Article Synopsis
  • Kirchhoff's law outlines the fundamental limits of thermal radiation, suggesting that breaking the principle of electromagnetic reciprocity could enhance thermal efficiency in engineering.
  • A new 1D photonic crystal design, using Weyl semimetals and dielectric layers, was created to optimize nonreciprocal infrared radiation absorptance without needing additional coupling structures like gratings.
  • The design process utilized genetic algorithms for global optimization and gradient ascent for local enhancement, resulting in a compact eight-layer magnetophotonic crystal that improves absorptance while reducing unwanted reciprocal light absorption.

Introduction: Lupus nephritis (LN) is a frequent complication of SLE, occurring in up to 60% of adult patients and ultimately progressing from acute inflammation to chronicity with fibrosis and end-stage kidney failure in 10%-30% of patients. Racial/ethnic minority patients with lupus have worse long-term outcomes, including progression to end-stage renal disease and overall mortality. A major challenge in the management of patients with SLE is delayed identification of early kidney disease, which ultimately leads to a greater burden on both patients and the health system.

A surveillance system for measuring patient-level antimicrobial adverse drug events (ADE) may support stewardship activities, however, design and implementation questions remain. In this national survey, stewardship experts favored simple, laboratory-based ADE definitions although there were tensions between feasibility, ability to identify attribution without chart review, and importance of specific ADE.

Objectives: Glucagon-like peptide-1 receptor agonists (GLP1-RA) are an emerging class of medications with demonstrated promise in improving cardiometabolic outcomes. Whether these drugs may be useful in mitigating the cardiac risk associated with SLE remains unknown, and a recent case of drug induced lupus secondary to GLP1-RA use calls the safety of GLP1-RAs in SLE patients into question. Accordingly, this retrospective analysis was initiated to evaluate outcomes of GLP1-RAs in SLE.

Article Synopsis
  • The Manhattan Lupus Surveillance Program (MLSP) investigated cardiovascular disease events in systemic lupus erythematosus (SLE) patients and compared their rates to population controls based on sex, age, and race/ethnicity.
  • In a study of 1,285 SLE patients, 13.9% experienced cardiovascular events, with higher risks associated with being male and over the age of 60.
  • The study found that Hispanic/Latino and non-Hispanic Black patients had significantly elevated risks for cardiovascular events compared to other racial groups, highlighting a higher overall prevalence of these events among SLE patients compared to the general population.

Article Synopsis
  • The study investigates how isolated lupus nephritis (LN) impacts health-related quality of life (HRQOL) compared to patients with additional extrarenal symptoms.
  • It involved 181 LN patients who completed the PROMIS-29 questionnaire, revealing that those with extrarenal symptoms reported worse pain, social participation, physical function, and fatigue than those with just renal symptoms.
  • The findings emphasize the need for treatment approaches that consider both renal and extrarenal issues to enhance overall patient well-being.

Intraoperative tumor thrombus embolization is a potentially lethal complication during inferior vena cava (IVC) thrombectomy for renal cell carcinoma (RCC). Intraoperative embolization is uncommonly encountered because IVC thrombectomy surgical technique is focused on avoiding this complication. Nonetheless, early recognition of embolization is essential so that emergent management can be instituted.

Effective de-implementation models often include replacement of an ineffective practice with an alternative. We co-developed patient education materials as a replacement strategy for inappropriate post-procedural antibiotics in cardiac device procedures. Lessons learned and developed materials may be used to promote infection prevention in other periprocedural settings.

Adequate nutrition is central to well-being and health and can enhance recovery during illness. Although it is well known that malnutrition, both undernutrition and overnutrition, poses an added challenge for patients with cancer diagnoses, it remains unclear when and how to intervene and if such nutritional interventions improve clinical outcomes. In July 2022, the National Institutes of Health convened a workshop to examine key questions, identify related knowledge gaps, and provide recommendations to advance understanding about the effects of nutritional interventions.

5-Fluorouracil and 5-fluorouracil-based prodrugs have been used clinically for decades to treat cancer. Their anticancer effects are most prominently ascribed to inhibition of thymidylate synthase (TS) by metabolite 5-fluoro-2'-deoxyuridine 5'-monophosphate (FdUMP). However, 5-fluorouracil and FdUMP are subject to numerous unfavorable metabolic events that can drive undesired systemic toxicity.

Economic evaluation for implementation science merits unique considerations for a local context, including the main audience of local decision makers. This local context is in contrast with traditional methods for developing coverage policy for medical tests and interventions, which typically emphasize benefits and costs more broadly, for society. Regardless of the strength of evidence backing the efficacy or effectiveness of a clinical intervention, local context is paramount when implementing evidence-based practices.
