In a retrospective study of patients with locally advanced or metastatic gallbladder cancer, we tried to define patient groups, that profit either from extended resection, palliative surgery or intervention. 102 out of 160 operative treated patients (73.8%) had UICC-tumor stages III and IV.

Since several years, there are reports from the USA about the esophageal obturator airway, a simple device for emergency ventilatory support in the unconscious patient. The simplicity of its use is of advantage and may easily be achieved by paramedics too (blind insertion into the esophagus without the need of a laryngoscope). The occlusion of the esophagus prevents gastric inflation as well as regurgitation, emesis and aspiration.

Diagnosing aspiration of foreign bodies in children still causes a great deal of difficulties, especially as the anamnesis is not always dependable and the aspiration has often not been noticed. This is why it sometimes takes months before the foreign bodies are discovered and removed. Quite often a child is treated over a long period for bronchitis which is actually caused by a foreign body.
