Objectives: This study aimed to determine the feasibility of using Chat Generative Pre-trained Transformer (ChatGPT) (https://chatgpt.com) as a patient resource for paediatric otolaryngology conditions and assess the quality of responses generated by ChatGPT when compared with information available on the internet.

Method: ChatGPT responses to common paediatric otolaryngology conditions were compared with top internet pages for readability (Flesch Reading Ease score, word count), expediency (time taken to generate response), validity (comparison of recommendations to the American Academy of Otolaryngology Head and Neck Surgery guidelines) and consistency (changes in recommendations based on question variation).

Clients with chronic and severe psychiatric illnesses, such as schizophrenia, often present with significant challenges in psychotherapy, particularly in establishing a stable therapeutic alliance. This paper details a novice therapist's experience and initial challenges in establishing a bond with a client diagnosed with chronic schizophrenia. After initial efforts and 12 months of fluctuating client engagement, the treatment team recognized the need for a stronger alliance.

Evaluation of Ki-67 expression and large cell content as prognostic markers in MZL: a multicenter cohort study.

Marginal zone lymphoma (MZL) can have varied presentations and pathologic features, including high Ki-67 expression ( > 20%) as well as increased numbers of large B cells (LC). However, there are limited data available demonstrating the prognostic significance of these variables in patients with MZL. In this multi-institutional retrospective cohort study of patients with MZL treated at 10 centers, we evaluated the association between the presence of Ki-67 expression and increased LCs on survival and risk of histologic transformation (HT).

Photocatalyst-free regioselective sulfonamidation of -(2-hydroxyaryl)amides in visible-light.

In this work, we report a regioselective sulfonamidation of -(2-hydroxyaryl)amides with iminoiodinanes and iodine in visible light at room temperature. The method does not require a strong oxidant, metal or photocatalyst and enables direct functionalization of a C-H bond to a C-N bond. Mechanistic investigations suggest generation of an N-centered radical from ,-diiodo-sulfonamide by homolytic N-I bond cleavage followed by its site-specific addition to -(2-hydroxyaryl)amides to furnish -sulfonamide derivatives.

FlavorDB2: An updated database of flavor molecules.

Flavor is expressed through the interaction of molecules via gustatory and olfactory mechanisms. Knowing the pivotal role of flavor molecules in food and fragrances, the existence of a comprehensive repository becomes valuable. Such a repository encompasses the information of flavor molecules characterizing their flavor profile, chemical properties, regulatory status, consumption statistics, taste/aroma threshold values, reported uses in food categories, and synthesis.

Chirality induction to porphyrin derivatives co-confined at the air-water interface with silica nano-helices: towards enantioselective thin solid film surfaces.

A supramolecular approach based on self-assembled structures allows the formation of large structured co-assemblies based on chiral and achiral compounds with original physicochemical features. In this contribution, an achiral and hydrophobic porphyrin was co-assembled at the air-water interface with mesoscopic silica nano-helices dispersed in the water subphase of a Langmuir trough without covalent bond formation. This procedure allowed transferring the porphyrin/nano-helix co-assemblies on a solid support within a thin hybrid layer.

Socioeconomic determinants of moderate and severe obstructive sleep apnea in children: A pre and post COVID analysis.

Introduction: COVID-19 has disproportionately affected healthcare access of certain minority groups, and thus could contribute to delay in timely evaluation and management of pediatric obstructive sleep apnea (OSA). Socioeconomic status (SES) is known to affect healthcare access in pediatric otolaryngology and may also be a risk factor for OSA and its associated co-morbidities.

Objectives: Our primary objective is to determine the impact of COVID-19 on time to diagnosis (TTD) and time to treatment (TTT) of polysomnography (PSG)-proven moderate to severe pediatric OSA in different socioeconomic classes.

Chimeric antigen receptor T-cell infusion for large B-cell lymphoma in complete remission: a center for international blood and marrow transplant research analysis.

