From 2000 to 2010, 720 patients with facial trauma were admitted in Plastic Surgery Service of Argerich Hospital, Buenos Aires, Argentina; 58 of them with panfacial fractures were included in this study. Height velocity impact is the principal etiology, and most concomitant extrafacial injuries are neurocranium and cervical spine. Common affected areas were orbits, nose, and malar-zygoma.

Soft and osseous tumors that develop into the frontal are the most profitable with the use of video-assisted surgery, thus avoiding also a visible scar. In the Department of Plastic Surgery at Argerich Hospital in Buenos Aires, Argentina, from 1999 to 2010, video-assisted operations were used in the treatment of 158 patients, 26 of them presented lipomas and osteomas into the frontal tissues. In all 26 patients, both local anesthesia and incisions behind the hairline were performed.

Giant cell tumors of the jaw (GCTJ) are common in the long bones but rare in the craniofacial region, with only 1% of cases occurring in the latter; they account for approximately 3% to 5% of all primary bone tumors and 15% to 20% of all benign bone tumors. The biologic behavior of central giant cell lesions of the jaws ranges from quiescent to aggressive with destructive expansion, and the clinical behavior of GCTJ of the jaws is variable and difficult to predict. A number of tumors that occur in the jaws contain giant cells but are not true benign giant cell tumors.

Ectopic thyroid tissue with no orthotopic gland is extremely rare; malign transformation of ectopic thyroid is also an uncommon event, and rarer is the follicular variant of papillary carcinoma. In this clinical report, a case with submandibular ectopic thyroid without orthotropic thyroid gland is presented. Treatment was a complete resection of tumor, and histologic findings reveal follicular variant of papillary carcinoma.

The incidence of lymphomas in salivary glands is 5%, and it represents 10% of malignant salivary tumors. Synchronous tumors arising in different major salivary glands are not only very infrequent but also extremely rare; on the other hand, synchronous lymphomas in ipsilateral major salivary glands have not been reported previously. This article reports a case of synchronous lymphomas in mucosa-associated lymphoid tissue in the ipsilateral parotid and submandibular glands.

Congenital tumors as branchiogenic cysts and hygromas are most likely to occur in the middle and superior third of the lateral area of the neck. They are found beneath the sternocleidomastoid (SCM); domus can lie just beneath anterior, middle, and posterior aspect of the muscle, depending on the development of the tumor. When tumor is placed on the anterior edge of SCM, an anterior approach is used; if it develops on the posterior border, a posterior way is indicated.

The recent advent of endoscopic procedures has compelled plastic surgeons to reconsider the conventional methods by which the excision of different types of head and neck tumors are classically achieved. Endoscopic resection is a safe and minimally invasive approach and spares unnecessary discomfort to the patient. Tissues of the pectoral area such as skin and pectoralis major muscle are used in safe and extended flaps for cervical and neck reconstructions. As blood supply is derived from medial vessels (internal mammary artery) or lateral (thoracodorsal and lateral thoracic arteries), 2 different flaps can be constructed: medial and lateral deltopectoral flaps. Medial deltopectoral flap was developed by Bakamjian as an axial-pattern skin flap, and its blood supply depends on perforating branches from the internal mammary artery.

The fascicular pattern is the most important component of a nerve when a microsurgical reconstruction is made. The intraneural topography of the extratemporal facial nerve does not have a reliable fascicular pattern, although our study in 25 extratemporal facial nerves showed a definite fascicular pattern in the different studied levels. The microanatomic intraneural investigation was made in four different levels; we studied the diameter of the trunk, number and diameter of each fascicle, fascicular pattern, characteristics of the epineurium, perineurium, and interfascicular connective tissue, and extra- and intraneural blood supply.

Despite the availability of efficient tear substitutes, many patients with dry eye syndrome experience severe corneal injuries and a subsequent loss of vision. Surgical techniques using mayor salivary glands to provide a substitute for tears have been reported; with this technique the drainage of saliva goes into the conjunctival fornix, permitting corneal and conjunctival humidification. The authors describe a new surgical approach in which minor salivary glands are autotransplanted into the conjunctival fornix by means of a graft of the intraoral mucosa-transporting salivary glands.

Closed versus open reduction in condyle fractures is a dilemma that may torment the plastic surgeon. Although at present it is accepted that there are fractures that must be open reduced as when the middle cranial fossa or temporal fossa are involucrated, foreign body are in the joint capsule, lateral extracapsular deviation of condylar deviation, and open fractures. Risdon or retromandibular approaches are used for the treatment of fractures in the condyle neck and superior third of the lower ramus.
