Atypical femur fractures (AFFs) have been reported with long term use of anti-resorptive drugs. Early identification is crucial because it allows early intervention to stop the progression to complete fracture, thus potentially reducing the ensuing burden. It has been shown that extending the scan image to take a full-length image of the femur (FFI) using single energy (SE) X-rays at the time of a dual-energy X-ray absorptiometry (DXA) scan can detect findings in the spectrum of AFF.

Context: Evidence for a beneficial role of vitamin D on blood pressure (BP) outcomes is inconclusive.

Objective: This work aimed to investigate the effect of 2 doses of cholecalciferol (vitamin D) supplementation coadministered with calcium on systolic blood pressure (SBP) and diastolic blood pressure (DBP).

Methods: Exploratory analyses were conducted from a 1-year, multicenter, double-blind, randomized controlled trial (RCT).

Article Synopsis
  • Vitamin D deficiency is common after bariatric surgery and may negatively affect bone health, but the ideal supplementation dosage is unclear.
  • This study aims to compare the effects of different vitamin D doses on adults with obesity undergoing bariatric surgery, including low, moderate, and high doses versus placebo.
  • Five trials with a total of 314 participants were analyzed, indicating that moderate-dose vitamin D (3200 IU/day) could improve vitamin D status with minimal adverse effects compared to a placebo.

Article Synopsis
  • The primary goal of osteoporosis management is to prevent fractures through individualized, goal-directed treatment plans.
  • Effective treatment requires assessing a patient's fracture history, bone mineral density (BMD), and other major risk factors to determine the best course of action.
  • Initial treatment should prioritize rapid fracture risk reduction for high-risk patients and take into account the likelihood of meeting BMD targets with either osteoanabolic or antiresorptive therapies.

This retrospective study investigates the prevalence of atypical femoral fractures (AFFs) among patients admitted with hip and shaft fractures at a tertiary referral center in Beirut, Lebanon. We analyzed electronic medical records and radiology studies of patients aged above 40 admitted with hip and shaft fractures between January 2006 and December 2019. Fractures were confirmed by ICD9 or ICD10 codes.

The Mediterranean Region registers an increasing prevalence of obesity. The region lacks a diet screener to assess obesogenic nutrients. This study aimed to evaluate the reproducibility and validity of the Modified Mediterranean Prime Screen (MMPS) in estimating obesogenic nutrients' intake among women of reproductive age, as compared with a culturally validated Food Frequency Questionnaire (FFQ), in Lebanon.

Background: Developing countries have a significantly higher incidence of breast cancer in patients younger than 40 years as compared to developed countries. This study aimed to examine if young age at diagnosis is an independent prognostic factor for worse survival outcomes in breast cancer as well as the effect of age on Disease-free survival (DFS) and local recurrence free survival (LRFS) after adjusting for various tumor characteristics, local and systemic treatments.

Methods: This is a secondary analysis of prospective cohort of patients from two existing databases.

The 6th International Conference, "Controversies in Vitamin D," was convened to discuss controversial topics, such as vitamin D metabolism, assessment, actions, and supplementation. Novel insights into vitamin D mechanisms of action suggest links with conditions that do not depend only on reduced solar exposure or diet intake and that can be detected with distinctive noncanonical vitamin D metabolites. Optimal 25-hydroxyvitamin D (25(OH)D) levels remain debated.

EndoBridge 2023 took place on October 20-22, 2023, in Antalya, Turkey. Accredited by the European Council, the 3-day scientific program of the 11 Annual Meeting of EndoBridge included state-of-the-art lectures and interactive small group discussion sessions incorporating interesting and challenging clinical cases led by globally recognized leaders in the field and was well attended by a highly diverse audience. Following its established format over the years, the program provided a comprehensive update across all aspects of endocrinology and metabolism, including topics in pituitary, thyroid, bone, and adrenal disorders, neuroendocrine tumors, diabetes mellitus, obesity, nutrition, and lipid disorders.

Various clinical manifestations of thyroiditis after parathyroidectomy have been reported in the literature, ranging from mild symptoms to tachyarrhythmias and myocardial infarction. We report 2 cases of post-parathyroidectomy thyroiditis. Both patients had primary hyperparathyroidism and underwent parathyroidectomy for a solitary parathyroid adenoma.

Background: Vitamin D supplements are widely used for improving bone health in children and adolescents, but their effects in vitamin D-deficient children are unclear.

Objectives: This study aimed to examine whether the effect of vitamin D supplementation on bone mineral density (BMD) in children and adolescents differs by baseline vitamin D status and estimate the effect in vitamin D-deficient individuals.

Methods: This is a systematic review and individual participant data (IPD) meta-analysis.

The Middle East region experiences a high prevalence of vitamin D deficiency, yet most genetic studies on vitamin D have focused on European populations. Furthermore, there is a lack of research on the genomic risk factors affecting elderly people, who are more susceptible to health burdens. We investigated the genetic determinants of 25-hydroxyvitamin D concentrations in elderly Lebanese individuals ( = 199) through a whole-exome-based genome-wide association study.

Multiple changes occur across various endocrine systems as an individual ages. The understanding of the factors that cause age-related changes and how they should be managed clinically is evolving. This statement reviews the current state of research in the growth hormone, adrenal, ovarian, testicular, and thyroid axes, as well as in osteoporosis, vitamin D deficiency, type 2 diabetes, and water metabolism, with a specific focus on older individuals.

Background: Hypercalcemia of malignancy (HCM) is the most common metabolic complication of malignancies, but its incidence may be declining due to potent chemotherapeutic agents. The high mortality associated with HCM has declined markedly due to the introduction of increasingly effective chemotherapeutic drugs. Despite the widespread availability of efficacious medications to treat HCM, evidence-based recommendations to manage this debilitating condition are lacking.

Background: Integrating shared decision making between patients and physicians and incorporating their values and preferences in the development of clinical practice guidelines (CPGs) is of critical importance to optimize CPG implementation and treatment adherence. This applies to many debilitating diseases, including hypercalcemia of malignancy (HCM).

Objective: Evaluate patient and physician values, preferences, and attitudes to better inform CPGs to treat HCM in adults.

The last international guidelines on the evaluation and management of primary hyperparathyroidism (PHPT) were published in 2014. Research since that time has led to new insights into epidemiology, pathophysiology, diagnosis, measurements, genetics, outcomes, presentations, new imaging modalities, target and other organ systems, pregnancy, evaluation, and management. Advances in all these areas are demonstrated by the reference list in which the majority of listings were published after the last set of guidelines.

Article Synopsis
  • This review examines the current understanding of primary hyperparathyroidism (PHPT), highlighting both classical and nonclassical symptoms, and is based on extensive literature research.
  • * The shift towards asymptomatic presentations of PHPT presents new challenges, particularly regarding the unclear progression of such cases and their potential links to cardiovascular and neuropsychological issues.
  • * The review calls for a clearer definition of asymptomatic PHPT, suggesting the need for more refined diagnostic criteria and further research into its various manifestations and impact on quality of life.
