The response of pancreatic polypeptide (PP) to testmeal (5 ml/kg) and intraduodenal acid (4 mEq HCl/5min) and its reaction to somatostatin (3.5 microgram/kg/h following bolus injection of 3.5 microgram/kg) was studied in dogs with chronic duodenal and gastric fistulae.
View Article and Find Full Text PDFIn 4 dogs with chronic duodenal and gastric fistulae, exocrine pancreatic function was assessed by cannulating the pancreatic duct and collecting the duodenal contents. Both methods were applied in each animal. Pancreatic secretion was stimulated by infusion of 2 CHR units of pancreozymin and secretin or by administration of a liquid test meal, injected into the stomach through the gastric fistula.

In the period 1972-1976, 5 of 17 patients admitted to the Cantonal Hospital, Basel, for falciparum malaria had to be treated in the intensive care unit. Two patients died. None had performed regular chemoprophylaxis and in all of them several days had elapsed between onset of symptoms and diagnosis.

The test was standardized and the effect of glucagon on exocrine pancreatic secretion was evaluated. The mean tryptic activity detected in 18 tests in 6 dogs was 32.

Cannulation of the papilla has been successful in 144 out of 197 ERCPs. The rate of success increases with the experience of the endoscopist. The intended filling of the pancreatic duct was successful in 87%, of the bile ducts in 64% and of both duct systems in 28%.

The motility of the polymorphonuclear leukocytes and their phagocytic and killing indices with and without leukokinin stimulation decreased in protein-depleted animals. Acid cathepsin decreased, DNA relatively increased, and peroxidase, alkaline phosphatase, acid phenylphosphatase, and lysozyme reached higher levels in the polymorphonuclear leukocytes of animals on a "O" protein diet.

Amylase did not act on any strain in concentrations to 10,000 IU/ml whereas trypsin increased vacuolization and lipase enhanced retraction of the protoplasm particularly in 2 of the 5 tested vibrio strains. Pancreatin caused damage hoth of the cell wall and the cytoplasm.

The pancreozymin-secretin-induced enzyme output and bile flow, as well as the enzyme concentration and duodenal volume during test meal stimulation were significantly reduced by SST. SST caused a less marked decrease in bicarbonate concentration, while volume and bicarbonate output did not change.

Two groups of 5 and 6 Vervet monkeys respectively were fed a protein-free diet. Both groups of protein depleted Vervets developed a minor decrease of serum and intestinal immunoglobulins, as well as marked impairment of exocrine pancreatic function. Thereafter the groups were challenged with Vibrio cholerae, while one group received purified pancreatic extract by oral administration.

The features of the disease and its differential diagnosis are discussed. Special reference is given to the diagnostic approach by endoscopic retrograde cholangiography and to treatment with Chenodeoxy-cholicacid.

The diagnostic value of coloscopy was assessed by analysis of the endoscopic findings in 310 patients investigated between January 1974 and December 1975. Altogether, 342 coloscopies were performed with a complication rate of 1.5% (3 perforations, 2 hemorrhages).

21 patients with gastroenterological disease and indication for the use of intravenous nutrition received an elemental diet (ED) for 5-44 days. In 6 out of 8 patients with exacerbation of Crohn's disease remissions were achieved, apart from 3 persistent fistulas. In 5 out of 9 cases with various primary diseases and postoperative intestinal fistulas, spontaneous healing was observed.

Both patients had undergone partial hepatectomy and were subsequently treated chemotherapeutically. A 36-year-old woman was admitted to hospital with a first attack of acute intermittent porphyria. At the same time increased serum levels of amylase and lipase as well as an increased amylase clearance to creatinine clearance ratio were observed, permitting the diagnosis of acute pancreatitis. The etiology of the latter could not be determined.

Six Patas monkeys were fed a protein-free diet while 16 animals of the same species receoved a standard monkey diet. The protein-depleted monkeys showed reduced absorption of folic acid but not Dxylose, and 5 out of 6 exhibited depressed exocrine pancreatic function. When challenged with Vibrio cholerae, animals with reduced folic acid absorption and pancreatic enzyme production excreted the micro-organisms for a longer time.

Advantages and disadvantages of various laboratory techniques used for the diagnosis of intestinal protozoa are discussed. The importance of a correct parasitological diagnosis is emphasized. The MIF procedure has been investigated with regard to its reliability and found to be very effective due to its high diagnostic yield and reproducibility in repeated examinations.

In the presence of chymotrypsin, PABA is split from the peptide and excreted in the urine. The amount of PABA excreted serves as a parameter of exocrine pancreatic function.

After eight weeks the animals were infected with cholera vibrios. IgG, IgA and IgM increased in the sera of all animals after the challenge but the avidity of the immunoglobulins was lower in vervets on LP diets.
