This chapter aims to explain and evaluate the evidence for psychological, physical and complementary therapies as part of a holistic plan for managing neuropathic pain. Psychological therapies refer to interventions targeting mental health, while physical therapies refer to interventions designed to target movement and functional ability, and complementary therapies are those that attempt to target key mechanisms of change to alter brain and body functioning, or thought processes related to the experience of pain. Each therapeutic modality is discussed to narratively report on the evidence and provide implications for clinicians.

Mercury (Hg) researchers have made progress in understanding atmospheric Hg, especially with respect to oxidized Hg (Hg) that can represent 2 to 20% of Hg in the atmosphere. In this study, we (a) formulated a general hypothesis about how wetland (functional and structural) traits influence avian diversity, (b) turned this hypothesis into a non-parametric Bayesian network, (c) disentangled the direct and indirect effects of the variables influencing waterbird species, and (d) simulated the changes expected to the levels of avian diversity as a result of numerous counterfactual and management scenarios. We applied our framework to the Sicilian wetlands as a whole; then, we downscaled simulations locally to a wetland of particular interest (Pantano Bruno). We found that (1) waterbird species are highly sensitive to wetland traits; (2) wetland traits have both direct and indirect effects upon alpha avian diversity; (3) the direct and indirect effects of wetland traits can be contrasting; (4) water level fluctuations (benefit), diversions (cost), and salinity (cost) are key factors for waterbird conservation; (5) these wetlands have the potential for hosting a level of alpha avian diversity that is double the baseline (from 19 to 38 species); (6) these wetlands are prone to ecological collapse if all traits deteriorate (from 19 to 6 species per wetland); and (7) the ecological information gained at the regional scale can be properly downscaled to the local scale to make inferences on single wetlands.

Pulmonary (Mtb) infection results in highly heterogeneous lesions ranging from granulomas with central necrosis to those primarily comprised of alveolitis. While alveolitis has been associated with prior immunity in human post-mortem studies, the drivers of these distinct pathologic outcomes are poorly understood. Here, we show that these divergent lesion structures can be modeled in C3HeB/FeJ mice and are regulated by prior immunity.

Background: The provision of graded supervision affording progressive autonomy is fundamental to the progression of a medical learner toward competency for independent practice; the decision of how much supervision versus autonomy to provide a trainee in the execution of clinical care constitutes an entrustment decision. Despite entrustment decision making occurring both daily in practice and summatively at points of matriculation through stages of medical training, the factors influencing entrustment decisions remain poorly understood across clinical contexts.

Objective: This study was designed to explore the central research question: How are entrustment decisions made in the medical intensive care unit (ICU)?

Methods: This qualitative case study used semistructured interviews with attending pulmonary and critical care physicians in the medical ICU at a major midwestern medical center to explore the entrustment decision-making process as it was enacted in the clinical environment.
