Left ventricular performance during percutaneous transluminal coronary angioplasty was assessed in 52 patients by intravenous digital subtraction ventriculography. After injection of contrast into the right atrium ventriculograms were obtained before and during balloon inflation. In 37 patients they were also obtained after the procedure.

Arrival-time analysis was applied to intravenous digital aortograms. A single static image was produced by representing the time to 90% maximum density for each image pixel on a grey scale. Arrival-time analysis confirmed the diagnosis in four of six patients with aortic dissection that had been shown by image subtraction.

Fifty-five consecutive cases of ventricular septal rupture following myocardial infarction were reviewed in order to ascertain clinical and haemodynamic determinants of in-hospital mortality. Factors associated with a poor prognosis included clinical evidence of a poor haemodynamic state or biochemical evidence of impaired renal function. Twenty-six patients managed before 1982 (group 1) were then compared with 29 managed subsequently (group 2) when a policy of earlier surgical intervention had been adopted.

Eleven patients presenting with the clinical diagnosis of suspected aortic dissection underwent intravenous digital subtraction aortography. In nine patients digital subtraction angiography (DSA) was performed as the investigation of first choice. In five of these the diagnosis was confirmed with this technique alone and surgical repair was undertaken without further investigation.

The interrelation between the loudness of the first heart sound, the time interval from the Q wave to the onset of the first heart sound (QM1), and the mitral valve closure rate was studied in nine patients presenting with left atrial myxomata. In seven patients the first heart sound was loud preoperatively and was associated with delayed mitral valve closure. After removal of the myxoma the onset of mitral valve closure returned towards normal, the mitral valve closure rate was reduced, and the first heart sound became softer.

We have shown previously that experimental autogenous vein grafts activate platelets for up to four months following operation. In the present study, animals were treated with middle dose ASA (10 mg X kg-1 X 24 h-1) plus dipyridamole (8 mg X kg-1 X 24 h-1) and followed for 8 months. Despite this treatment, platelets were activated by the vein graft for up to four months after operation but not after this time.

A new animal model was used to study the effect of autogenous vein grafts on platelet function and to follow the time course of any changes. Reversed external jugular vein was grafted to the ipsilateral carotid artery in lop-eared rabbits. The platelet activation index (PAI), a measure of platelet responsiveness to adenosine diphosphate, was used to measure the effect of the graft on platelets at set postoperative times and the results compared with pre-operative values.
