Early recognition and aggressive treatment of gastroesophageal reflux following repair of esophageal atresia.

Twelve patients with gastroesophageal reflux following repair of esophageal atresia are presented. Reflux produced recurrent stricture, failure to thrive, repeated pneumonitis, and in one patient, respiratory arrest and nearly death. Treatment consists of positional therapy or fundoplication operation both of which seem less successful in this combination of lesions than with GER and a normal esophagus.

Vertebral osteomyelitis due to Pseudomonas aeruginosa.

A case of vertebral osteomyelitis due to Pseudomonas aeruginosa is described and the problems of diagnosis and treatment discussed.

Lysis if enterobacteria by cefoxitin, cefuroxime, and cephalothin.

Cefoxitin, cefuroxime, and cephalothin were added to dense populations of beta-lactamase-producing enterobacteria, and the subsequent turbidity changes were monitored continuously. Viable counts and antibiotic assays were made at intervals after the addition of antibiotic, and the morphological appearances of the organisms were observed. Cephalothin caused lysis of most of the organisms tested, but even at high concentrations, after a few hours the antibiotic was destroyed and the organisms recommenced logarithmic growth.

Effects of chronic hypoxia on canine gastric secretion.

Interdigestive gastirc acid output and acid secretory response to histamine, insulin and pentagastrin were studied in five dogs before and after creation of a right to left extracardiac vascular shunt. The arterial PO2 decreased from 81.5 +/- 5.

Fracture healing in the presence of chronic hypoxia.

Recent studies have demonstrated that fracture healing normally takes place under hypoxic conditions. The present study evaluated the effect of a state of chronic systemic hypoxia on fracture healing in dogs. A significant delay in fracture healing was demonstrated in the hypoxic animals roentgenographically, histologically, and by strength-testing criteria.

In vivo control of mitochondrial enzyme concentrations and activity by oxygen.

Newborn and adult dog heart mitochondria were prepared from animals chronically adjusted to varying arterial oxygen tensions. Similarly, rat liver and heart mitochondria were isolated from animals acutely exposed to lowered inspired oxygen. After isolation, all mitochondrial samples were assayed under normoxic conditions.

Beta-lactamase resistance of cephazolin and other cephalosporins.

The turbidities of cultures of bacterial were monitored continuously at 37 degrees C. in a biophotometer. In the early phase of logarithmic growth, at approximately 2 X 10(7) organisms per ml.

Cephazolin treatment of pneumonia in the elderly.

Cephazolin, a semi-synthetic cephalosporin for parenteral use, was evaluated in 39 elderly hospital patients. Thirty-three of these patients were seriously ill at the start of treatment, suffering from pulmonary infections. In the other six patients, the drug was used post-operatively as a prophylactic, and it was effective in all cases in preventing any subsequent infection.

Computer printing and filing of microbiology reports. 2. Evaluation and comparison with a manual system, and comparison of two manual systems.

A manual system of microbiology reporting with a National Cash Register (NCR) form with printed names of bacteria and antiboitics required less time to compose reports than a previous manual system that involved rubber stamps and handwriting on plain report sheets. The NCR report cost 10-28 pence and, compared with a computer system, it had the advantages of simplicity and familarity, and reports were not delayed by machine breakdown, operator error, or data being incorrectly submitted. A computer reporting system for microbiology resulted in more accurate reports costing 17-97 pence each, faster and more accurate filing and recall of reports, and a greater range of analyses of reports that was valued particularly by the control-of-infection staff.

Computer printing and filing of microbiology reports. 1. Description of the system.

From March 1974 all reports from this microbiology department have been computer printed and filed. The system was designed to include every medically important microorganism and test. Technicians at the laboratory bench made their results computer-readable using Port-a-punch cards, and specimen details were recorded on paper-tape, allowing the full description of each specimen to appear on the report.

Improved methods for the study of hepatic HMG CoA reductase: one-step isolation of mevalonolactone and rapid preparation of endoplasmic reticulum.

Two new methods are described for the study of hepatic 3-hydroxy-3-methylglutaryl coenzyme A (HMG CoA) reductase. (1) Endoplasmic reticulum was rapidly prepared by diluting a 10,000 g supernatant with buffer containing 8 mM calcium chloride. The yield of protein and the specific activity of HMG CoA reductase in the pellet subsequently obtained by low speed centrifugation were nearly identical to those in the microsomal pellet prepared by ultracentrifugation.

Effects of rate of infusion and probenecid on serum levels, renal excretion, and tolerance of intravenous doses of cefoxitin in humans: comparison with cephalothin.

Using a randomized crossover design, 1-g intravenous doses of cephalothin and cefoxitin, a cephalosporinase-resistant cephamycin, were infused into 12 normal adult males over periods of 120, 30, and 3 min, the last with and without prior intravenous infusions of probenecid (1 g). Mean peak serum concentrations of antibiotic activity after cephalothin infusions were 23, 56, 103, and 102 mug/ml, respectively, and after cefoxitin infusions they were 27, 74, 115, and 125 mug/ml, respectively. Probenecid treatment prolonged the terminal serum half-life of cephalothin-like activity from 0.

An outbreak of common colds at an Antarctic base after seventeen weeks of complete isolation.

Six of 12 men wintering at an isolated Antarctic base sequentially developed symptoms and signs of a common cold after 17 weeks of complete isolation. Examination of specimens taken from the men in relation to the outbreak has not revealed a causative agent.
