Objective: Central venous catheter (CVC) insertion is a common practice for anesthetists, but this invasive procedure generates anxiety and pain in patients that are often underestimated. We compared target-controlled infusion of remifentanil and local lidocaine infiltration with placebo and local lidocaine infiltration to decrease pain scores during CVC placement.

Materials And Methods: We included conscious, adult patients without contraindication to remifentanil or lidocaine.

Article Synopsis
  • ICU residents often feel unprepared for end-of-life care due to a lack of training, which can negatively impact their ability to support dying patients and their families.
  • Formal ethics training was introduced from 2007 to 2012, and a survey of 430 junior intensivists showed increased participation and improved comfort levels in handling life-sustaining treatment decisions after the training.
  • The study found that factors like age and sex positively influenced perceptions of the dying process, while anxiety about family reactions and lack of training were linked to more negative views, highlighting the importance of training for better patient care.

We compared them with a group of 20 patients with proven idiopathic thrombotic thrombocytopenic purpura hospitalized during the same period.