CD19 CAR T-cell (CAR-T) therapy is commonly administered to patients with relapsed or refractory large B-cell lymphomas (LBCL), but salvage or bridging therapy can sometimes lead to a complete response (CR) prior to infusion. Limited studies have assessed the outcomes of patients infused in CR. A total of 134 patients with LBCL in CR prior to CAR-T infusion were identified from the CIBMTR registry, with median prior lines of therapy of 3 (range 2-9).

We report a one-step approach for the conversion of β-enaminonitriles to synthetically versatile α-keto amides in moderate to high yields under visible light irradiation photosensitized by porphyrins. The method is mild, cost-effective, and sustainable and requires air as the sole reagent/oxidant. The reaction is believed to proceed via an ene-type pathway initiated by O, followed by dehydration, imine hydrolysis, and subsequent nucleophilic substitution of the cyanide group by amine.

Synthesis of bioengineered heparin chemically and biologically similar to porcine-derived products and convertible to low MW heparin.

Heparins have been invaluable therapeutic anticoagulant polysaccharides for over a century, whether used as unfractionated heparin or as low molecular weight heparin (LMWH) derivatives. However, heparin production by extraction from animal tissues presents multiple challenges, including the risk of adulteration, contamination, prion and viral impurities, limited supply, insecure supply chain, and significant batch-to-batch variability. The use of animal-derived heparin also raises ethical and religious concerns, as well as carries the risk of transmitting zoonotic diseases.

A Cephalometric Study of Sella Turcica: Correlation of Its Size with Different Skeletal Malocclusions.

Introduction: The present study was done to determine the size of the sella turcica in different skeletal type subjects and to evaluate if any significant difference exists between them, which could be the basis for early diagnosis.

Materials And Methods: A total of 60 lateral cephalograms of patients above 15 years of age were selected and distributed according to skeletal malocclusion into class I ( = 20), class II ( = 20), and 20 class III ( = 20). Syndromic patients (physically/mentally/both) or patients with major illnesses were not included.

Understanding and Controlling Molecular Compositions and Properties in Mixed-Linker Porphyrin Metal-Organic Frameworks.

Porphyrin-based metal-organic frameworks (MOFs) are attractive materials for photo- and thermally activated catalysis due to their unique structural features related to the porphyrin moiety, guest-accessible porosity, and high chemical tunability. In this study, we report the synthetic incorporation of nonplanar β-ethyl-functionalized porphyrin linkers into the framework structure of PCN-222, obtaining a solid-solution series of materials with different modified linker contents. Comprehensive analysis by a combination of characterization techniques, such as NMR, UV-vis and IR spectroscopy, powder X-ray diffraction, and sorption analysis, allows for the confirmation of linker incorporation.

Validation of an algorithm for sound-based voided volume estimation.

A voiding diary is commonly used in clinical practice to monitor urinary tract health. However, manual recording and use of a measuring cup can cause significant inaccuracy and inconvenience. Recently sound-based voided volume estimation algorithms such as proudP have shown potential to accurately measure the voided volumes of patients urination while overcoming these inconveniences.

The optimal management of relapsed and refractory Hodgkin lymphoma: post-brentuximab and checkpoint inhibitor failure.

The treatment landscape of classical Hodgkin lymphoma has changed dramatically over the past decade. Relapsed and refractory mainstay therapeutics such as brentuximab vedotin (BV) and checkpoint inhibitors (CPIs) are being moved to earlier lines of therapy. However, the treatment of patients who progress after BV and CPI remains a challenge.

Defining the Time-limited Trial for Patients with Critical Illness: An Official American Thoracic Society Workshop Report.

In critical care, the specific, structured approach to patient care known as a "time-limited trial" has been promoted in the literature to help patients, surrogate decision makers, and clinicians navigate consequential decisions about life-sustaining therapy in the face of uncertainty. Despite promotion of the time-limited trial approach, a lack of consensus about its definition and essential elements prevents optimal clinical use and rigorous evaluation of its impact. The objectives of this American Thoracic Society Workshop Committee were to establish a consensus definition of a time-limited trial in critical care, identify the essential elements for conducting a time-limited trial, and prioritize directions for future work.
