Melanoma remains a formidable challenge in oncology, causing the majority of skin cancer deaths in the United States, with brain metastases contributing substantially to this mortality. This paper reviews the current therapeutic strategies for melanoma brain metastases, with a focus on delayed and concurrent stereotactic radiosurgery (SRS). While surgery and traditional chemotherapy offer limited efficacy, recent advances in immunotherapy, particularly immune checkpoint inhibitors (ICIs), have played a major role in the advancement and improved efficacy of the treatment of cancers, including brain metastases.

Rare side effects of immune-checkpoint inhibitors (ICIs) are known as neurological immune-related adverse events (n-irAEs). Typically, n-irAEs affect the peripheral nervous system, primarily presenting as myositis, polyradiculoneuropathy, or cranial neuropathy. Less commonly, they impact the central nervous system, resulting in encephalitis, meningitis, or myelitis.

Deep learning (DL) has been demonstrated to be a valuable tool for analyzing signals such as sounds and images, thanks to its capabilities of automatically extracting relevant patterns as well as its end-to-end training properties. When applied to tabular structured data, DL has exhibited some performance limitations compared to shallow learning techniques. This work presents a novel technique for tabular data called adaptive multiscale attention deep neural network architecture (also named excited attention).

Article Synopsis
  • Leukemias are common cancers, and bone marrow mesenchymal stem cells (MSCs) play a role in forming a supportive environment for blood stem cells, potentially influencing leukemia development.
  • The study examined the impact of secretomes (substances secreted by cells) from healthy and AML patient-derived MSCs on leukemia cell lines, assessing cell growth, survival, and gene expression related to drug resistance.
  • Results showed that the secretome from healthy MSCs reduced the proliferation and viability of leukemia cells, increased cell death, and activated specific proteins, while MSCs from AML patients did not exhibit these effects, suggesting they may aid in leukemia progression instead.

Background: Patients with relapsed intracranial germinoma can achieve durable remission with standard chemotherapy regimens and/or reirradiation; however, innovative therapies are required for patients with relapsed and/or refractory intracranial nongerminomatous germ cell tumors (NGGCTs) due to their poor prognosis. Improved outcomes have been reported using reinduction chemotherapy to achieve minimal residual disease, followed by marrow-ablative chemotherapy (HDCx) with autologous hematopoietic progenitor cell rescue (AuHPCR). We conducted a phase II trial evaluating the response and toxicity of a 3-drug combination developed for recurrent intracranial germ cell tumors consisting of gemcitabine, paclitaxel, and oxaliplatin (GemPOx).

Background: Glioblastoma is a malignant brain tumor requiring careful clinical monitoring even after primary management. Personalized medicine has suggested the use of various molecular biomarkers as predictors of patient prognosis or factors utilized for clinical decision-making. However, the accessibility of such molecular testing poses a constraint for various institutes requiring identification of low-cost predictive biomarkers to ensure equitable care.

Post-resection radiologic monitoring to identify areas of new or progressive enhancement concerning for cancer recurrence is critical during patients with glioblastoma follow-up. However, treatment-related pseudoprogression presents with similar imaging features but requires different clinical management. While pathologic diagnosis is the gold standard to differentiate true progression and pseudoprogression, the lack of objective clinical standards and admixed histologic presentation creates the needs to (1) validate the accuracy of current approaches and (2) characterize differences between these entities to objectively differentiate true disease.

Article Synopsis
  • Isocitrate dehydrogenase (IDH)-mutant grade 2 gliomas are aggressive brain tumors, and vorasidenib is a promising oral treatment that targets these mutations and showed preliminary effectiveness.
  • A randomized phase 3 trial involved 331 patients with untreated residual or recurrent gliomas, comparing vorasidenib to a placebo over 28-day cycles, focusing on progression-free survival as the main outcome.
  • Results indicated that patients taking vorasidenib had significantly longer progression-free survival (27.7 months) compared to those on placebo (11.1 months) and experienced better outcomes before needing further treatment, although adverse effects were more common in the vorasidenib group.

Purpose: To describe, in controls and in a population with patellar instability, magnetic resonance imaging values of measurements representing major associated factors for patellar instability (patellar height, trochlear dysplasia, and extensor mechanism alignment), as well as their cutoff values.

Methods: In total, 323 knee magnetic resonance imaging scans, 142 with patellar instability and 181 controls without patellofemoral complaints (anterior cruciate, medial collateral ligament, meniscus ruptures or normal) were evaluated. Means, normality values in the control population, ideal cutoff values through receiver operating characteristic curves analysis, and interobserver reliability (intraclass correlation coefficient) were described for a series of measurements.

Purpose: Isocitrate dehydrogenase () and mutations (mt) are frequent in glioma. Preclinical studies suggest mts confer "BRCAness" phenotype, a vulnerability that can be targeted through PARP inhibition. To test this hypothesis, we conducted a multicenter study of olaparib monotherapy in patients with mt gliomas.

Background: Primary central nervous system lymphoma (PCNSL) lesions often show avid contrast enhancement on T1-weighted contrast-enhanced MRI sequences. However, several case reports and a clinical study have described PCNSL in patients with no contrast enhancement on MRI. We assessed whether overall survival (OS) time was related to any tumor characteristics (lesion location, volume, and number; contrast enhancement; necrosis; proximity to the subarachnoid space; and edema) on MRI in patients with PCNSL.

Nitric oxide (NO) production in the tumor microenvironment is a common element in cancer. S-nitrosylation, the post-translational modification of cysteines by NO, is emerging as a key transduction mechanism sustaining tumorigenesis. However, most oncoproteins that are regulated by S-nitrosylation are still unknown.
