Survival with Trastuzumab Emtansine in Residual HER2-Positive Breast Cancer.

Background: Patients with human epidermal growth factor receptor 2 (HER2)-positive early breast cancer with residual invasive disease after neoadjuvant systemic therapy have a high risk of recurrence and death. The primary analysis of KATHERINE, a phase 3, open-label trial, showed that the risk of invasive breast cancer or death was 50% lower with adjuvant trastuzumab emtansine (T-DM1) than with trastuzumab alone.

Methods: We randomly assigned patients with HER2-positive early breast cancer with residual invasive disease in the breast or axilla after neoadjuvant systemic treatment with taxane-based chemotherapy and trastuzumab to receive T-DM1 or trastuzumab for 14 cycles.

The importance of organisational culture for health system resilience: A qualitative analysis of factors that supported health care workers in Germany during the COVID-19 pandemic.

Health system resilience is defined as the ability of a system to prepare, manage, and learn from shocks. This study investigates the resilience of the German health system by analysing the system-related factors that supported health care workers, a key building block of the system, during the COVID-19 pandemic. We thematically analysed data from 18 semi-structured interviews with key informants from management, policy and academia, 17 in-depth interviews with health care workers, and 10 focus group discussions with health care workers.

Target trial emulation (TTE) is a popular framework for observational studies based on electronic health records (EHR). A key component of this framework is determining the patient population eligible for inclusion in both a target trial of interest and its observational emulation. Missingness in variables that define eligibility criteria, however, presents a major challenge towards determining the eligible population when emulating a target trial with an observational study.

Understanding the causes of past atmospheric methane (CH) variability is important for characterizing the relationship between CH, global climate and terrestrial biogeochemical cycling. Ice core records of atmospheric CH contain rapid variations linked to abrupt climate changes of the last glacial period known as Dansgaard-Oeschger (DO) events and Heinrich events (HE). The drivers of these CH variations remain unknown but can be constrained with ice core measurements of the stable isotopic composition of atmospheric CH, which is sensitive to the strength of different isotopically distinguishable emission categories (microbial, pyrogenic and geologic).
