Although an effective whooping cough vaccine has been available in the UK since the 1950s, its current association with neurotoxicity has resulted in poor uptake: as a consequence major epidemics (with significant morbidity and mortality) are still being experienced. Component (sub-unit) vaccines, which incorporate those antigens thought to be concerned with generating a protective effect, have been developed and are now available for field testing. This paper addresses how such a vaccine might be evaluated, the organization of a trial and the difficulties to be expected.

We report on a study of 6 cases of Necrotising Sarcoidal Granulomatosis (NSG) and details of another problem case illustrating the difficulties of classification. We have compared the clinical and pathological features with those of sarcoidosis, Wegener's Granulomatosis (WG), Churg Strauss Granulomatosis (CSG), Pulmonary Hyalinizing Granuloma (PHG) and various metal lung diseases. We distinguish NSG from sarcoidosis, in particular by the prominence of vasculitis, necrosis and the rarity of extrapulmonary manifestations, but accept that problem cases can occur.

Lungs from seven cases of diffuse pleural fibrosis with known asbestos exposure were studied to determine the gross and microscopic pathological features and relate these to the analysed mineral fibre content of the lung. All seven individuals had had substantial exposure, ranging from two to 25 years, and chronic chest problems and at necropsy all cases met the criteria for compensatable disease. Macroscopically, all had extensive visceral pleural fibrosis and extensive areas of adhesions, and four also had discrete parietal pleural plaques.

A protein inhibitor was isolated from commercial preparations of salmon sperm and its physical and anticoagulant properties were compared with an inhibitor isolated earlier from commercial preparation of bovine testicular hyaluronidase. The inhibitor from bovine source was heat and acid labile and had a molecular weight of approximately equal to 35000 while the one from salmon sperm had a molecular weight of approximately equal to 5700 and was stable to heat and acid. To determine the mechanism of the inhibitory effect, a system of purified components consisting of isolated prothrombin, Factor Xa, Factor Va, Ca++, and vesicles of phosphatidylcholine (PCPS, 25% PS) was used.

The relationships of twelve tymoviruses have been assessed by cDNA-RNA hybridization. In addition, the percentage molar nucleotide composition of the genome of the PD strain of Kennedya yellow mosaic virus and the percentage molar amino acid composition of the coat proteins of cacao yellow mosaic, Kennedya yellow mosaic and turnip yellow mosaic (Cardamine strain) viruses were estimated. These as well as published serological comparisons and genome and coat protein composition determinations were used to compute classifications of tymoviruses using various "metrics", and simple numerical methods were used to compare the classifications.

Comparisons of the nucleotide sequences of viral genomes, and derived amino acid sequences, mostly confirm the traditional taxonomic groupings of viruses. These comparisons have also shown unexpected homologies between genes of viruses from different groups previously thought to be unrelated, and between some viral and non-viral genes. Comparisons of the three-dimensional structures of the particle proteins of some viruses have also revealed unexpected relationship, and, together with the sequence homologies, suggest that some ancestral viruses had 'modular' origins.

The genomic sequences of four isolates of turnip yellow mosaic virus (TYMV-Cd) from Australia, and three TYMV-1 (type) and three TYMV-2 (cauliflower) isolates from Europe were compared by cDNA-RNA hybridization tests, by analysis of the fragments produced from cDNA-RNA hybrids by restriction endonuclease treatment, and by determining the 3' terminal nucleotide sequences of their coat protein mRNAs. All three methods showed only slight differences (ca. 1%) between the mRNA sequences of different TYMV-1 and TYMV-Cd isolates, and did not distinguish between those groups of isolates.

A review of case notes of patients attending a general hospital psychiatric outpatient clinic revealed that 75% of patients with depressive disorders and 38% of patients with neurotic disorders were prescribed antidepressants. Antidepressants were often prescribed for long periods in low doses and in combination with benzodiazepines. There was less polypharmacy, however, with more sedating antidepressants.

