Unraveling vulnerabilities in chronic lymphocytic leukemia (CLL) represents a key approach to understand molecular basis for its indolence and a path toward developing tailored therapeutic approaches. In this study, we found that CLL cells are particularly sensitive to the inhibitory action of abundant serum protein, apolipoprotein E (ApoE). Physiological concentrations of ApoE affect CLL cell viability and inhibit CD40-driven proliferation.

Article Synopsis
  • - The ELOQUENT-3 trial found that the combination of elotuzumab, pomalidomide, and dexamethasone (EloPd) is more effective and safer than pomalidomide and dexamethasone (Pd) for treating relapsed/refractory multiple myeloma (RRMM) patients who have undergone at least two prior therapies.
  • - An 18-month follow-up of 319 RRMM patients treated with EloPd in Italy revealed that 66.4% experienced disease progression or death, with median progression-free survival and overall survival recorded at 7.5 and 19.2 months, respectively.
  • - While EloPd remains a viable treatment option,

  • Light-chain amyloidosis (AL) is a rare condition where misfolded proteins build up in various organs, commonly affecting the heart, with 50-70% of patients showing symptoms at diagnosis.
  • A study analyzed 67 patients diagnosed with advanced cardiac AL to identify factors that influence overall survival (OS).
  • Key findings include that better OS is correlated with a performance status score (ECOG PS) of 1 or lower, a heart function classification (NYHA FC) of 2 or lower, and achieving specific responses to treatment (hematological and cardiac) after initial therapy.

Chronic Lymphocytic Leukemia (CLL) patients have a defective expression of the proapoptotic protein p66Shc and of its transcriptional factor STAT4, which evoke molecular abnormalities, impairing apoptosis and worsening disease prognosis and severity. p66Shc expression is epigenetically controlled and transcriptionally modulated by STAT4; epigenetic modifiers are deregulated in CLL cells and specific histone deacetylases (HDACs) like HDAC1, are overexpressed. Reactivation of STAT4/p66Shc expression may represent an attractive and challenging strategy to reverse CLL apoptosis defects.

Escape from immunosurveillance is a hallmark of chronic lymphocytic leukemia (CLL) cells. In the protective niche of lymphoid organs, leukemic cells suppress the ability of T lymphocytes to form the immune synapse (IS), thereby hampering T-cell mediated anti-tumoral activities. By binding its cognate receptor PD-1 at the surface of T lymphocytes, the inhibitory ligand PD-L1, which is overexpressed in CLL cells, mediates the T-cell suppressive activities of CLL cells.

The tumor microenvironment (TME) plays a central role in the pathogenesis of chronic lymphocytic leukemia (CLL), contributing to disease progression and chemoresistance. Leukemic cells shape the TME into a pro-survival and immunosuppressive niche through contact-dependent and contact-independent interactions with the cellular components of the TME. Immune synapse (IS) formation is defective in CLL.

The prognostic impact of achieving and in particular maintaining measurable residual disease (MRD) negativity in multiple myeloma is now established; therefore, identifying among MRD-negative patients the ones at higher risk of losing MRD negativity is of importance. We analyzed predictors of unsustained MRD negativity in patients enrolled in the FORTE trial (NCT02203643). MRD was performed by multiparameter flow cytometry (sensitivity of 10-5) at premaintenance and every 6 months thereafter.

Article Synopsis
  • Patients with chronic lymphocytic leukemia (CLL) have a significantly heightened risk of developing other malignancies (OMs), with a study tracking nearly 20,000 CLL patients revealing 3,513 OMs diagnosed over 129,254 years of follow-up.
  • The study found that treatment with fludarabine and cyclophosphamide increased the likelihood of developing myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML), while non-melanoma skin cancer (NMSC) and prostate cancer were common solid tumors in treated patients.
  • Importantly, patients with CLL who developed OMs had lower overall survival rates, especially those diagnosed with AML and MDS, highlighting that C

Introduction: Amyloid light-chain amyloidosis is a rare condition characterized by the abnormal production of immunoglobulin light chain that misshape and form amyloid fibrils. Over time, these amyloid deposits can accumulate slowly, causing dysfunction in organs and tissues. Early identification is crucial to ensure optimal treatment.

A panel of chronic lymphocytic leukemia (CLL) experts from Tuscany propose a real-life diagnostic and therapeutic approach CLL that considers the role of genomic and somatic prognostic factors in risk stratification and treatment decisions. Safety and efficacy of new agents has been demonstrated now not only in clinical trials but also in many real-world series. The BTK inhibitors, ibrutinib and acalabrutinib, and BH3 mimetic venetoclax are now indicated as first-line therapy and chemoimmunotherapy can be spared to the majority of CLL patients, thus preventing unnecessary hematological and non-hematological toxicity and second primary tumors.

Background: Hairy cell leukemia commonly presents with pancytopenia, indolent course, and predisposition as infectious complications. Current first-line therapeutic options are purine analogues, particularly cladribine, with a high percentage of complete responses and durable remissions. However, their use is poorly investigated in patients affected by severe chronic renal insufficiency.

Chronic lymphocytic leukemia (CLL) therapies differ in efficacy, side effects, route, frequency, and duration of administration. We assessed patient preferences for treatment attributes and evaluated associations with disease stage, treatment line, and socio-demographic characteristics in a cross sectional, observational study conducted at 16 Italian hematology centers. Study visits occurred between February and July 2020; 401 adult patients with CLL (201 Watch and Wait (W&W), 200 treated) participated in a discrete choice experiment (DCE), composed of 8 choices between pairs of treatment profiles with different levels of 5 attributes of currently available CLL treatments (length of response, route and duration of administration, risk of side effects including diarrhea, infections, or organ damage).

Article Synopsis
  • - A study analyzing 165 multiple myeloma (MM) patients found that those who developed second primary malignancies (SPMs), particularly hematological ones, generally had a shorter survival time and were more likely to have had previous cancer treatments.
  • - Patients with hematological SPMs tended to be younger and had a higher rate of prior autologous hematopoietic cell transplantation, with treatment often stopped upon SPM detection, especially in cases of hemato-SPM.
  • - Overall survival (OS) post-SPM detection was only 8.5 months, with the main cause of death being the SPM; hence, regular monitoring and early intervention are crucial for improving patient outcomes.

Multiple myeloma (MM) is characterized by multiple relapse and, despite the introduction of novel therapies, the disease becomes ultimately drug-resistant. The tumor microenvironment (TME) within the bone marrow niche includes dendritic cells, T-cytotoxic, T-helper, reactive B-lymphoid cells and macrophages, with a complex cross-talk between these cells and the MM tumor cells. Tumor-associated macrophages (TAM) have an important role in the MM pathogenesis, since they could promote plasma cells proliferation and angiogenesis, further supporting MM immune evasion and progression.

Here, the International Myeloma Working Group (IMWG) updates its clinical practice recommendations for the management of multiple myeloma-related renal impairment on the basis of data published until Dec 31, 2022. All patients with multiple myeloma and renal impairment should have serum creatinine, estimated glomerular filtration rate, and free light chains (FLCs) measurements together with 24-h urine total protein, electrophoresis, and immunofixation. If non-selective proteinuria (mainly albuminuria) or involved serum FLCs value less than 500 mg/L is detected, then a renal biopsy is needed.

Novel drug availability has increased the depth of response and revolutionised the outcomes of multiple myeloma patients. Minimal residual disease evaluation is a surrogate for progression-free survival and overall survival and has become widely used not-only in clinical trials but also in daily patient management. Bone marrow aspiration is the gold standard for response evaluation, but due to the patchy nature of myeloma, false negatives are possible.
