Article Synopsis
  • Cardiac amyloidosis is a serious condition where abnormal amyloid fibrils accumulate in the heart, causing it to function poorly. It's mainly classified into two types: immunoglobulin light chain amyloidosis and transthyretin amyloidosis (ATTR), which comes in non-hereditary and hereditary forms.
  • The rise in cases of ATTR cardiac amyloidosis is partly due to better diagnostic methods, and survival rates are improving thanks to new treatment options like tafamidis and emerging therapies currently in trials.
  • New treatments such as acoramidis, eplontersen, vutrisiran, patisiran, and an experimental antibody NI006 are being tested to enhance cardiac function, with results expected in a few

: Automated methods for the analysis of myocardial perfusion studies have been incorporated into clinical practice, but they are currently used as adjuncts to the visual interpretation. We aimed to investigate the role of automated measurements of summed stress score (SSS), summed rest score (SRS), and summed difference score (SDS) as long-term prognostic markers of morbidity and mortality, in comparison to the prognostic value of expert reading. : The study was conducted at the Nuclear Medicine Laboratory of the University of Thessaly, in Larissa, Greece.

Cardiotoxicity may present as (pulmonary) hypertension, acute and chronic coronary syndromes, venous thromboembolism, cardiomyopathies/heart failure, arrhythmia, valvular heart disease, peripheral arterial disease, and myocarditis. Many of these disease entities can be diagnosed by established cardiovascular diagnostic pathways. Nuclear medicine, however, has proven promising in the diagnosis of cardiomyopathies/heart failure, and peri- and myocarditis as well as arterial inflammation.

: Myocardial perfusion imaging (MPI) has an important role in the non-invasive investigation of coronary artery disease. The interpretation of MPI studies is mainly based on the visual evaluation of the reconstructed images, while automated quantitation methods may add useful data for each patient. However, little evidence is currently available regarding the actual incremental clinical diagnostic performance of automated MPI analysis.

Article Synopsis
  • CZT-based detectors are more sensitive than conventional Anger-MPI in diagnosing coronary artery disease (CAD) during myocardial perfusion imaging (MPI), but they show lower specificity and accuracy.
  • The study involved 83 patients who underwent stress/rest tests with both CZT and conventional SPECT cameras, with 15 international readers evaluating the images for comparison.
  • Results indicated that while CZT-MPI had better image quality and higher sensitivity (87.5% vs. 62.5%), it also had significantly lower specificity (40% vs. 100%), highlighting the need for additional training for less experienced readers.

In daily clinical practice, clinicians integrate available data to ascertain the diagnostic and prognostic probability of a disease or clinical outcome for their patients. For patients with suspected or known cardiovascular disease, several anatomical and functional imaging techniques are commonly performed to aid this endeavor, including coronary computed tomography angiography (CCTA) and nuclear cardiology imaging. Continuous improvement in positron emission tomography (PET), single-photon emission computed tomography (SPECT), and CT hardware and software has resulted in improved diagnostic performance and wide implementation of these imaging techniques in daily clinical practice.

White adipose tissue (WAT) thermogenic activity may play a role in whole-body energy balance and two of its main regulators are thought to be environmental temperature (T) and exercise. Low T may increase uncoupling protein one (UCP1; the main biomarker of thermogenic activity) in WAT to regulate body temperature. On the other hand, exercise may stimulate UCP1 in WAT, which is thought to alter body weight regulation.

With this summarized document we share the standard for positron emission tomography (PET)/(diagnostic)computed tomography (CT) imaging procedures in cardiovascular diseases that are inflammatory, infective, infiltrative, or associated with dysfunctional innervation (4Is) as recently published in the European Journal of Nuclear Medicine and Molecular Imaging. This standard should be applied in clinical practice and integrated in clinical (multicentre) trials for optimal standardization of the procedurals and interpretations. A major focus is put on procedures using [18F]-2-fluoro-2-deoxyglucose ([18F]FDG), but 4Is PET radiopharmaceuticals beyond [18F]FDG are also described in this summarized document.