In order to test the hypothesis that patients who are predisposed to depression have an enduring cognitive style, dysfunctional attitudes (Dysfunctional Attitudes Schedule), neuroticism and extraversion (Maudsley Personality Inventory), and severity of depression (Levine-Pilowsky Depression Questionnaire) were measured in a survey of former patients with previous diagnoses of either depressive or nondepressive psychiatric conditions. We found that there were no significant differences in dysfunctional attitudes between these groups of patients and that their scores were similar to those reported for normal populations. There was, however, a correlation between introversion and high dysfunctional attitude scores.

A combined clinical, radiological, pathological, and mineralogical study was undertaken on 62 cases referred to the Medical Research Council Pneumoconiosis Unit by the Cardiff Pneumoconiosis Panel as Cornish china clay workers. Considerable pathological lesions were found in the lungs, both nodular and interstitial fibroses being present. Some men had worked with china stone but others had worked entirely with china clay.

The tissue surrounding carbon fibre reinforced epoxy resin plates applied to forearm and tibial fractures was biopsied in 32 patients at the time the plates were removed. The reaction was minimal and was compared with that in a control group of 16 similar patients in whom stainless steel plates were used. No significant histological differences were found.

The structural requirements for the inhibition of ferrochelatase by N-alkylated porphyrins were investigated and experiments carried out to explore the mechanism of enzyme inhibition. Three dicarboxylic porphyrins, all substrates of the enzyme, are strongly inhibitory when N-alkylated; in contrast, uroporphyrin and coproporphyrin (which are not substrates) do not inhibit after N-alkylation. Free carboxylic acid functions are required for inhibition, as the methyl ester derivatives are not themselves inhibitory.

Immunohistological methods were used to investigate the presence of carcinoembryonic antigen, beta 1 pregnancy specific glycoprotein, beta subunit of human chorionic gonadotrophin, human placental lactogen, calcitonin, and keratin in formalin fixed tissue from 29 malignant mesotheliomas and 27 pulmonary adenocarcinomas. Malignant mesotheliomas were negative for tumour markers except for the beta subunit of human chorionic gonadotrophin and keratin, one and 13 cases respectively being positive for these. Pulmonary adenocarcinomas, however, were frequently positive for tumour markers--namely, carcinoembryonic antigen, beta 1 pregnancy specific glycoprotein, beta subunit of human chorionic gonadotrophin, human placental lactogen, calcitonin, and keratin.

An immunohistochemical investigation of vimentin, an intermediate filament, was carried out on formalin fixed paraffin embedded sections of 44 malignant mesotheliomas of the pleura and 24 pulmonary adenocarcinomas, in order to assess its value in differential diagnosis. Seventy-five percent of the malignant mesotheliomas showed positive staining for vimentin. An unexpected finding was the presence of vimentin in 46% of the pulmonary adenocarcinomas.

Two hundred female rats, 91-101 days of age, were used in this investigation which provided data on cell content and proliferative response of the thymus. The observations began in oestrous phase virgin females from two highly inbred strains AO(RT1u/AgB2) and DA(RT1a/AgB4), continued at five day intervals throughout syngeneic and allogeneic pregnancy in both strains and were completed in the immediate post-partum period. This experimental protocol permitted sequential observations throughout gestation and allowed syngeneic: allogeneic and interstrain comparisons to be made.

A prospective study of 125 elderly patients (over 65 years) with proximal femoral fracture was carried out to compare pre-fracture sociomedical factors with the local and general complications following surgical treatment, in predicting mortality and length of stay. A poor pre-fracture mental state was more important than associated physical illness in predicting mortality. Pre-fracture mobility state was the most useful early prognostic predictor of length of hospitalization.

Maps of restriction endonuclease sites in the DNA genomes of orthopoxviruses were compared by computer classification methods. Data from such maps were recorded as qualitative binary information, and gave closely similar dendrograms when different dissimilarity measures and sorting strategies were used for the computations. A method for calculating dendrograms from small data sets, by 'hand', is described in detail to assist those who do not have access to suitable computer programs.

One hundred and sixty two people working in various departments of cotton spinning and weaving mills measured and recorded their own peak expiratory flow rate (PEFR) at two hourly intervals during Monday, Wednesday, and Thursday of the same work week, from waking in the morning throughout the day until going to bed and a last time the following morning after waking.
