The use of the term "treatment resistance" (TR) in psychiatry has blossomed over the last two decades, with 20%-60% of all cases now labeled as such. Remaining effective treatment options, however, can be identified for two-thirds of these cases. There is no consensus on definition or criteria for TR within any diagnostic category, and literature review supports a broad range of individual causes of treatment failure (TF), rather than an actual entity of TR.

This article addresses several facelift challenges involving anatomic conditions, including platysma banding, endomorphic facial habitus, and midface hypoplasia. In addition, patient counseling and conveying realistic expectations about limitations of facelift alone, with and without adjunctive procedures, are presented. In addition, a few technical modifications of the facelift procedure contributing to more uniform success and longevity are discussed.

Nasal septal deformity is a central feature of the crooked nose, contributing to functional and aesthetic problems. Straightening of the septum often requires resection, scoring, or incision of the septum--maneuvers that inevitably weaken the cartilaginous dorsal and caudal L-shaped struts, which together are known as the L-strut. Compromise of this L-strut predisposes to septal buckling, recurrent deviation, and saddle nose deformity.
