Article Synopsis
  • Researchers explored how large language models (LLMs) can aid in generating guideline questions for allergic rhinitis by using online search data and direct LLM interactions.
  • They identified 3975 unique search queries, distilling them to 37 relevant questions, with 6 new questions prioritized for the 2024 ARIA guidelines that were not initially considered by the expert panel.
  • The study suggests that LLMs can complement traditional methods in developing clinical guideline questions, although the findings are limited to a single case study.

Article Synopsis
  • * Data from 670 patients revealed that while IVT may improve some outcomes, such as higher chances of achieving a modified Rankin Scale score of 0-2 in univariable analysis, this benefit was not consistently observed in more rigorous multivariable analyses.
  • * Overall, the findings suggest that adjunctive IVT may not significantly enhance clinical outcomes or safety compared to MT alone for this patient population.

Article Synopsis
  • Researchers aimed to identify factors predicting aneurysm occlusion and retreatment following WEB embolization due to limited existing studies.
  • The study reviewed data from 763 patients across 30 institutions, finding a 65.1% complete occlusion rate at follow-up and a 7.3% retreatment rate.
  • Key negative predictors for complete occlusion included smoking history, larger aneurysm size, and wall branch presence, while intraprocedural occlusion significantly improved long-term outcomes.

Background: Recent interest in understanding cardiomyocyte cell cycle has been driven by potential therapeutic applications in cardiomyopathy. However, despite recent advances, cardiomyocyte mitosis remains a poorly understood process. For example, it is unclear how sarcomeres are disassembled during mitosis to allow the abscission of daughter cardiomyocytes.

  • Preterm birth (PTB) is a major cause of infant mortality globally, and studies show mixed results regarding its rates during COVID-19 lockdowns, with changes ranging from -90% to +30%.
  • Analysis of data from 52 million births in 26 countries indicates modest reductions in PTB rates during the first three months of lockdown, but no significant changes in the fourth month.
  • High-income countries showed an increase in stillbirth risk during the first month of lockdown, with Brazil experiencing increased stillbirth rates throughout the entire lockdown period, highlighting the need for further investigation into these trends.

Background And Objectives: COVID-19-related inflammation, endothelial dysfunction, and coagulopathy may increase the bleeding risk and lower the efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate the safety and outcomes of revascularization treatments in patients with AIS and COVID-19.

Methods: This was a retrospective multicenter cohort study of consecutive patients with AIS receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 and June 2021 tested for severe acute respiratory syndrome coronavirus 2 infection.

Background: Primary valvular heart disease is a prevalent cause of morbidity and mortality in both industrialized and developing countries. Although the primary consequence of valvular heart disease is myocardial dysfunction, treatment of valvular heart diseases centers around valve repair or replacement rather than prevention or reversal of myocardial dysfunction. This is particularly evident in primary mitral regurgitation (MR), which invariably results in eccentric hypertrophy and left ventricular (LV) failure in the absence of timely valve repair or replacement.

Article Synopsis
  • The study investigated the genetic variant rs1333049 at chromosome 9p21 to see its impact on subsequent coronary heart disease (CHD) events in over 100,000 Europeans with existing CHD.
  • Results indicated no significant association between the variant and the risk of CHD death or myocardial infarction among those already diagnosed, contrasting with a strong link found in a separate group of CHD cases compared to healthy controls.
  • There was a slight positive correlation found between the variant and subsequent revascularization procedures, suggesting some potential role in this specific outcome, but overall, the variant did not predict acute CHD events for those already affected.

Background: The Genetics of Subsequent Coronary Heart Disease (GENIUS-CHD) consortium was established to facilitate discovery and validation of genetic variants and biomarkers for risk of subsequent CHD events, in individuals with established CHD.

Methods: The consortium currently includes 57 studies from 18 countries, recruiting 185 614 participants with either acute coronary syndrome, stable CHD, or a mixture of both at baseline. All studies collected biological samples and followed-up study participants prospectively for subsequent events.

The objective of this study was to evaluate the nutritional status of children under 5 years of age in Brazil in 2009 and its association with social and demographic factors. Data from the Household Budget Survey (Pesquisa de Orçamento Familiar - POF 2008-2009) were used, in which the nutritional profile was evaluated according to the weight-for-age (W/A), height-for-age (H/A) and weight-for-height (W/H) indices (n = 14,569). The association was estimated by applying the Pearson association test, a logistic regression and a correspondence analysis.

Context: In active Cushing's syndrome (CS), patients suffer from endothelial dysfunction and premature atherosclerosis. However, it is uncertain to what extent vascular health recovers after long-term remission. This is highly relevant because this topic relates to future development of cardiovascular disease.

Objective: To compare the agreement in the classification of the weight-for-age (W/A) and height-for-age (HA) indexes for children and adolescents with Down syndrome (DS) according to selected international reference distributions.

Methods: A cross-sectional study was carried out in children (2 to 9.9 years old) and adolescents (10 to 17.

Design: Prospective clinical trial in a 12-month follow-up period.

Setting: Children with end-stage renal disease (ESRD) who attended the pediatric nephrology clinic of Universidade Federal de São Paulo-Escola Paulista de Medicina (UNIFESP-EPM), São Paulo, Brazil.
