A novel synthesis of selenoesters and telluroesters based on the reactions of nucleophilic siloxycarbenes, which were generated by the visible-light-induced isomerization of the corresponding aroyl-, heteroaroyl-, or alkenoylsilanes, with electrophilic chalcogen reagents was developed. The use of appropriate selenides or ditellurides/Lewis acids enabled the coupling at temperatures below ambient temperature with a broad substrate scope and high functional-group tolerance. To the best of our knowledge, this is the first example of a synthetic method for selenoesters and telluroesters involving the combination of an acylanion equivalent and cationic chalcogen synthons.

Sunlight-driven photocatalytic water splitting shows promise for green H production. In an attempt to achieve seawater splitting, we constructed a new stoichiometric brine splitting system that produces H along with Cl instead of O. Cl-a more potent high-value-added oxidant than O-was obtained with 100% selectivity over 10 h by adjusting the solution pH to acidic using a UV-light-driven Pt-loaded TiO photocatalyst.

A 49-year-old woman who had surgery for left breast cancer and subsequently underwent a two-stage deep inferior epigastric perforator(DIEP)flap reconstruction. One month postoperatively, she became aware of abdominal distention and visited a local hospital. CT scan revealed subcutaneous fluid accumulation with capsular formation in the lower abdomen.

Article Synopsis
  • Therapeutic options are limited for breast cancer patients with brain metastases (BM) and leptomeningeal carcinomatosis (LMC), but trastuzumab deruxtecan (T-DXd) shows promise.
  • In a study of 104 HER2-positive breast cancer patients treated with T-DXd, the overall response rate (ORR) was 55.7%, with median progression-free survival (PFS) of 16.1 months.
  • For patients with BM, intracranial ORR was 62.7%, while in those with LMC, 12-month PFS and overall survival (OS) rates were 60.7% and 87.1%, indicating effective disease control.

Background: Tumor-infiltrating lymphocytes (TILs) predict response to neoadjuvant chemotherapy (NAC) in triple-negative breast cancer (TNBC) patients. However, the TIL level can be determined at a few facilities. By contrast, neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) are easily and objectively determined from the results of full blood counts.
