A 39-year-old man with an extrinsic esophageal lesion was found to have an anterior herniation of a soft degenerated cervical disc. Only two cases of symptomatic anterior cervical disc herniation have been reported previously. Dysphagia produced by anterior cervical osteophytes is more common and is a recognized clinical entity.

Assuming that anti-allotypic anti-Ig in rheumatoid arthritis is stimulated by the individual's allotype, it would be reasonable to expect a higher incidence of anti-G1m(a) in individuals carrying G1m(a). There is marked divergence among various populations in allotype frequencies. G1m(a) allotype and the prevalence of anti-Ig reactive with G1m(a) Ig were determined in 18 Kuwaiti, 23 Japanese, and 41 Swedish rheumatoid arthritis patients.

Although the presence of a hemodynamically significant carotid artery lesion is commonly used as an indicator of impaired cerebral circulation, the effect of such lesions on cerebral perfusion pressure and cerebral blood flow has never been determined accurately. We used positron emission tomography (PET) to study 19 patients with unilateral hemodynamically significant carotid artery disease (greater than 66% diameter reduction) and no evidence of cerebral infarction. According to PET measurements in the cerebral hemisphere distal to the lesion, 7 patients had normal cerebral hemodynamics, 8 had reduced perfusion pressure with normal blood flow, and 4 had reduced blood flow.

The case records of 114 patients were reviewed to ascertain the efficacy of bedside twist-drill craniotomy and continuous closed-system catheter drainage for the treatment of chronic subdural hematomas. Ninety-eight (86%) patients achieved an excellent outcome, and seven (6%) had no significant improvement. The total mortality from all cases was 8% in this group.

This study was undertaken to determine the minimum CBF and CMRO2 required by the human brain to maintain normal function and viability for more than a few hours. Positron emission tomography (PET) was used to perform regional measurements in 50 subjects with varying degrees of cerebral ischemia but no evidence of infarction. There were 24 normal subjects, 24 subjects with arteriographic evidence of vascular disease of the carotid system, and two subjects with reversible ischemic neurological deficits due to cerebral vasospasm.

Acute bilateral damage to large areas of both cerebellar hemispheres including the dentate nuclei led to temporary loss of speech in six children. In each case muteness was unassociated with motor paralysis, loss of higher cognitive functions, or cranial nerve dysfunction. Muteness lasted one to three months.

Gm phenotype G1m 1,2, and 3 frequencies were examined in 103 Finnish myasthenia gravis patients. There were no significant differences as compared with the expected frequency, either in the total material or when patients were subdivided according to sex, age of onset of disease, thymus pathology, or presence of HLA-B8. However, individuals with Gm(+1) had a significantly higher concentration of antibodies against acetylcholine receptor, possibly indicating a correlation with severity of disease.

Positron emission tomography (PET) was used to investigate the regional hemodynamic and metabolic changes that accompany focal reductions in cerebral blood flow to ischemic but uninfarcted regions of the brain. Studies were performed on 7 patients chosen from a larger group of subjects with transient ischemic attacks and normal computed tomographic findings, specifically because their PET studies showed decreased blood flow to the symptomatic hemisphere rather than symmetrical flow to the two hemispheres. In regions with decreased blood flow, the cerebral metabolic rate for oxygen was also decreased, but to a lesser degree.

After superficial temporal artery-middle cerebral artery bypass, cerebral blood flow (CBF) in the operated hemisphere increased in 6 of 17 patients. Preoperatively, the symptomatic hemisphere showed lower CBF in all six, lower oxygen metabolism in five, higher blood volume in four, and higher oxygen extraction in two. With the postoperative increase in hemispheric CBF, there was a decrease in oxygen extraction, but no change in blood volume or oxygen metabolism.
