Article Synopsis
  • Silver-Russell Syndrome (SRS) is a disorder that leads to growth failure, characteristic physical features, and feeding issues, with significant genetic causes remaining unclear in many cases.
  • The study aimed to assess the genetic variants in undiagnosed SRS patients and determine if (epi)genetic patients show distinct characteristics compared to genetic patients.
  • Findings revealed that only 9.1% of patients had identifiable pathogenic variants, emphasized body asymmetry as a key trait in (epi)genetic SRS, and recommended including IGF1R sequencing in the diagnostic process for SRS.

Article Synopsis
  • - Cushing's syndrome (CS) is caused by too much cortisol in the body, either from external glucocorticoid-related treatments or from the body producing it excessively, with childhood cases mostly stemming from medication use rather than natural causes.
  • - The most frequent type of endogenous CS in kids is due to pituitary tumors that release ACTH, with adrenal-related causes being less common but still significant, especially in early childhood.
  • - Diagnosing CS can be challenging due to its gradual onset and subtle symptoms; key signs include changes in facial appearance, weight gain, and growth issues, and accurate diagnosis usually requires multiple tests to confirm hypercortisolism.

The management of adrenal insufficiency is challenging, and the overall goals of treatment are to prevent life-threatening adrenal crises, to optimize linear growth, to control androgen levels without overdosing in subjects with congenital adrenal hyperplasia (CAH) and to improve quality of life in affected individuals. Standard glucocorticoid formulations fail to replicate the circadian rhythm of cortisol and control the adrenal androgen production driven by adrenocorticotropic hormone. In order to personalize and tailor glucocorticoid therapy and to improve patient outcomes, new pharmacological strategies have been developed that best mimic physiological cortisol secretion.

Background: Prader-Willi syndrome (PWS) is a rare and complex genetic disease, with numerous implications on metabolic, endocrine, neuropsychomotor systems, and with behavioural and intellectual disorders. Rare disease patient registries are important scientific tools (1) to collect clinical and epidemiologic data, (2) to assess the clinical management including the diagnostic delay, (3) to improve patients' care and (4) to foster research to identify new therapeutic solutions. The European Union has recommended the implementation and use of registries and databases.

Diabetes mellitus (DM) has been frequently associated with an impaired immune response against infectious agents, making affected patients at risk for more severe disease and sometimes causing worse outcomes. The recent COVID-19 pandemic has seriously affected patients with both diabetes, in particular those carrying comorbidities or with poor glycemic control. As regards pediatric diabetes mellitus, the availability of more accurate and technological tools for glycemic management and the improved markers of metabolic control might mitigate the negative impact of infections.

Context: The etiology of central precocious puberty (CPP) includes a spectrum of conditions. Girls younger than age 6 years with CPP should undergo cranial magnetic resonance imaging (MRI), but it remains controversial whether all girls who develop CPP between the ages of 6 and 8 years require neuroimaging examination.

Objective: To investigate the frequency of brain MRI abnormalities in girls diagnosed with CPP and the relationship between maternal factors, their age at presentation, clinical signs and symptoms, hormonal profiles, and neuroimaging findings.

Article Synopsis
  • - The study focused on evaluating how rhGH treatment affects growth and adult height in children with Noonan syndrome (NS), particularly those with growth hormone deficiency (GHD).
  • - Researchers analyzed data from 228 NS patients over key ages, finding that rhGH treatment significantly improved height standard deviation scores, leading to height gains during treatment.
  • - Despite the improvement, there were no notable differences in height measurements between treated and untreated patients at adult height; further research is suggested to understand genotype-specific responses to rhGH.

Article Synopsis
  • * Diagnosing CDI is complex and involves a thorough medical history, physical exams, lab tests, imaging (like MRI), and sometimes tissue analysis to identify the underlying causes.
  • * Early detection and monitoring of CDI are vital to prevent serious complications, as conditions may evolve over time, and symptoms like improved water intake and urine output can mask underlying damage to the hypothalamus and pituitary.

Growth hormone (GH) deficiency (GHD) in children is defined as impaired production of GH by the pituitary gland that results in growth failure. This disease might be congenital or acquired, and occurs in isolation or in the setting of multiple pituitary hormone deficiency. Isolated GHD has an estimated prevalence of 1 patient per 4000-10,000 live births and can be due to multiple causes, some of which are yet to be determined.
