Background: Mild histologic lesions of tubulo-interstitial inflammation could represent a "response-to-wounding" rather than allorecognition. Tissue gene expression may complement histopathology for T-cell-mediated rejection (TCMR) diagnostics.

Methods: We report on the incorporation of tissue gene expression testing using a Molecular Microscope Diagnostic System into the management of kidney transplant biopsies with suspected TCMR.

Article Synopsis
  • Hypoalbuminemia, or low albumin levels, in critically ill patients, particularly those with cardiac issues, is linked to higher mortality rates, especially when undergoing veno-arterial (V-A) ECMO treatment.
  • A study analyzed albumin levels in 318 patients needing V-A ECMO, finding that those with pre-ECMO albumin levels ≤3 g/dL had a significantly higher in-hospital mortality rate (44.9%) compared to those with higher levels (27.5%).
  • The research suggests that lower pre-ECMO albumin levels are correlated with not only increased mortality but also more complications, like the need for platelet transfusions and gastrointestinal bleeding, indicating that albumin levels could

Article Synopsis
  • Pre-transplant weight change significantly influences post-heart transplant (HTx) survival, with limited previous research on this topic.
  • An analysis of over 9,600 patients revealed that weight loss (over 10 kg or 3 BMI points) increases the risk of death after the transplant, while some weight gain in non-obese patients can improve survival rates.
  • The study highlights that the effects of weight change before HTx depend on a patient's initial body mass index (BMI), emphasizing the need for personalized weight management strategies.

Purpose: We sought to investigate the clinical results of preemptive temporary right ventricular assist device (RVAD) placement with left ventricular assist device (LVAD) implantation for anticipated right ventricular failure.

Methods: Patients at risk for right ventricular failure were identified by the heart team. After median sternotomy, Protekduo (LivaNova, London, UK) dual-stage cannula was inserted from the right internal jugular vein under fluoroscopy guidance, and advanced into the pulmonary artery to the bifurcation of the main pulmonary artery.
