Article Synopsis
  • A study was conducted to assess the prevalence of tuberculosis (TB) among HIV-positive inmates entering Malaysia's largest prison, Kajang prison, beginning in 2013.
  • The research involved interviewing participants and collecting sputum samples for testing, revealing a 12.6% TB prevalence rate among the 214 recruited individuals, with specific risk factors identified.
  • The findings highlight the urgent need for improved TB control programs in prisons, as there is a lack of global data on TB at prison entry, particularly from Malaysia.

Article Synopsis
  • The text talks about a way to check for tuberculosis (TB) in people living with HIV, using a method to look for four symptoms first, then a special test called Xpert if those symptoms are present.*
  • Researchers wanted to see if there are better ways to test for TB, so they looked at a lot of studies and data from several years to compare different testing methods.*
  • They found that the original four-symptom method works pretty well, but there are other tests, like checking for C-reactive protein, that might also be helpful in finding TB.*

The aim of this study was to simulate the effects of tuberculosis (TB) treatment strategies interventions in an overcrowded and poorly ventilated prison with both high (5 months) and low (3 years) turnover of inmates against improved environmental conditions. We used a deterministic transmission model to simulate the effects of treatment of latent TB infection and active TB, or the combination of both treatment strategies. Without any intervention, the TB prevalence is estimated to increase to 8.

Incarcerated people living with HIV and opioid dependence face enormous challenges to accessing evidence-based treatment during incarceration and after release into the community, placing them at risk of poor HIV treatment outcomes, relapse to opioid use and accompanying HIV transmission risk behaviors. Here we describe in detail the design and implementation of Project Harapan, a prospective clinical trial conducted among people living with HIV and opioid dependence who transitioned from prison to the community in Malaysia from 2010 to 2014. This trial involved 2 interventions: within-prison initiation of methadone maintenance therapy and an evidence-based behavioral intervention adapted to the Malaysian context (the Holistic Health Recovery Program for Malaysia, HHRP-M).

Setting: Xpert® MTB/RIF is the most widely used molecular assay for rapid diagnosis of tuberculosis (TB). The number of polymerase chain reaction cycles after which detectable product is generated (cycle threshold value, CT) correlates with the bacillary burden.OBJECTIVE To investigate the association between Xpert CT values and smear status through a systematic review and individual-level data meta-analysis.
