Article Synopsis
  • Fatigue is a common issue in patients with inflammatory bowel disease (IBD), with 54.1% experiencing fatigue and 37.1% reporting severe fatigue.
  • Patients with active IBD experience significantly higher levels of fatigue compared to those with inactive disease.
  • Key factors affecting fatigue include age, gender, sick leave, physical pain, emotional well-being, and sleep quality, suggesting that a holistic treatment approach is essential for managing IBD-related fatigue.

Objectives: The emergence of biologics has improved the course of inflammatory bowel diseases (IBD) in the elderly population despite a potential higher risk of infections. We conducted a one-year, prospective, multicenter, observational study to determine the frequency of occurrence of at least one infectious event in elderly IBD patients under anti-TNF therapy compared with that in elderly patients under vedolizumab or ustekinumab therapies.

Methods: All IBD patients over 65 years exposed to anti-TNF, vedolizumab or ustekinumab therapies were included.

Background: The combination of infliximab and immunosuppressant therapy is a standard management strategy for patients with Crohn's disease. Concerns regarding the implications of long-term combination therapy provided the rationale for a formal clinical trial of treatment de-escalation. Our aim was to compare the relapse rate and the time spent in remission over 2 years between patients continuing combination therapy and those stopping infliximab or immunosuppressant therapy.

Article Synopsis
  • Anti-TNFα medications are recommended to prevent postoperative recurrence of Crohn's disease in patients with risk factors, yet there's limited data on their effectiveness after prior failures.
  • A study was conducted comparing the success of anti-TNFα against other therapies in preventing recurrence in Crohn's patients who had previously failed anti-TNFα treatment.
  • Results showed that only 23.9% of patients using anti-TNFα experienced recurrence within two years, compared to 44.9% in those using alternative treatments, suggesting anti-TNFα is still a viable option.

Introduction: The objective of this study was to describe the efficacy and safety of infliximab (IFX) reintroduction in Crohn's disease (CD) after stopping for loss of response or intolerance.

Methods: We conducted a prospective multicenter observational cohort study including adult patients with clinically (CD Activity Index >150) and objectively active luminal CD in whom IFX was reintroduced after at least 6 months of discontinuation. The reasons for the initial discontinuation could be a secondary loss of response or IFX intolerance.

Introduction: There are currently no comparative data on the efficacy and safety of vedolizumab and ustekinumab in ulcerative colitis (UC) after anti-TNF therapy fails.

Methods: We retrieved the full datasets of two observational, multicentre, retrospective studies of patients with UC for whom anti-TNF therapy failed and the patients were then treated with either vedolizumab or ustekinumab. The outcomes included steroid-free clinical remission, clinical remission, treatment persistence, colectomy, hospitalization, and serious and infectious adverse events.

Association of vulvar Crohn's disease (CD) with pyoderma gangrenosum (PG) has been described, yet due to its low prevalence, data on therapeutic management of this association are still limited. We here present a 22-year-old woman with severe vulvar and perianal CD with a major inguinal and perineal ulceration. In the hypothesis of an associated PG, oral corticosteroids were prescribed at a dose of 40 mg with limited efficacy for a duration of 3 months.
