers found that increasing SKI can halt cell proliferation and trigger cell cycle arrests, suggesting that targeting SKI could be a promising approach for treating intrahepatic CC.

Article Synopsis
  • The study investigates whether adding more hospitals with moderate comprehensive stroke center (CSC) capabilities is a better approach than focusing only on high-level CSC hospitals for thrombectomy procedures.
  • Using data from the J-ASPECT database (2013-2016), researchers analyzed how mechanical thrombectomy (MT) usage, CSC scores, and emergency medical service (EMS) response times affected patient outcomes after acute ischemic strokes.
  • Findings suggest that while higher CSC scores were previously linked to better outcomes, by the second phase of the study, this relationship diminished, indicating that increasing hospitals with moderate capabilities could enhance nationwide access to MT and improve patient results.
