Divide and structure: generating and interswitching orthogonal eigenstates of complementary petal beams using a π-shifted Sagnac interferometer.

One of the many facets of structured light are Ferris wheel/petal beams that can be generated by the addition/superposition of two beams with opposite vorticity/orbital angular momentum (OAM). We demonstrate a simple scheme employing a -shifted Sagnac interferometer (SI) containing a spiral phase plate (SPP) that divides and structures an incoming beam into two azimuthally complementary petal beams representing orthogonal eigenstates. The half-wave plate in the SI can interswitch/route these intensity patterns between the two outputs of the interferometer.

Testosterone therapy and the risk of cardiovascular disease in older, hypogonadal men.

The debate over the cardiovascular (CV) implications of testosterone therapy (TT) have resulted in diverging safety recommendations and clinical guidelines worldwide. This narrative review synthesizes and critically evaluates long-term studies examining the effects of TT within the context of aging, obesity, and endogenous sex hormones on CV disease (CVD) risk to support informed clinical decision-making. Observational studies have variably linked low endogenous testosterone with increased CVD risk, while randomized controlled trials (RCTs) demonstrate that TT yields cardiometabolic benefits without increasing short-term CV risk.

Cannabis Abuse Is Associated With Greater Medical Complications, Emergency Department Visits, and Readmissions Following Open Reduction and Internal Fixation for Distal Radius Fractures.

Background: Despite increased legalization, little is known about the influence of cannabis use disorder (CUD) following open reduction and internal fixation (ORIF) for distal radius fractures (DRFs). The aims were to determine whether CUD patients undergoing ORIF for DRF have increased: (1) medical complications; and (2) health care utilization (emergency department [ED] visits and readmission rates).

Methods: Patients were identified from an insurance database from 2010 to 2020 using Current Procedural Terminology codes: 25607, 25608, and 25609.

Social determinants of health in patients undergoing hemiarthroplasty: are they associated with medical complications, healthcare utilization, and payments for care?

Introduction: Social determinants of health (SDOH) have previously been shown to impact orthopedic surgery outcomes. This study assessed whether greater socioeconomic disadvantage in patients undergoing hemiarthroplasty following femoral neck fracture was associated with differences in (1) medical complications, (2) emergency department (ED) utilization, (3) readmission rates, and (4) payments for care.

Methods: A US nationwide database was queried for hemiarthroplasties performed between 2010 and 2020.

Methods and Reproducibility of Liver Fat Measurement Using 3-Dimensional Liver Segmentation From Noncontrast Computed Tomography in EVAPORATE Cohort.

Objective: Nonalcoholic fatty liver disease not only shares multiple risk factors with cardiovascular disease but also independently predicts its increased risk and related outcomes. Here, we evaluate reproducibility of 3-dimensional (3D) liver volume segmentation method to identify fatty liver on noncontrast cardiac computed tomography (CT) and compare measures with previously validated 2-dimensional (2D) segmentation CT criteria for the measurement of liver fat.

Methods: The study included 68 participants enrolled in the EVAPORATE trial and underwent serial noncontrast cardiac CT.

Divide and focus: generating novel focal polarization modalities by symmetry-based phase tailoring in one dimension.

Symmetry-based tailoring of photonic systems recently heralded the advent of novel concepts, such as photonic topological insulators and bound states in the continuum. In optical microscopy systems, similar tailoring was shown to result in tighter focusing, spawning the field of phase- and polarization-tailored light. Here, we show that even in the fundamental case of 1D focusing using a cylindrical lens, symmetry-based phase tailoring of the input field can result in novel features.

Increasing trend toward joint-preserving procedures for hip osteonecrosis in the United States from 2010 to 2019.

The objectives of this study were: (1) to use a nationwide sample of patients to report the incidence and timing of C. difficile colitis in geriatric patients who underwent surgery for hip fractures, (2) to identify preoperative factors associated with developing C.

A highly complex crystal structure of stoichiometric Mg()(BH) was solved from single crystal synchrotron X-ray diffraction and confirmed by neutron powder diffraction (NPD) on isotopically substituted Mg()(BD). We highlight the role of the amorphous Mg(BH) in the reactivity of the Mg(BH)- system and characterized a previously overlooked phase, Mg()(BH).

Major Global Coronary Artery Calcium Guidelines.

This review summarizes the framework behind global guidelines of coronary artery calcium (CAC) in atherosclerotic cardiovascular disease risk assessment, for applications in both the clinical setting and preventive therapy. By comparing similarities and differences in recommendations, this review identifies most notable common features for the application of CAC presented by different cardiovascular societies across the world. Guidelines included from North America are as follows: 1) the 2019 American College of Cardiology/American Heart Association Guideline on the Primary Prevention of Cardiovascular Disease; and 2) the 2021 Canadian Cardiovascular Society Guidelines for the Management of Dyslipidemia for Prevention of Adult Cardiovascular Disease.

Vectorial spin Hall effect of light upon tight focusing.

The spin Hall effect of light is a manifestation of angular momentum conservation in the process of spin-orbit interaction of light. This optical Hall effect is exhibited in tight focusing of a circularly polarized asymmetric input beam as a shift of the center of gravity of the focal spot in the transverse plane, perpendicular to the direction/axis of symmetry breaking. It is commonly established that the direction of this shift depends on the sign of the spin.

Arterial stiffness and left ventricular structure assessed by cardiac computed tomography in a multiethnic population.

Background: Arterial stiffness expressed by cardio-ankle vascular index (CAVI) is a marker of arteriosclerosis. It can increase vascular load, which in turn may affect the viscoelastic myocardial properties and the left ventricular compliance. In the present study, we sought to investigate the association between CAVI and left ventricular structure assessed by cardiac computed tomography (CT) in a multiethnic adult cohort.

Introduction: Studies evaluating the association of dementia in patients undergoing total hip arthroplasty (THA) for femoral neck fractures are limited. The aim was to investigate whether patients who have dementia undergoing THA for femoral neck fractures have higher rates of (1) in-hospital lengths of stay (LOS); 2) complications (medical and prostheses-related); and 3) healthcare expenditures.

Methods: A retrospective query using the PearlDiver database from January 1st, 2005 to March 31st, 2014 to identify patients with dementia undergoing primary total hip arthroplasty for the treatment of femoral neck fractures was performed.

Purpose: This study analyzed the effectiveness of minimizing surgical trauma, reducing the severity of the stress reaction, and restoring the normal functioning of the body after planned gallbladder operations under an enhanced recovery program.

Materials And Methods: This prospective comparison study included 30 patients from the surgical department of Irkutsk Clinical Hospital No. 1 who had been diagnosed with cholelithiasis in 2019-2020.

Aortic Arch Calcification in Ungated Lung Computed Tomography Scans as Predictors of Atherosclerotic Cardiovascular Disease: Methodology and Reproducibility in a Multi-Ethnic Study of Atherosclerosis.

Background: Aortic arch calcium (AAC), which is frequently detected on ungated lung computed tomography (CT) due to a large field of view, can serve as a marker of subclinical atherosclerotic burden. Our study sought to validate novel cardiac screening metrics of subclinical atherosclerosis by evaluating the inter- and intra-observer reproducibility of AAC measurements with ungated lung CT.

Methods: The authors randomly selected 100 ungated lung CT scans from the Multi-Ethnic Study of Atherosclerosis cohort.
