There is a paucity of studies reporting the use of automated contouring function on cone-beam computed tomography (CBCT) images acquired during image-guided radiotherapy (IGRT) with offline adaptive radiotherapy for locally advanced non-small cell lung cancer (NSCLC). This case report discusses the use of an automated contouring function on CBCT images acquired during IGRT to quantify the dose distribution variations associated with tumor shrinkage in a patient with large NSCLC. A 72-year-old woman with locally advanced squamous cell carcinoma of the lung (T4N1M0; tumor diameter: 14 cm) underwent chemoradiotherapy.

Article Synopsis
  • Researchers aimed to create and validate a prediction score for futile recanalization (FR) in patients with low Alberta Stroke Program Early Computed Tomography Scores (ASPECTS) who underwent endovascular thrombectomy (EVT) for large vessel occlusions (LVO).
  • The study analyzed data from 219 patients with anterior circulation LVO and identified key predictors of FR, resulting in the development of the SNAP score, which considers factors like the site of occlusion and patient age.
  • The SNAP score successfully predicted FR with an area under the curve of 0.79 in both training and validation cohorts, indicating its potential usefulness for guiding expectations in patients undergoing EVT for severe strokes.

  • Gastric conduit obstruction (GCO) can occur after esophagectomy, and this case highlights a rare instance where it was successfully repaired using laparoscopic revision surgery.
  • A 66-year-old man experienced difficulty in fluid passage 14 months post-surgery due to a twisted and deviated gastric conduit, necessitating revision surgery.
  • The case underscores the importance of optimal fixation methods during the initial surgery to prevent GCO and indicates that intraoperative endoscopy can effectively assist in diagnosing and resolving obstructions.

Article Synopsis
  • Hepatic portal venous gas is a rare condition linked to severe bowel issues like ischemic colitis or cancer, as illustrated by a case involving a 72-year-old man with ileocecal cancer and sepsis from Klebsiella pneumoniae.
  • The patient was admitted for diabetes control but developed fever and septic shock without typical abdominal symptoms; imaging revealed gas in the portal vein, indicating a serious underlying infection.
  • Early detection through abdominal ultrasound was crucial, highlighting that hepatic portal venous gas should prompt urgent investigation to address potential infections or complications from underlying conditions.

Background: A higher number of recanalization attempts reduces the efficacy of endovascular thrombectomy (EVT) for acute ischemic stroke secondary to large vessel occlusion (LVO). We assessed the impact of switching EVT techniques after a failed first pass on procedural and clinical outcomes.

Methods: This multicenter international study, conducted between January 2013 and December 2022, included patients undergoing EVT for anterior circulation LVO (internal carotid artery or M1 segments) with failed first pass recanalization.
